Occupant and crash characteristics in thoracic and lumbar spine injuries resulting from motor vehicle collisions.

BACKGROUND CONTEXT: Motor vehicle collisions (MVC) are a leading cause of thoracic and lumbar (T and L) spine injuries. Mechanisms of injury in vehicular crashes that result in thoracic and lumbar fractures and the spectrum of injury in these occupants have not been extensively studied in the literature. PURPOSE: The objective was to investigate the patterns of T and L spine injuries after MVC; correlate these patterns with restraint use, crash characteristics, and demographic variables; and study the associations of these injuries with general injury morbidity and fatality. STUDY DESIGN/SET

The road most travelled: the geographic distribution of road traffic injuries in England.

BACKGROUND: Both road safety campaigns and epidemiological research into social differences in road traffic injury risk often assume that road traffic injuries occur close to home. While previous work has examined distance from home to site of collision for child pedestrians in local areas, less is known about the geographic distribution of road traffic injuries from other modes. This study explores the distribution of the distance between home residence and collision site (crash distance) by mode of transport, geographic area, and social characteristics in England. METHODS: Using 10 years of

Pedestrian injury patterns according to car and casualty characteristics in france.

This paper describes the injury patterns of pedestrians involved in collisions with cars, compares them with other road casualties and estimates the possible effect of car front profile on injury location. Injury patterns were identified using the Rhône Road Trauma Registry which covers all the casualties resulting from crashes in the Rhône Département (1.6 million inhabitants) who seek medical care in health facilities. Fatality rates were estimated from national police reports for the same years (1996-2007), and the two data sources were linked to obtain information on the front profile of t

Injuries among powered two-wheeler users in eight European countries: a descriptive analysis of hospital discharge data.

Powered two-wheelers (PTWs--mopeds, motorcycles, and scooters) remain the most dangerous form of travel on today's roads. This study used hospital discharge data from eight European countries to examine the frequencies and patterns of injury among PTW users (age≥14 years), the predicted incidence of the loss of functional ability, and the mechanisms of the head injuries observed (all in light of increased helmet use). Of 977,557 injured patients discharged in 2004, 12,994 were identified as having been injured in PTW collisions. Lower extremity injuries accounted for 26% (25.6-26.7, 95% C.I.)

[Quad driving--life-endangering fun? A medical and technical analysis].

AIM: Quads or all-terrain vehicles do not seem to play a major role either in traffic accidents or in hospital admissions in Germany. However, reports about spectacular quad crashes in the press are not that infrequent. In contrast, no scientific survey or study regarding the issue of quad crashes in Germany has been published so far. Thus, this study aims to evaluate the present incidence of quad crashes, accident circumstances and resulting injury patterns, and to discuss possible consequences. METHOD: At a level I trauma centre, data from accident and hospital records of quad drivers were

The risk of pedestrian injury and fatality in collisions with motor vehicles, a social ecological study of state routes and city streets in King County, Washington.

This study examined the correlates of injury severity using police records of pedestrian-motor-vehicle collisions on state routes and city streets in King County, Washington. Levels of influence on collision outcome considered (1) the characteristics of individual pedestrians and drivers and their actions; (2) the road environment; and (3) the neighborhood environment. Binary logistic regressions served to estimate the risk of a pedestrian being severely injured or dying versus suffering minor or no injury. Significant individual-level influences on injury severity were confirmed for both type

Blunt cardiac trauma: lessons learned from the medical examiner.

OBJECTIVE: The objective of this study was to analyze autopsy findings after blunt traumatic deaths to identify the incidence of cardiac injuries and describe the patterns of associated injuries. METHODS: All autopsies performed by the Los Angeles County Forensic Medicine Division for blunt traumatic deaths in 2005 were retrospectively reviewed. Only cases that underwent a full autopsy including internal examination were included in the analysis. The study population was divided into two groups according to the presence or absence of a cardiac injury and compared for differences in baseline c

Classification tree modeling to identify severe and moderate vehicular injuries in young and middle-aged adults.

OBJECTIVES: Motor vehicle crashes are a leading cause of mortality and morbidity worldwide. Even though trauma centers provide the gold standard of care for motor vehicle crash patients with life- or limb-threatening injuries, many whose lives might be saved by trauma center care are treated instead at non-trauma center hospitals. Triage algorithms, designed to identify patients with life- or limb-threatening injuries who should be transported to a trauma center, lack appropriate sensitivity to many of these injuries. The challenge to the trauma community is differentiating patients with life-

All-terrain vehicle fatalities--West Virginia, 1999-2006.

