Symptomatic cervical disc herniation following a motor vehicle collision: return to work comparative study of workers' compensation versus personal injury insurance status.

BACKGROUND CONTEXT: Patients with approved workers' compensation injuries receive guaranteed compensation for the duration of their injury, whereas patients with personal injury claims are only compensated, if at all, at the time of a successful settlement or trial verdict at a time point distant from their injury. PURPOSE: This study compares the financial impact and loss of work patterns due to a workers' compensation (WC) claim or personal injury in patients with a symptomatic cervical disc herniation resulting from a motor vehicle collision. STUDY DESIGN: A prospective study of patients

Endovascular treatment of traumatic thoracic aortic rupture combined with diaphragmatic rupture--a case report.

The purposes of this report are to describe the immediate repair of a diaphragmatic rupture and subsequent endovascular treatment of a thoracic aortic rupture secondary to blunt chest and abdominal trauma and to discuss the outcome in the light of the trauma scores. A 29-year-old man was injured in a car collision. There were signs of a left diaphragmatic rupture and spleen injury. After urgent laparotomy, the diaphragm was repaired. The spleen appeared uninjured; however, there was a dissection with a transmural tear of the thoracic aorta, which was subsequently treated intraluminally by usin

Management of the burned trauma patient: balancing conflicting priorities.

PURPOSE: Approximately 5% of multiple trauma patients sustain concomitant burns. Complicated management issues arise in these patients as burn and trauma care often conflict. This study reviews 53 consecutive burned multiple trauma patients in order to examine common management conflicts and recommend appropriate compromises in caring for these difficult patients. PROCEDURE: A retrospective review of 53 consecutive burn patients with coincident trauma admitted to The Massachusetts General Hospital (MGH) from 1993-2001 was performed. FINDINGS: In the study period, 53 patients were admitted to

Under-reporting of motor vehicle traffic crash victims in New Zealand.

Our aim was to ascertain the extent of under-reporting of seriously injured motor vehicle traffic crash victims, as recorded by police in New Zealand, and to what extent this coverage was biased by crash, injury, demographic, and geographic factors. Hospital data and police records were linked using probabilistic methods. During 1995, less than two-thirds of all hospitalised vehicle occupant traffic crash victims were recorded by the police. Reporting rates varied significantly by age, injury severity, length of stay in hospital, month of crash, number of vehicles involved, whether or not a co

[Inline skating--high fracture risk. Two of three injured are boys and young men. Wrist fractures are most common].

In-line skating injuries have increased in recent years. Hospital based data from Umeå concerning 135 persons injured in in line skating collisions were analyzed. The highest yearly incidence of injury was found in males 10-19 years of age, at 1.7 per 1,000 inhabitants; the corresponding figure for females was 0.5. Two-thirds of incidents were caused by falls due to balance problems without the influence of any "external factor" such as rough road surfaces. No collisions with motor vehicles or other road users were registered. Nearly half of the injuries were fractures or dislocation injuries,

[Reconstrudive significance of stretch mark tears of the skin].

Stretchmark-like tears of the inguinal region are commonly referred to as typical lesions in pedestrian accidents when the victim is run over by a motorcar. These lesions can also be observed when a pedestrian is hit by a vehicle while being in an upright position. Stretchmark-like tears are due to hyperextension or excessive abduction of the hip joint. Characteristic morphological features are variable numbers of equally superficial and parallel tears of the skin following the skin's cleavage lines. Drivers of two-wheeled vehicles may exhibit inguinal stretchmark-like tears when they hit an o

Gender patterns in minor head and neck injuries: an analysis of casualty register data.

This study was designed to determine the age- and gender-specific incidences of moderate and minor head and neck injuries from emergency room casualty registers in two cities in northern Sweden. By contrast with findings in most studies on injuries, the incidence of minor and moderate head and neck injuries was higher among women than men in the 15-17- and the 39-53-year-old age group, and the incidence of neck injuries as high among women as among men in the 15-65-year-old age group. As compared to men, women more often sustained their head and neck injuries as pedestrians and as car passenge

Injuries resulting from bicycle collisions.

OBJECTIVE: To review all bicycle crash-related injuries reported to the Oregon Injury Registry for 1989 to compare patterns of injury and other features in adults vs children and adolescents. METHODS: A retrospective descriptive study was conducted using data from the Oregon Injury Registry. For 1989, this registry included all injury-related deaths and approximately 75% of hospital admissions of 24 hours or more for injury in the State of Oregon. Deaths, helmet use, neurologic injuries, and concurrent ethanol use were evaluated for all patients and for the two age groups. RESULTS: There wer

Cycling to school--a significant health risk?

OBJECTIVES: The risk of injury to children riding bicycles has been previously documented. However, the specific risk arising from the use of bicycles as a mode of transportation to and from school is unknown. This study examines the incidence of bicycle related injuries among school age children. METHODS: A comprehensive prospective injury registration system was established in Stavanger, Norway. Data were obtained from this system to identify bicycle related injuries occurring from 1990-3 to children aged 10-15. The incidence of injuries was computed for two groups of children: (1) children

Pancreatic injuries from blunt trauma.

