Lymphoepithelial Cyst Presenting as a Lateral Neck Mass: A Diagnostic Pitfall in Adults.

Cystic lateral neck masses in adults are most commonly attributed to branchial cleft cysts. Lateral cervical lymphoepithelial cyst is a rare benign entity that may represent a diagnostic challenge because of its nonspecific clinical presentation. We report the case of a 32-year-old woman presenting with a slowly enlarging right lateral cervical swelling evolving for one year. Clinical examination revealed a painless, well-defined, and mobile mass. Cervical ultrasonography followed by CT demonstrated a well-circumscribed cystic lesion measuring 37 × 45 × 65 mm, initially suggestive of a branchi

A retrospective study of cervical chondrocutaneous branchial remnants.

BACKGROUND: Cervical chondrocutaneous branchial remnants (CCBRs) are rare developmental anomalies that arise during early embryogenesis. Because of their rarity, most published reports consist of small case series, and no standardized classification system has been established. The purpose of this study was to define the standardized clinical characteristics of CCBRs and to propose a comprehensive classification system based on the largest cohort reported to date. METHODS: We retrospectively reviewed 55 CCBRs in 53 patients who underwent surgical excision between 2014 and 2023. Data collected

Management of a First Branchial Cleft Cyst in a Pediatric Patient: A Case Report.

First branchial cleft cysts are rare congenital anomalies in the pediatric population. The variable clinical presentations, ranging from asymptomatic neck masses to recurrent infections or otorrhea, pose significant diagnostic challenges, especially in young children, where anatomical landmarks are less defined, and misdiagnosis is common. This case report presents a detailed analysis of a Work type II first branchial cleft cyst in a one-year-old girl, emphasizing diagnostic strategies, imaging modalities, and individualized management considerations to optimize outcomes while minimizing compl

Bilateral, non-syndromic second branchial cleft fistula in an 8-year-old child: a case report.

BACKGROUND: Second branchial cleft anomalies are the most common congenital cervical malformations, accounting for approximately 90% of branchial cleft lesions. They usually present unilaterally, while bilateral involvement is exceedingly rare and most often associated with genetic syndromes such as branchio-oto-renal (BOR) syndrome. CASE PRESENTATION: We report the case of an 8-year-old Moroccan boy with no relevant past medical history who presented with bilateral, low-lying paramedian cervical fistulas that had been recurrently infected since early childhood. Clinical examination revealed

[Case of concurrent second branchial cleft cyst and diffuse large B-cell lymphoma].

Second branchial cleft cyst and lymphoma are two common types of neck masses. The etiology, clinical manifestations, imaging findings, treatment options, and prognosis of these two entities differ. Clinically, the occurrence of both lesions at the same anatomical site is rare. Herein, we report a case of concurrent second branchial cleft cyst and diffuse large B-cell lymphoma in the same anatomical region. The final diagnosis was confirmed by clinical features, ima-ging findings, and ultrasound-guided biopsy. The patient received the R-CHOP (rituximab, cyclophosphamide, doxorubicin, vincristin

Excision of a Giant Bilobed Second Branchial Cleft Cyst: A Case Report.

Second branchial cleft cysts (BCCs) are congenital cystic anomalies of the lateral neck that rarely attain giant proportions exceeding 10 cm. When they do, their size and proximity to critical neurovascular structures present distinctive diagnostic and surgical challenges. We present the case of a 24-year-old woman with a one-year history of a painless, enlarging left neck mass. Imaging revealed a large, bilobed cystic lesion deep to the sternocleidomastoid muscle extending from the skull base to the subclavicular region. Fine-needle aspiration cytology excluded malignancy, and the cyst recurr

Clinical and Genetic Features in EYA1-Associated Branchio-Oto Syndrome: Cochlear Nerve Deficiency in Five of Thirteen Patients.

Background/Objectives: Branchio-oto syndrome (BOS) is an autosomal dominant disorder primarily associated with pathogenic variants in EYA1, mainly characterized by branchial anomalies, auricular abnormalities, and hearing loss. However, the co-occurrence of inner ear malformations in BOS remains understudied, especially severe malformations. This study aimed to investigate the clinical and genetic characteristics of patients with EYA1-associated BOS, with emphasis on cochlear nerve deficiency (CND). Methods: From January 2020 to April 2026, patients diagnosed with EYA1-associated BOS at an oto

LACHT syndrome with branchial cyst in a female infant; case report and review of literature.

BACKGROUND: Lung agenesis is a rare congenital malformation. The combination of lung agenesis, congenital heart defects, and thumb anomalies characterizes LACHT syndrome, an extremely rare condition with an unclear genetic basis. To date, only 15 cases have been reported in the literature. CASE PRESENTATION: We report a new case of LACHT syndrome and review previously published cases of this rare association. Our patient was a three-month-old female infant who presented with the typical features of LACHT syndrome and an associated ipsilateral branchial cleft cyst, which has not been previousl

Rare intrathyroid nodule-like lesions: Two case reports.

Thyroid nodules and nodule-like lesions, as well as other structural abnormalities of the thyroid gland, are often detected during ultrasonography. True thyroid nodules should be evaluated for sonographic features suggestive of malignancy and appropriately triaged for further diagnostic workup to reduce the risk of missed thyroid cancer. In addition to true thyroid nodules, other lesions may coexist with or mimic them. Among these are branchial cleft cysts and thyroglossal duct cysts, 2 common embryological remnants. Although these lesions are typically found in the lateral neck and midline, r

Collision tumor of poorly differentiated thyroid carcinoma and intrathyroid thymic carcinoma arising in association with a branchial cleft-like cyst: a case report.

BACKGROUND: Primary squamous carcinoma of the thyroid is an exceptionally rare malignancy, and its coexistence with poorly differentiated thyroid carcinoma (PDTC) is exceedingly uncommon. The pathogenesis of thyroid squamous carcinomas remains controversial, particularly when associated with congenital epithelial remnants such as branchial cleft-like cysts or thymic/ultimobranchial remnants. We report an unusual collision tumor composed of PDTC and intrathyroid thymic carcinoma (ITTC) arising in association with a longstanding cystic thyroid lesion. CASE PRESENTATION: A 59-year-old man initia

Nasopharyngeal Branchial Cleft Cyst in an Elderly Man: An Atypical Presentation.

INTRODUCTION: Branchial cleft cysts (BCCs) account for nearly one-third of congenital cervical masses, but their occurrence within the nasopharynx is exceedingly uncommon. Most reported nasopharyngeal cases involve children or young adults; presentation in older patients raises concern for cystic metastasis from an occult head and neck malignancy. CASE REPORT: We describe a 64-year-old man with a three-year history of progressive bilateral nasal obstruction and recurrent rhinosinusitis. Endoscopy disclosed a smooth, mobile, midline pinkish nasopharyngeal mass; cross-sectional imaging confirme

[Pediatric Thyroiditis Secondary to Fourth Branchial Arch Anomaly: A Case Series].

INTRODUCTION: A fourth branchial cleft remnant is relatively rare and often misdiagnosed, primarily presenting as a sinus with an opening in the pyriform sinus, known as pyriform sinus fistula. Confirmation of the diagnosis is achieved through either a barium swallow test, or direct rigid laryngoscopy. We present our experience in diagnosing this anomaly. BACKGROUND: The series comprised 6 children, 4 boys and 2 girls 1-13 years of age, all with left cervical infection. Five patients presented with a primary episode of cervical infection, while in one case, it was a second infection. Diagnosi
