Long-term safety of steroid-sparing immunosuppression in uveitis.

PURPOSE OF REVIEW: Corticosteroid-sparing immunosuppression is standard for chronic noninfectious uveitis, yet its long-term safety remains a dominant clinical concern. We synthesize the contemporary evidence on long-term safety, common adverse effects that drive treatment discontinuation, and how those effects are managed across various medication classes. RECENT FINDINGS: The largest cohort data show no statistically significant excess in overall or cancer mortality for antimetabolites, calcineurin inhibitors, or antitumor necrosis factor agents, whereas alkylating agents remain the princip

Global Burden of Elevated LDL-C: Findings From the Global Burden of Disease Study 2023.

IMPORTANCE: Elevated low-density lipoprotein cholesterol (LDL-C) is a modifiable risk factor for cardiovascular disease, the leading cause of premature death worldwide. Assessing the LDL-C-related burden is critical for guiding prevention and treatment strategies. OBJECTIVES: To estimate the global, regional, and national burden of ischemic heart disease and ischemic stroke attributable to elevated LDL-C (relative to 35-54 mg/dL) from 1990 to 2023 and to quantify the contributions of population growth, aging, risk-deleted burden, and exposure changes to burden trends. Unilateral Iris Mass Lesions in Presumed Ocular Tuberculosis.

Iris mass lesions are uncommon and often raise suspicion of intraocular malignancy. However, infectious and inflammatory conditions, particularly ocular tuberculosis (TB), can closely mimic neoplastic lesions and pose significant diagnostic challenges, especially in the absence of systemic disease. A 36-year-old male presented with unilateral redness, pain, and reduced vision in the right eye for 15 days. Ocular examination revealed granulomatous anterior uveitis with two lobulated iris mass lesions and 360° posterior synechiae. Ultrasound biomicroscopy and anterior segment optical coherence t

Optic Neuropathy in Neurosarcoidosis Masquerading as Optic Nerve Sheath Meningioma: A Case Report and Literature Review.

Sarcoidosis is a multisystem granulomatous disorder that often mimics inflammatory, demyelinating, or neoplastic neuro-ophthalmic conditions. We describe a 39-year-old woman with two episodes of progressive visual loss in the right eye over 6 months, with partial responsiveness to steroid. Examination revealed optic disc edema and mild proptosis. Extensive evaluation for demyelinating, autoimmune, infectious, and neoplastic causes was unremarkable. A diagnosis of optic nerve sheath meningioma was initially made at a peripheral center based on the presence of a "tram-track" appearance along the

Indocyanine green angiography in uveitis.

Indocyanine green angiography (ICGA) represents an indispensable imaging modality for the precise evaluation and management of posterior uveitis by allowing to explore the choroidal compartment. ICGA leverages near-infrared fluorescence and extensive protein binding to provide an unhindered, dynamic assessment of the choriocapillaris and choroidal stroma. This review explores the principles, standard protocols, and clinical applications of ICGA in posterior uveitis. It details ICGA's critical role in classifying and diagnosing primary inflammatory choriocapillaropathies, like multiple evanesce

Isolated Unilateral Optic Neuritis Secondary to Presumed Ocular Tuberculosis in an Immunocompetent Adult.

This case report describes tuberculous optic neuritis as the presenting feature of tuberculosis (TB) in an immunocompetent adult. A 23-year-old inmate presented with four days of painless left eye vision loss without systemic TB symptoms. Visual acuity (VA) was hand movement with relative afferent pupillary defect. Left eye light brightness was reduced, but no red desaturation. Funduscopy showed a hyperaemic optic disc and blurred disc margin, with dilated, tortuous vessels. Visual field revealed a central scotoma. Systemic examination was unremarkable. Investigation showed a normal chest X-ra

Global burden of adverse effects of medical treatment from 1990 to 2021: a Global Burden of Disease Study 2021.

BACKGROUND/AIMS: This study aims to evaluate the global burden of adverse effects of medical treatment (AEMT) using data from the Global Burden of Disease Study (GBD) 2021. METHODS: Data were extracted from the GBD 2021, covering 204 countries/territories from 1990 to 2021. AEMT was defined using ICD-9 and ICD-10 codes, encompassing complications from medical procedures, treatments, or healthcare exposures. Estimates were categorized into fatal and non-fatal outcomes and stratified by age, sex, year, and covariates, including the Socio-demographic Index (SDI). Mortality-incidence ratios (MIRs

Burden of chronic respiratory disease in Asia, 1990-2023: a systematic analysis for the Global Burden of Disease Study 2023.

