[Technical operation standards for modified Bacon procedure (2026 edition)].

In China, mid-to-low rectal cancers represent the majority of rectal cancer surgeries. Organ preservation, function preservation, and improvement of patients' quality of life are current directions in surgical development. The modified Bacon procedure, as an ultra-low extreme sphincter-preserving technique, offers advantages including no need for a diverting ileostomy, no auxiliary abdominal incision, and a low incidence of complications, leading to its increasingly widespread clinical application. Although the Expert consensus on modified Bacon procedure (2025 version) has clarified the definition and indications of this procedure, the lack of standardized surgical techniques has hindered its standardized adoption. To standardize the operational procedures of the modified Bacon operation, reduce complication rates, and guide postoperative care, the Colorectal Cancer Committee of the Chinese Medical Doctor Association, the Colorectal Cancer Committee of the China Anti-Cancer Association, and the NOSES Committee of the China Anti-Cancer Association convened experts in relevant fields to jointly develop the Technical operation standards for modified Bacon procedure (2026 edition) based on domestic and international literature and clinical practice experience. This consensus provides detailed descriptions of preoperative anal function assessment, bowel preparation, surgical platform selection, classification of tumor division methods, fixation techniques for the exteriorized bowel,classification and management of exteriorized bowel necrosis, prevention and management of anastomotic stricture, and perioperative nursing care, thereby providing technical standards for the standardized implementation of the modified Bacon procedure in China.
