Metformin reduces intracellular M. tuberculosis replication and inflammation in vitro: A potential host-directed therapy for ocular tuberculosis?

Ocular tuberculosis (TB) remains a leading cause of infectious uveitis in TB-endemic settings. Antitubercular therapy, with or without adjunctive corticosteroids or immunosuppressive agents, is the mainstay of treatment; however, more rapid resolution of intraocular inflammation is desirable to reduce visual morbidity. This preliminary study investigates metformin as a potential host-directed therapy (HDT) for ocular TB through in silico prediction and validation using in vitro disease models. In silico drug discovery was conducted using a bioinformatics platform. In vitro assays were performe

Sudden monocular vision decrease caused by isolated choroidal tuberculoma during anti-tuberculosis treatment: a case report.

Mycobacterium tuberculosis primarily causes pulmonary diseases; however, it can also affect extrapulmonary organs, including the eyes. The choroid, as the intraocular tissue with the richest blood supply, is one of the most susceptible structures to intraocular tuberculosis. Abnormal ocular symptoms that occur during systemic anti-tuberculosis treatment (ATT) can easily be misdiagnosed as drug side effects or treatment failure. Abruptly discontinuing a specific medication could lead to the recurrence of extraocular tuberculosis and irreversible vision loss. This case study focuses on a young f

Headache in a case of tuberculosis: think beyond meningitis-a case report.

BACKGROUND: Intracranial hypertension may be idiopathic (IIH) or due to some underlying causes, such as an intracranial space-occupying lesion or drug induced. CASE PRESENTATION: A 14-year-old South Asian male was being treated for MDR-TB with anti-tubercular therapy (including levofloxacin) when he developed a headache lasting 8 days. Examination revealed bilateral papilledema. The CSF study was unremarkable. MRI revealed left transverse and sigmoid sinus stenosis. A probable diagnosis of levofloxacin-induced intracranial hypertension was made, and the drug was stopped. The symptoms resolved

Genome-wide association study reveals a novel tuberculosis susceptibility locus in multiple East Asian and European populations.

BACKGROUND: Tuberculosis (TB) continues to be a leading cause of morbidity and mortality worldwide. Although numerous genome-wide association studies (GWAS) have explored TB susceptibility across various ethnic groups, multi-population replication of findings has been very limited, particularly outside the HLA region, and a significant portion of TB heritability remains unexplained. METHODS: We conducted GWAS in the Singapore Chinese and Vietnamese, followed by a comprehensive meta-analysis incorporating 4 independent East Asian datasets (N = 11,841 cases; N = 197,373 controls). The transfera

From systemic disease to the eye: Why senescence deserves attention in ocular infections.

The global increase in life expectancy has led to a growing elderly population, bringing new challenges in managing infectious diseases. Advancing age is accompanied by progressive dysfunction of both innate and adaptive immunity, resulting in impaired responses to pathogens and elevated morbidity and mortality. Cellular senescence, a state of permanent cell-cycle arrest with extensive molecular and phenotypic changes, including chromatin remodelling and the senescence-associated secretory phenotype (SASP), has emerged as a key factor in this process. Though initially protective as a tumor-sup

Effect of Anti-Tubercular Therapy Duration (6 Months versus ≥9-Months) on Treatment Outcomes in Serpiginous-Like Choroiditis: A Multicenter Retrospective Observational Study.

PURPOSE: Serpiginous-Like Choroiditis (SLC) is considered to be a subtype of tuberculosis associated uveitis and has been treated with either 6 or 9 months of Anti Tuberculosis therapy (ATT). However, ATT is associated with several adverse effects including an increased risk of ethambutol associated optic neuropathy. The aim of this study was to evaluate the effect of anti-tubercular therapy (ATT) duration (6 months vs ≥ 9 months) on treatment outcomes in SLC. METHODS: A retrospective observational study of patients with SLC was conducted from two campuses of a tertiary care eye institute ide

A rapid molecular assay for the detection of Mycobacterium avium using loop-mediated isothermal amplification (LAMP) combined with a lateral flow biosensor.

Mycobacterium avium is a slow-growing nontuberculous mycobacterium (NTM) capable of infecting humans and other animals. As an opportunistic pathogen, it can cause severe diseases such as pulmonary infections and lymphadenitis in immunocompromised individuals. In this study, we evaluated a loop-mediated isothermal amplification (LAMP) assay combined with a lateral flow biosensor (LFB) for the rapid detection of Mycobacterium avium.LAMP is a simple, rapid, and isothermal nucleic acid amplification method, and when combined with LFB, its results can be directly observed with the naked eye. We des

Otopathology of Pediatric Acute Tuberculous Meningitis-Case Series.

Tuberculous meningitis (TBM) is a severe presentation of the Mycobacterium tuberculosis that can cause sensorineural hearing loss (SNHL) similarly to other etiologies of meningitis. The pathophysiology for this group remains unclear. We report a series with 3 temporal bones of infants who died from TBM, showing acute damage to the inner ear, such as fibrosis of the cochlea, organ of Corti atrophy, neuronal loss and internal auditory canal with abundant pus cells, predominantly mononuclear. The cochlear aqueduct, endolymphatic sac/duct and Rosenthal's canal appear to be routes of dissemination

Bilateral granulomatous uveitis mimicking tuberculosis: an atypical ocular presentation of biopsy-proven disseminated histoplasmosis.

Ocular histoplasmosis typically presents as presumed ocular histoplasmosis syndrome, characterised by chorioretinal scars and peripapillary atrophy without intraocular inflammation. Active inflammatory presentations are exceedingly rare and may closely mimic tuberculosis in endemic regions, posing significant diagnostic challenges. We report a middle-aged immunocompetent male farmer who presented with acute bilateral visual loss, redness and floaters. Examination revealed bilateral granulomatous anterior uveitis with dense vitritis with mutton-fat keratic precipitates, dense vitritis and 4+ ant

Global, regional, and national burden of tuberculosis and multidrug-resistant tuberculosis by HIV status, 1990-2023: a systematic analysis for the Global Burden of Disease Study 2023.

BACKGROUND: Tuberculosis (TB) is the leading global cause of death from a single infectious agent. Recent reductions in global health funding have threatened TB control, making comprehensive assessment of TB, HIV-related TB, and drug-resistant TB burdens before these disruptions essential for shaping effective responses. The WHO End TB Strategy sets targets of a 95% reduction in TB deaths and a 90% reduction in TB incidence between 2015 and 2035. Using results from the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2023, this study aims to assess the burden of TB and multidr
