BACKGROUND: Oral Submucous Fibrosis (OSMF) is a chronic, progressive, potentially malignant disorder characterized by fibrosis of the oral mucosa, resulting in restricted mouth opening, burning sensation, mucosal blanching, and reduced cheek flexibility. Effective management remains challenging, and newer regenerative therapies such as platelet-rich plasma (PRP) have shown promising results.
AIM: To compare the efficacy of intralesional PRP and triamcinolone acetonide in improving mouth opening, burning sensation, cheek flexibility, and mucosal colour in patients with OSMF.
MATERIALS AND METHODS: This prospective comparative clinical study included 30 patients with OSMF (Group I-III according to the Ranganathan classification). Participants were randomly allocated into two groups of 15 each. Group I received intralesional PRP prepared using a standardized double-spin centrifugation technique, while Group II received intralesional triamcinolone acetonide. Six weekly injections were administered along with adjunct physiotherapy. Clinical parameters were assessed at baseline, 1 week, 6 weeks, and 3 months. Statistical analysis included two-way mixed ANOVA, Friedman test, Mann-Whitney U test, and Fisher's exact test.
RESULTS: Both groups demonstrated significant improvement in all clinical parameters (p < 0.001). No significant intergroup difference was observed in mouth opening. PRP showed significantly greater reduction in burning sensation at 3 months (p < 0.001) and superior improvement in mucosal colour, whereas triamcinolone demonstrated greater improvement in cheek flexibility (p < 0.05).
CONCLUSION: Both PRP and triamcinolone acetonide were effective in the management of OSMF. PRP provided superior symptomatic relief and mucosal improvement, whereas triamcinolone demonstrated greater improvement in cheek flexibility.
