Concomitant Pulmonary and Tonsillar Tuberculosis.

We report the case of a 54-year-old man presenting with concomitant pulmonary and tonsillar tuberculosis. The coexistence of these two localizations poses a diagnostic challenge because of the nonspecific clinical manifestations and the potential confusion with malignant diseases. The diagnosis of pulmonary tuberculosis was confirmed by positive sputum GeneXpert MTB/RIF, while histopathological examination of the tonsillar lesion revealed non-caseating granulomatous inflammation, with no evidence of malignancy. The patient received the standard anti-tuberculosis therapy for six months, with complete clinical improvement and resolution of the pulmonary lesions at follow-up. This case highlights the importance of considering tuberculosis in patients presenting with concurrent pulmonary and tonsillar lesions while maintaining suspicion for malignancy because of the overlapping clinical presentation. Early recognition is essential to ensure prompt diagnosis and appropriate management.
