Two contrasting cases of adrenal insufficiency in adults: a case report.

BACKGROUND: Adrenal insufficiency (AI) is a clinical syndrome characterized by insufficient secretion of glucocorticoids and/or mineralocorticoids. The incidence of AI is low, and its clinical manifestations are non-specific, including fatigue, generalized weakness, nausea, and weight loss. Consequently, the condition can easily be misdiagnosed or overlooked. If not treated promptly, it may progress to a life-threatening adrenal crisis. This paper reports one case of primary and one case of secondary adrenal insufficiency, aiming to enhance clinical vigilance regarding this disease through a c

Diagnosing hypocortisolism in disguise: clinical insights from delayed-onset endocrine disorders.

BACKGROUND: Adrenal insufficiency is characterized by inadequate cortisol production, with or without aldosterone and adrenal androgen precursor deficiencies. This can make it difficult for the body to respond to stress and maintain essential functions. The symptoms of adrenal insufficiency are often non-specific, leading to delayed diagnosis and frequent misinterpretation. CASE ILLUSTRATION: We report three cases of adrenal insufficiency: (1) a 61-year-old male with autoimmune polyendocrine syndrome II (cortisol 4.76 μg/dl baseline →10.6/12.2 post-adrenocorticotropic hormone; thyroid-stimula

The impact of bacterial and viral diseases on dromedary camel (Camelus dromedarius) welfare: a comprehensive review.

Dromedary camels (Camelus dromedarius) are vital to food security, livelihoods and cultural identity in arid and semi-arid regions of Africa, the Middle East and South Asia, with global populations exceeding 35-40 million and continuing to rise as climate change favors livestock adapted to harsh environments. Despite their expanding roles in meat and milk production, transport, tourism and racing, camel health and welfare remain comparatively underexplored, particularly regarding infectious diseases. This review synthesizes current knowledge on major bacterial infections, including brucellosis

Pathological findings in 61 free-ranging leopards (Panthera pardus) from the Greater Kruger National Park, South Africa. Part Two: non-parasitic conditions.

Few reports on diseases in free-ranging leopards (Panthera pardus) are available. A retrospective review of tissues from free-ranging leopards from the Greater Kruger National Park, South Africa, that were submitted for histological examination was conducted. Non-parasitic lesions were recorded in 61 free-ranging juvenile to elderly leopards. They had died or been euthanized between 1998 and 2023. Parasitic conditions in these leopards have been reported separately. Traumatic injury was common (41%). Respiratory tract conditions included pneumoconiosis (36%) and tuberculosis (23%), presumed to

Adrenal Tuberculosis Presenting as Adrenal Crisis: A Rare Presentation of Common Disease.

We report a case of a 45-year-old man presenting with hypotension, hyponatremia, hyperkalemia, hypoglycemia and diffuse hyperpigmentation. Monitoring serum cortisol was 0.5 μg/dL, and the computerized tomography (CT) scan showed bilateral adrenal enlargement, consistent with primarily adrenal insufficiency (AI) likely due to adrenal tuberculosis (TB). There was no active pulmonary TB. This case underscores the diagnostic delay and management challenges of TB-related AI in resource-limited settings. Even today, clinicians should remain aware of AI due to TB in resource-limited settings.

The overlooked burden: anxiety and depression in patients with tuberculosis.

BACKGROUND: Tuberculosis (TB) is a major global cause of infectious disease-related morbidity and mortality. Beyond its physical burden, TB is associated with significant psychological distress. Anxiety and depression are highly prevalent among TB patients but often remain underrecognized and undertreated, despite their negative impact on treatment adherence, disease outcomes, and quality of life. OBJECTIVE: This review summarizes current evidence on the prevalence, biological mechanisms, treatment-related factors, and psychosocial determinants of anxiety and depression in patients with TB, a

Probable Primary Adrenal Insufficiency Associated With Granulomatous Adrenal Calcifications, Non-tuberculous Mycobacterial Ileitis, and Crohn's Disease: A Case Report.

Granulomatous adrenal involvement is an uncommon cause of primary adrenal dysfunction, while intestinal non-tuberculous mycobacterial (NTM) infection and Crohn's disease share substantial clinical, endoscopic, radiological, and histopathological overlap. The coexistence of these entities represents an exceptional diagnostic challenge, and epidemiological data regarding their association are currently lacking. We report the case of a 61-year-old man who presented with progressive cutaneous hyperpigmentation. Computed tomography demonstrated bilateral adrenal calcifications, calcified lymph node

Ischaemic stroke associated with Takayasu arteritis and latent tuberculosis: a diagnostic and therapeutic challenge.

A man in his early 40s presented late with right-sided weakness and dysarthria. MRI showed an infarct in the posterior limb of the left internal capsule in the anterior choroidal artery territory. CT angiography of the head and neck showed no large-vessel occlusion, significant cervicocephalic stenosis or dissection. Given his young age, limited vascular risk factors apart from hypertension and systemic vascular clues, further assessment revealed pretreatment CT angiographic evidence of large-vessel arteritis involving the abdominal aorta and branch vessels. Rheumatology favoured Takayasu arte

Efficacy and Safety of Corticosteroids in Tuberculous Meningitis: Systematic Review, Meta-Analysis, and Meta-Regression.

