OBJECTIVE/BACKGROUND: Lower-extremity lymphedema is difficult to treat, especially after failing conservative management. Liposuction (also termed lipoaspiration or suction-assisted lipectomy) has been identified as a treatment option for patients who failed conservative management in lower-extremity lymphedema. This systematic review evaluates the clinical and imaging criteria used to determine candidacy for liposuction in adults with lower-extremity lymphedema.

METHODS: A systematic review was conducted in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. The review searched Embase, PubMed, and Cochrane Library from inception through April 2026. Six reviewers screened 3057 studies using Covidence software. Of the 74 full-text articles assessed, seven studies met inclusion criteria. Inclusion criteria were cohort studies with greater than 50 participants and systematic reviews containing extractable lower-extremity liposuction data in patients with lymphedema. Studies were excluded for wrong study design, wrong surgical procedure, or absence of extractable lower-extremity data. Outcomes of interest were limb volume reduction, cellulitis frequency, quality of life, functional status, and compression garment dependency. Evidence was synthesized narratively due to heterogeneity across studies.

RESULTS: Seven studies were included: 3 prospective cohort studies (n = 190), 2 retrospective cohort studies (n = 145), and 2 systematic reviews with quantitative synthesis. Five categories of candidacy criteria were identified: inadequate response to conservative management, International Society of Lymphology (ISL) clinical stage II to III, tissue composition assessment (including nonpitting edema test and magnetic resonance imaging), lymphedema etiology (primary vs secondary), and confirmed capacity for lifelong compression garment compliance. ISL stage II patients demonstrated significantly better volume reduction outcomes than stage III patients (P < .001). Secondary lymphedema responded more favorably than primary lymphedema (101% vs 79% volume reduction at 24 months). Volume reduction was sustained up to 96 months but required continuous compression therapy. Magnetic resonance imaging was the only imaging modality explicitly required as a formal candidacy criterion in any included study. Quality of life and functional status were inadequately assessed across all studies.

DISCUSSION: The evidence is constrained by the absence of randomized controlled trials evaluating liposuction candidacy criteria in the lower extremity. Nonpitting edema, ISL stage II to III disease with fibroadipose predominance and confirmed compression compliance are the most consistently supported criteria for liposuction candidacy in lower-extremity lymphedema. Prospective validation of candidacy algorithms and adoption of standardized patient-reported outcome measures are needed.