Pancreatic injuries from blunt trauma are infrequent, and their diagnosis and management can be extremely difficult. Over the past 10 years we treated 13 patients with major pancreatic injuries from blunt trauma. Twelve had been involved in motor vehicle collisions, none of whom were wearing seat belts. One patient was injured in an assault. Only five patients had physical findings suggesting intra-abdominal injury. Serum amylase levels were elevated in seven of the nine patients (78%) in whom this test was obtained. Computed tomography scans demonstrated pancreatic injury in three of the four

Deaths from motor vehicle crashes: patterns of injury in restrained and unrestrained victims.

A time comparison study of motor vehicle crashes in Monroe County, New York, from 1983 to 1986 was completed. Using a database of police accident reports, hospital logs, and autopsy reports from the county coroner, the hospital and autopsy reports of 91 unrestrained and 27 restrained fatally injured victims were reviewed. The hypothesis was that safety belts do not change patterns of injury in fatally injured victims. Patient data, seating position, and direction of impact were the same for both groups, while ejections occurred only in the unrestrained group (19.8%). Injury Severity Score (ISS

Bicycle accidents often cause disability--an analysis of medical and social consequences of nonfatal bicycle accidents.

Social and medical consequences of 278 children and 264 adults injured in bicycle accidents and seen in two hospitals in Helsinki in 1985-86 were analyzed. Information was collected from patient records, by means of a special questionnaire and by telephone interview. A child outpatient required 1.7 and a child inpatient 3.0 physician visits on an average, while adults required 2.2 and 4.9 visits, respectively. The average duration of hospital stay was 8 days for hospitalized adults and 6 days for children. Rehabilitative care outside the hospital was received by 6% of the adult outpatients and

Elderly in the traffic environment: analysis of fatal crashes in northern Sweden.

Fatal crashes of the elderly (> or = 60 years; N = 379) in northern Sweden in the traffic environment, between 1977 and 1986, were investigated using autopsy and police reports. Males had double the death rate of females. The car occupants made up the largest category (43%) followed by pedestrians, pedalcyclists, and two-wheel-motorvehicle (TWMV) riders, but the risk of fatal injury per unit distance travelled was highest for pedestrians and pedalcyclists. Most car occupants were killed in vehicle-vehicle crashes, mostly in the daylight and at intersections or straight roads. Ice and/or snow (

Alcohol and drug use among motor vehicle collision victims admitted to a regional trauma unit: demographic, injury, and crash characteristics.

This study examined the incidence of alcohol and drugs in a sample of seriously injured motor vehicle collision victims, and differences related to pre-crash use of alcohol and/or other drugs on demographic variables, injury severity measures, and crash variables. The sample selected were all motor vehicle collision admissions to the Regional Trauma Unit at the Sunnybrook Health Science Centre in Toronto, Ontario, over a 37-month period (N = 854). Prospective demographic and injury-related information were collected from hospital charts, and crash data were collected from motor vehicle collisi

Accidents in the north. Some aspects on snowmobile accidents and moose-car collisions.

Snowmobile accidents and moose-car crashes are typical accidents in Northern Sweden. In this region there is about 1 snowmobile/10 inhabitants. The present paper combines previously published studies. The studies on snowmobile accidents are based on a material comprising all 61 fatally injured snowmobile drivers from the four northern counties of Sweden during the period 1973-1987. The helmet usage was analyzed in two clinical study populations including 200 injured from the county of Västerbotten from two periods 1979-1980 and 1985-86. Of the fatally injured (median age 32 years) 86% were dri

Restrained front seat car occupant fatalities--the nature and circumstances of their injuries.

The circumstances of a rural and urban sample of fatalities in vehicles less than six years old is described. The data originate from an in-depth, multidisciplinary study of accidents conducted in England since 1983. The sample is biased towards collisions that result in occupant injury, and this paper will concentrate on those accidents in which an occupant has been fatally injured. The initial police reporting of the fatalities included 11% of the deaths occurring from natural causes. Of the crash-related deaths with complete data, some 43% were frontal and lateral impacts, and they are anal

[Accident causes and prevention of injuries in Enduro-motorbike sport].

365 participants of the 64. Enduro-World Championship in 1989 reported 232 injuries within a 10-years-period of motorbike driving. The knee was most frequent (21.5 pc., mainly ligament ruptures) injured, followed by fracture of the clavicle (18.5 pc.), hand (15.5 pc.) and damages of the shoulder. Fractures are very common (55.5 pc.) in motorbike sports. Injuries often were produced by collisions with other motorbikes or trees. Driving mistakes, jumps, slipping wheels and overturn were other causes for an accident. During the championship 13 accidents were registered. In a clinical examination

Environmental factors and the risk for childhood pedestrian-motor vehicle collision occurrence.

In King County, Washington, during 1985-1986, 98 children aged less than 15 years involved in pedestrian-motor vehicle collisions resulting in death or injuries severe enough to require hospitalization were identified through the Trauma Registry at the Regional Trauma Center and the Medical Examiner's Office. Two age- and sex-matched controls were selected for each case, one through random digit dialing and the other from children undergoing appendectomy. Various environmental and traffic characteristics were ascertained from visits to the neighborhoods of all subjects. Children living in mult