BACKGROUND: Chronic respiratory diseases are an important global issue, particularly in Asia, where burden patterns vary widely across countries. With more than half the world's population living in Asia, understanding the national and regional burden of chronic respiratory diseases is essential; however, research on this area remains inadequate. We aimed to investigate the burden of chronic respiratory diseases in Asia at national and regional levels, and to identify key risk factors. METHODS: The Global Burden of Diseases, Injuries, and Risk Factors Study 2023 provides estimates for assessi

Lupus Retinopathy Mimicking Ocular Tuberculosis: A Diagnostic Dilemma.

Systemic lupus erythematosus (SLE) or Lupus is often called the "great mimicker" due to its ability to mimic other systemic disorders. SLE is a multisystem autoimmune disorder with varied manifestations, including ocular involvement. Retinal vascular occlusions are an uncommon but potentially vision-threatening presentation of lupus retinopathy. When coexisting systemic features mimic infectious etiologies such as tuberculosis (TB), diagnosis becomes challenging, delaying appropriate immunosuppressive therapy. We report a diagnostically complex case of a 22-year-old female who presented with s

Recurrent Episcleritis Revealing Pulmonary Tuberculosis in an Adolescent: A Case Report and Literature Review.

INTRODUCTION: Tuberculosis (TB) can affect the eye through direct infection or immune mechanisms. While ocular TB most often presents as uveitis or scleritis, episcleritis is an uncommon manifestation and can be mistaken for an idiopathic or autoimmune disease. CASE PRESENTATIONS: A 17-year-old female patient presented at our hospital with recurrent episcleritis. The first two flare-ups were mild and responded well to conservative treatment; therefore, no further investigation was pursued at the time. However, during the third episode, routine baseline workup revealed a mildly elevated erythr

The Diverse Spectrum of Presumed Ocular Tuberculosis Including Optic Neuropathy in a Tuberculosis-Endemic Region: A Case Series from India.

Ocular tuberculosis (TB) presents with diverse clinical manifestations and remains a diagnostic and therapeutic challenge, particularly in TB-endemic regions. In addition, antitubercular therapy (ATT), especially ethambutol and linezolid, may contribute to treatment-related optic neuropathy and visual morbidity. This case series aimed to describe the clinical manifestations of presumed ocular TB and to assess the burden of ATT-induced optic neuropathy during the study duration. Diagnosis was based on clinical features, tuberculin skin test (Mantoux test) or interferon-gamma release assay (e.g.

Distinction between allergic rhinitis alone and allergic rhinitis + asthma in MASK-air® real-world studies; a ARIA-WAO task force review.

The recent ARIA-MeDALL hypothesis proposed in 2023 that allergic rhinitis (AR) alone and allergic rhinitis and asthma (A) multimorbidity (AR + A) represent 2 distinct diseases. To improve the knowledge on this topic, we have gathered data from real-world studies using MASK-air®. According to our analyses: (i) An "extreme allergy phenotype" [A + AR + C, Conjunctivitis] was confirmed and found to be more severe (symptoms and work productivity) than single diseases alone. (ii) Patients with AR + A required more rhinitis medications, and had more severe VAS levels for nasal and ocular symptoms tha

Case Report: Presumed ocular tuberculosis masquerading as acute retinal necrosis: the diagnostic and therapeutic journey of an elderly female.

BACKGROUND: Acute retinal necrosis (ARN) is typically caused by varicella-zoster or herpes simplex virus, but Mycobacterium tuberculosis can present an identical clinical picture. Distinguishing tuberculous uveitis (TBU) from ARN is challenging when etiological tests are unavailable. CASE PRESENTATION: A 78-year-old woman presented with rapid visual loss (hand motion) in the left eye. Examination revealed elevated intraocular pressure, mutton-fat keratic precipitates, severe anterior chamber and vitreous inflammation, retinal arteritis, and peripheral necrotic lesions, which met the 2015 Japa

Phlyctenular keratoconjunctivitis with viral triggers.

Phlyctenular keratoconjunctivitis (PKC) goes beyond limbal nodules. This pediatric ocular surface condition caused by delayed-type hypersensitivity to microbial antigens. The trigger is context-dependent: Mycobacterial antigens in tuberculosis-endemic areas; staphylococcal eyelid disease and rosacea in high-income areas. Although classically bacterial-driven, virus-associated presentations like herpes simplex virus (HSV)-linked phlyctenular disease, pediatric PKC during acute coronavirus disease 2019 (COVID-19) infection, and molluscum contagiosum-driven keratoconjunctivitis suggest the same a
