BACKGROUND: The presence of multiple intracranial aneurysms is not infrequent. Surgical strategies to manage this condition vary, including staged procedures and multiple craniotomies.
OBSERVATIONS: A 40-year-old female presented with headache and syncope and was found to harbor 4 aneurysms involving the pericallosal, anterior choroidal, and bilateral middle cerebral arteries. Given her young age and the presence of multiple aneurysms across distinct vascular territories, microsurgical clip ligation via a single incision and dual craniotomies was selected over endovascular treatment.
LESSONS: Dual ipsilateral craniotomies through a single incision can provide safe and effective corridors for microsurgical treatment of multiple aneurysms across distinct vascular territories. https://thejns.org/doi/10.3171/CASE2633.
