BACKGROUND AND AIMS: Gallbladder perforation has been considered a contraindication to EUS-guided gallbladder drainage (EUS-GBD). We evaluated the feasibility of EUS-GBD using an electrocautery-enhanced lumen-apposing metal stent (EC-LAMS) in inoperable acute cholecystitis (AC) complicated by Niemeier type II perforation.
METHODS: We evaluated 19 consecutive high-risk surgical patients with moderate-to-severe AC and radiologic evidence of Niemeier type II perforation. The primary outcome was technical success. Secondary outcomes included clinical success and adverse events (AEs).
RESULTS: After excluding 5 patients who received alternative interventions, 14 underwent EUS-GBD using EC-LAMSs. Technical success was achieved in all cases (14 of 14), and clinical success was observed in 92.9% (13 of 14). During a median follow-up period of 314.5 days, overall AEs occurred in 35.7% of patients (5 of 14), with no severe or fatal AEs.
CONCLUSIONS: Niemeier type II gallbladder perforation with adjacent encapsulated collections may not represent an absolute contraindication to EUS-GBD in patients with AC.
