OBJECTIVES: Lower eyelid pockets and tear troughs not only affect appearance but may also lead to a tired and aging facial look. At present, the main method for treating lower eyelid pockets and lacrimal troughs depression is surgical treatment. Although the traditional lower eyelid bags removal surgery can significantly reduce the swelling of the lower eyelid bags, it cannot effectively improve the lacrimal troughs. For severe lacrimal troughs depression, orbital septal fat release and filling surgery is the first choice. However, if the fixation of orbital septal fat is unstable during the operation or the sutures fall off, it often leads to the recurrence of lacrimal troughs depression and the aggravation of the lower eyelid pockets. To address this issue, the authors invented a method of fixing orbital septal fat through the oral approach for the treatment of lower eyelid pockets and lacrimal troughs depression.
METHODS: A total of 54 patients were included in this study from January 2024 to January 2025, among whom 46 were females and 8 were males. The age ranged from 37 to 56 years old, with an average of (47.43 ± 5.27) years old. All the patients adopted the surgical method of fixing the orbital septum fat through the oral approach for the treatment of lower eyelid pockets and lacrimal troughs depression. The therapeutic effect of this surgical method was evaluated by the satisfaction rate of patients after the operation and the incidence of complications during the follow-up period.
RESULTS: The average follow-up time for patients in this study was (17.7 ± 3.54) months. The satisfaction rate of patients with fixation of orbital septum fat through the oral approach for the treatment of lower eyelid pockets and lacrimal troughs depression was 96.4%. Postoperative complications occurred in 4 patients after the operation, with an incidence rate of 7.4%.
CONCLUSION: The oral approach for fixing orbital septal fat in the treatment of lower eyelid bags and lacrimal troughs depression has a remarkable effect and is a reliable surgical method for improving eyelid bags and lacrimal trough.