BACKGROUND: Tuberculous meningitis (TBM) carries a case fatality rate of up to 50% in resource-limited settings despite standard anti-tuberculosis therapy. Adjunctive corticosteroids are widely recommended, yet their effect on mortality and safety across diverse populations remains imprecisely defined. METHODS: We conducted a systematic review and meta-analysis of randomized and quasi-randomized controlled trials identified through PubMed, Cochrane Library, ClinicalTrials.gov, Scopus, and Google Scholar up to May 2025. The outcomes were all-cause mortality, gastrointestinal bleeding, visual a

Infectious diseases targeting the adrenal glands: A review.

100 years ago in the US, tuberculosis was the most common cause of primary adrenal insufficiency (AI) (Addison's disease). Autoimmune adrenalitis is now responsible for 80-90% of AI in this country and Europe; nevertheless today, in the US and certainly abroad, diverse infections can involve the adrenals in both the immunocompetent and immunocompromised and potentially produce AI. Such infections range widely: bacterial (eg, Mycobacterium tuberculosis, in particular), fungal (eg, histoplasmosis), viral and even protozoal (eg, severe Plasmodium falciparum malaria). Suspecting AI, especially if

An unusual cause of chronic recurrent vomiting and life-threatening hyperkalemia: primary adrenal insufficiency due to probable bilateral adrenal tuberculosis.

BACKGROUND: Adrenal tuberculosis remains a leading cause of primary adrenal insufficiency in tuberculosis-endemic regions. Its clinical presentation is often insidious and non-specific, leading to significant diagnostic delays. Gastrointestinal symptoms, though common in adrenal insufficiency, may dominate the clinical picture and direct attention away from the underlying endocrine disorder. CASE SUMMARY: A 40-year-old Ethiopian woman presented with a five-month history of intermittent abdominal pain and recurrent non-bloody vomiting. Associated symptoms included myalgia, arthralgia, intermit

The evidential role of economic models in legal proceedings.

A complex trauma history is progressively detrimental and interferes with education in emerging adults. Instead of completing education at a higher level where fewer cases are found, previous studies have mostly targeted educational failure at the undergraduate level or below. A shift to a more positive view of the future for complex trauma survivors needs to be explored. A study of emerging adult graduate student complex trauma survivors helps understand the possibility of posttraumatic growth. This article presents a case study of a 24-year-old male who completed his graduate studies despite

Deep fungal co-infection with Trichosporon asahii and Candida parapsilosis in an immunocompetent individual: a case report and review of literature.

Deep fungal infections are endemic to tropical and subtropical regions and can occur in individuals who are both immunocompetent and immunocompromised. Clinically, these infections may present as verrucous plaques, subcutaneous nodules, abscesses or ulcerations. Common causative organisms in immunocompetent hosts include Sporothrix schenckii, Madurella mycetomatis and Fonsecaea pedrosoi. However, emerging evidence suggests that Trichosporon asahii and Candida parapsilosis are increasingly implicated in these infections. A 64-year-old Cambodian man with a history of recently treated pulmonary t

Mycobacterium lentiflavum complicating insulin injection sites.

Non-tuberculous mycobacteria (NTM)/atypical mycobacteria are opportunistic pathogens ubiquitous in the environment. Recently, NTM cases causing skin and soft tissue infections are on the rise, owing to the improved diagnostic techniques as well as heightened number of procedures. A man in the mid 60s with diabetes, on insulin for the past 20 years, presented with multiple painful swellings, a few of them with pus discharge, over the insulin injection sites of arms, abdomen and thighs for the past 10 months. Local ultrasonography revealed anechoic collections surrounding increased vascularity i

Primary cutaneous squamous cell carcinoma of the neck mimicking tuberculous lymphadenitis.

BACKGROUND: Neck lymphadenopathy is frequently associated with infectious etiologies such as tuberculosis (TB); however, malignant conditions can present with similar clinical and radiological features, leading to diagnostic uncertainty and potential misdiagnosis. CASE PRESENTATION: We describe a 35-year-old immunocompetent man who presented with persistent fever and a progressively enlarging right neck mass. Initial fine-needle aspiration (FNA) revealed multinucleated giant cells consistent with granulomatous inflammation, prompting a presumptive diagnosis of tuberculous lymphadenitis and ini

Research progress of SPP1 in cancer regulation: Tumorigenesis, metastasis, tumor microenvironment, immunity and therapeutic targets (Review).

OBJECTIVES: Psoriatic arthritis (PsA) has been defined as a systemic, chronic, inflammatory arthritis, usually seronegative for rheumatoid factor (RF), associated with cutaneous psoriasis. The exact prevalence of PsA is unknown and its estimation has been difficult, partly due to the lack of a widely accepted classification criteria. Agreed and validated criteria will facilitate comparison between centres and different countries in the areas of epidemiology, outcome studies and therapeutic trials. A number of classification criteria have been published by Moll & Wright (M & W), Bennett's, Vase

Immuno-metabolic dysregulation in female genital tuberculosis: Multi-omics insights into infertility mechanisms and therapeutic targets.

Female genital tuberculosis (FGTB) is a major cause of infertility in regions where it is common, causing permanent damage to the reproductive tract. It mainly causes fibrosis and blockage of the fallopian tubes, directly hindering gamete movement. Meanwhile, FGTB leads to a decline in ovarian reserve and function by disrupting folliculogenesis in the ovaries, resulting in decreased levels of anti-Müllerian hormone (AMH). Mycobacterium tuberculosis also affects endometrial receptivity by inhibiting the STAT3/VEGF pathway and the imbalance of Th1/Th2 immune responses. This review explains these