An all-terrain vehicle (ATV) is a motorized vehicle designed for off-road use with low-pressure tires, a seat that is straddled by the operator, and handlebars for steering. Currently, only four-wheeled models are produced in the United States; production of three-wheeled ATVs ended in 1987 because of safety concerns. During the 1990s, West Virginia led the United States in per capita deaths from ATV crashes, with rates approximately eight times higher than the national average. In an attempt to curtail this trend, West Virginia enacted legislation in 2004 to regulate ATV use. This law prohibi

[Two wheels - too dangerous? Analysis of real-world crash data and federal statistics].

BACKGROUND: Motorcyclists profit from improved safety measures and medical care only slightly compared with other road users. The aims of this study were to gain new insights into crash mechanisms and the resulting injuries of motorcyclists and to understand trends in road traffic accident statistics over the last 10 years. METHODS: In our prospective analysis we documented data from collisions in which at least one subject sustained an injury of 2-6 on the Maximum Abbreviated Injury Scale (MAIS). Additionally, key parameters published by the German Federal Statistical Office were analysed.

Car crash and injury among young drivers: contribution of social, circumstantial and car attributes.

The objective of the study was to assess the independent contribution of individual, car and circumstantial features in severe and fatal car crashes involving young drivers. A prospective longitudinal, register-based cohort study was conducted at national level (in Sweden), in which people born in the years 1970-1972 (n = 334070) were followed up for the period 1988-2000 (aged 16-18 years in 1988) for their first two-car crashes leading to severe or fatal injury. Ten variables descriptive of the driver (sociodemographics), the car (safety level) and the crash have been analysed using multiple

[Inline skating--high fracture risk. Two of three injured are boys and young men. Wrist fractures are most common].

In-line skating injuries have increased in recent years. Hospital based data from Umeå concerning 135 persons injured in in line skating collisions were analyzed. The highest yearly incidence of injury was found in males 10-19 years of age, at 1.7 per 1,000 inhabitants; the corresponding figure for females was 0.5. Two-thirds of incidents were caused by falls due to balance problems without the influence of any "external factor" such as rough road surfaces. No collisions with motor vehicles or other road users were registered. Nearly half of the injuries were fractures or dislocation injuries,

Cycling to school--a significant health risk?

OBJECTIVES: The risk of injury to children riding bicycles has been previously documented. However, the specific risk arising from the use of bicycles as a mode of transportation to and from school is unknown. This study examines the incidence of bicycle related injuries among school age children. METHODS: A comprehensive prospective injury registration system was established in Stavanger, Norway. Data were obtained from this system to identify bicycle related injuries occurring from 1990-3 to children aged 10-15. The incidence of injuries was computed for two groups of children: (1) children

Pancreatic injuries from blunt trauma.

Pancreatic injuries from blunt trauma are infrequent, and their diagnosis and management can be extremely difficult. Over the past 10 years we treated 13 patients with major pancreatic injuries from blunt trauma. Twelve had been involved in motor vehicle collisions, none of whom were wearing seat belts. One patient was injured in an assault. Only five patients had physical findings suggesting intra-abdominal injury. Serum amylase levels were elevated in seven of the nine patients (78%) in whom this test was obtained. Computed tomography scans demonstrated pancreatic injury in three of the four

Deaths from motor vehicle crashes: patterns of injury in restrained and unrestrained victims.

A time comparison study of motor vehicle crashes in Monroe County, New York, from 1983 to 1986 was completed. Using a database of police accident reports, hospital logs, and autopsy reports from the county coroner, the hospital and autopsy reports of 91 unrestrained and 27 restrained fatally injured victims were reviewed. The hypothesis was that safety belts do not change patterns of injury in fatally injured victims. Patient data, seating position, and direction of impact were the same for both groups, while ejections occurred only in the unrestrained group (19.8%). Injury Severity Score (ISS

Bicycle accidents often cause disability--an analysis of medical and social consequences of nonfatal bicycle accidents.

Social and medical consequences of 278 children and 264 adults injured in bicycle accidents and seen in two hospitals in Helsinki in 1985-86 were analyzed. Information was collected from patient records, by means of a special questionnaire and by telephone interview. A child outpatient required 1.7 and a child inpatient 3.0 physician visits on an average, while adults required 2.2 and 4.9 visits, respectively. The average duration of hospital stay was 8 days for hospitalized adults and 6 days for children. Rehabilitative care outside the hospital was received by 6% of the adult outpatients and
