Assessing diagnostic accuracy of onchocerciasis rapid diagnostic tests using Bayesian latent class models.

BACKGROUND: Onchocerciasis elimination programs increasingly rely on rapid diagnostic tests (RDTs) that detect antibodies to the Ov16 antigen, yet the performance of currently available rapid tests remains uncertain. The commercially available SD BIOLINE Ov16 RDT has shown inconsistent sensitivity when used on whole blood and does not consistently meet international performance thresholds for mapping or decisions to stop mass drug administration (MDA). Two novel onchocerciasis RDTs have been developed, DDTD RDT and GADx RDT, but their diagnostic accuracy has not been fully evaluated using meth

Potential health impacts and costs of active case finding guided by Mycobacterium tuberculosis immunoreactivity survey results in Blantyre, Malawi: A mathematical modeling study.

BACKGROUND: Active case finding (ACF) for tuberculosis (TB) can reduce transmission, yet efficient targeting requires high-quality surveillance data. We investigated the potential costs and impact of targeted ACF, guided by local estimates of the annual risk of TB infection (ARTI) derived from Mycobacterium tuberculosis (Mtb) immunoreactivity survey data in children aged <5 years. METHODS AND FINDINGS: Using mathematical models parameterized with local data, we compared three case-finding approaches across 33 urban neighborhoods in Blantyre, Malawi: passive case finding (PCF) only; PCF with u

Survival and Reoperation After Intralesional Procedures Versus Resection and Reconstruction for Renal Cell Carcinoma Bone Metastases.

BACKGROUND: The management of renal cell carcinoma (RCC) long-bone metastases often involves surgery with either resection and reconstruction (R&R) or intralesional (IL) procedures. Some literature suggests that R&R improves survival and reduces the risk of recurrence and reoperation, while IL procedures offer lower surgical morbidity and faster recovery. We compared survival and reoperation outcomes following IL versus R&R procedures for RCC long-bone metastases treated at our institution. METHODS: We retrospectively reviewed patients with histologically confirmed RCC long-bone metastases tr

A US-Based National Virtual Cohort of People Living With HIV at Risk for Viral Nonsuppression: Protocol for the EPI-LoVE Prospective Cohort Study.

BACKGROUND: Despite advances in HIV treatment, many people living with HIV fail to achieve or maintain viral suppression. Individuals who experience interruptions in HIV care, adherence challenges, and viral nonsuppression are often underrepresented in clinic-based cohort studies. Virtual cohort methodologies offer opportunities to recruit, engage, and retain these populations while integrating longitudinal behavioral, biomarker, and digital engagement data needed to characterize dynamic changes in HIV care engagement and viral suppression. OBJECTIVE: The Exploring, Predicting, and Intervenin

Determining the Causal Effect of Neurotrophic Tropomyosin Receptor Kinase Gene Fusions on Survival Prognosis for Adult Patients With Advanced Cancers.

PURPOSE: Uncertainty about the prognostic significance of neurotrophic tropomyosin receptor kinase (NTRK) fusions complicates clinical decisions and interpretation of survival outcomes in oncology care. Drawing on the largest cohort of NTRK fusion-positive (NTRK+) patients, we establish the prognostic effect of NTRK fusions in adults with advanced cancers. METHODS: We used a large real-world clinicogenomic research database from the United States to emulate a pragmatic randomized trial that determines the causal effect of harboring an NTRK gene fusion (NTRK+) on survival prognosis for adult p

AI-Driven and Automated Systems for Continuous Oxygen Saturation Monitoring in Long-Term Oxygen Therapy: Systematic Review.

BACKGROUND: Long-term oxygen therapy (LTOT) improves outcomes in selected patients with severe chronic hypoxemia, but conventional LTOT uses fixed oxygen flow prescriptions that may not reflect changing needs during activity, sleep, or exacerbations. AI and automated oxygen systems may support continuous peripheral capillary oxygen saturation (SpO2) monitoring, signal-quality assessment, and adaptive oxygen titration. Evidence comparing AI-driven and non-AI automated approaches across performance, clinical readiness, LTOT applicability, and equity remains limited. OBJECTIVE: This systematic r

Detection of Acute Hepatitis C Outbreaks in California Using SaTScan: Statewide Spatiotemporal Analysis.

BACKGROUND: Acute hepatitis C virus outbreaks in California are identified by local health jurisdictions through the investigation of cases reported by laboratories and health care providers to the state's public health surveillance system. However, acute hepatitis C cases are widely underreported, limiting timely outbreak detection and early intervention. SaTScan (Space and Time Scan Statistics) has been proposed as a valuable tool to better detect disease clusters that may be missed by traditional surveillance, even when the disease is underreported, but it is not routinely used for acute he

Increased performance in tutoring systems during COVID-19 in Uruguay and Germany.

BACKGROUND: COVID-19-related school closures globally disrupted students' education and were followed by declines in academic performance in many contexts. At the same time, some studies reported performance increases within adaptive learning software during school closures. Yet, this evidence stems from a limited range of countries (e.g., Germany), leaving unclear whether these findings extend to other parts of the world. AIM: We sought to evaluate whether performance trajectories within intelligent tutoring systems (ITSs) for learning mathematics during COVID-19-related school closures were
