[Pediatric Thyroiditis Secondary to Fourth Branchial Arch Anomaly: A Case Series].

INTRODUCTION: A fourth branchial cleft remnant is relatively rare and often misdiagnosed, primarily presenting as a sinus with an opening in the pyriform sinus, known as pyriform sinus fistula. Confirmation of the diagnosis is achieved through either a barium swallow test, or direct rigid laryngoscopy. We present our experience in diagnosing this anomaly. BACKGROUND: The series comprised 6 children, 4 boys and 2 girls 1-13 years of age, all with left cervical infection. Five patients presented with a primary episode of cervical infection, while in one case, it was a second infection. Diagnosi

18F-FAPI-74 PET/CT findings of an inflammatory branchial cleft cyst mimicking cervical metastasis from carcinoma of unknown primary: A case report.

Fibroblast-activation protein inhibitor (FAPI) positron-emission tomography (PET) is emerging as a promising alternative to 18F-fluorodeoxyglucose (FDG) PET for head-and-neck cancer. Herein, we describe the case of a 51-year-old man with a cystic level-II neck mass that was intensely FDG-avid and even more avid on 18F-FAPI-74 PET. FDG-PET was performed as standard imaging modality to search for the primary tumor, while 18F-FAPI-74 PET was conducted as part of a clinical trial to detect potential occult lesions missed by FDG. While cytology suggested metastatic squamous-cell carcinoma and FDG P

Ranula vs. Atypical Sublingual Branchial Cleft Cyst: A Case Report.

Congenital cystic lesions in the floor of the mouth in infants present significant diagnostic challenges owing to overlapping clinical and radiological features. We report a rare case of a one-year-old boy with a gradually enlarging, painless left sublingual swelling noticed since birth. Clinical examination revealed a soft, fluctuant 3 × 4 cm mass that did not move with deglutition or tongue protrusion, along with a small midline submental dimple. Contrast-enhanced computed tomography (CECT) showed a well-defined, non-enhancing cystic lesion (4.1 × 2.1 × 3.9 cm) in the left sublingual space,

Posterior Auricular and External Auditory Canal Involvement from an Infected Preauricular Fistula Mimicking a First Branchial Cleft Cyst.

Preauricular fistula is a common congenital anomaly that typically presents with localized infection near the auricular pit. We report a case of an infected preauricular fistula in a 9-year-old girl presenting with atypical inflammation involving the posterior auricular sulcus and the external auditory canal, initially mimicking a first-branchial cleft anomaly. Despite prolonged antibiotic therapy, the infection persisted. Contrast-enhanced computed tomography suggested a branchial cleft lesion. However, intraoperative findings demonstrated that the fistulous tract terminated within the auricu

miR‑324‑5p in oncology: Molecular mechanisms and clinical outlook (Review).

BACKGROUND: Batracylin is a heterocyclic arylamine topoisomerase inhibitor with preclinical anticancer activity. Marked species differences in sensitivity to the toxicity of batracylin were observed and attributed to differential formation of N-acetylbatracylin by N-acetyltransferase. A Phase I trial of batracylin in cancer patients with slow acetylator genotypes identified a dose-limiting toxicity of hemorrhagic cystitis. To further explore the metabolism of batracylin and N-acetylbatracylin across species, detailed studies using human, rat, and dog liver microsomal and hepatocyte preparation

RUNX2 Rearrangement as a Recurrent Molecular Event and Diagnostic Marker in Salivary Gland Keratocystoma.

Keratocystoma is considered a rare, benign salivary gland tumor predominantly occurring in the parotid gland. Owing to its rarity, insufficiently recognized clinicopathological features, immunohistochemical (IHC) phenotype, and molecular characteristics, keratocystoma remains diagnostically challenging. This study aimed to investigate the clinical, histological, and molecular features of keratocystoma to refine the histological diagnostic boundaries. Nineteen cases of keratocystoma were analyzed using fluorescence in situ hybridization, targeted RNA sequencing, and IHC. Histological mimics, in

Progressive facial Swelling and Airway Symptoms Revealing a Deep Neck Abscess With Atypical Microbiology.

Deep neck abscesses have become less common in the antibiotic era; yet, they still pose major risks of morbidity and mortality if not addressed appropriately. Their proximity to the upper respiratory tract and mediastinal structures further exacerbates this risk. We present the case of a 56-year-old man with a medical history of type 2 diabetes mellitus, who exhibited a two-month history of recurrent left-sided facial edema, pain, and dysphagia. Computed tomography imaging confirmed a substantial fluid collection extending from the submandibular region into the mid-cervical area, from which as

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Complete second branchial cleft fistula in a fifteen-year-old boy: A case report.

INTRODUCTION AND IMPORTANCE: The branchial or pharyngeal apparatus, crucial in embryological development, consists of clefts, arches, pouches, and membranes. Anomalies arising from this apparatus particularly involving the second branchial arch, are rare. Among these anomalies, complete second branchial cleft fistulas, with both external and internal openings, are exceptionally uncommon. This case report presents such a rarity in a fifteen-year-old boy, highlighting the clinical presentation, diagnostic approach, surgical management, and outcome. Papillary thyroid microcarcinoma presenting as a large cystic lymph node: A case report.

Papillary thyroid carcinoma (PTC) may deviate from the expected clinical course, posing diagnostic challenges to clinicians. The present study presents a case of longstanding swelling on the left side of the neck, initially diagnosed as a cystic lesion, which was later revealed to be metastatic thyroid cancer. A 21-year-old woman presented with a 3-year history of a neck mass that had initially been diagnosed as a likely hemangioma or branchial cleft cyst in 2019. However, subsequent scans in 2022 revealed a suspicious cyst and a thyroid nodule, which were confirmed to be PTC with metastasis t

The Sesquialter (First and a Halfth) Branchial Cleft Anomaly.

A 6-month-old presented with a sinus in the neck coursing from below the body of the mandible, between the carotid and parotid spaces, and terminating posterior to the stylomastoid foramen on MRI. Facial nerve monitoring was used, a lacrimal probe was placed in the sinus, an elliptical incision was made in the skin, and the tract was followed to the parapharyngeal space. Pathology demonstrated a keratinous squamous epithelial lining, skin adnexae, and elastic cartilage. Because there were clinical, radiographic, and pathological features intermediate to first and second branchial anomalies, we

Two-year follow-up of a rare primary branchial cleft carcinoma: Case analysis and management insights.

Branchial cleft cysts are congenital anomalies of development, with the second branchial cleft cyst being the most common clinically. In extremely rare cases, branchial cleft cysts can undergo malignant transformation. Herein, we present a rare case of a 65-year-old male patient who had a painless neck mass on the left side for 6 months. Routine postoperative histopathological examination of the resected mass revealed branchial cleft carcinoma. We excluded the possibility of systemic metastasis through PET-CT and subsequently performed radical neck dissection. Routine follow-up revealed no rec

Papillary thyroid carcinoma in a branchial cleft cyst.

SUMMARY: Papillary thyroid carcinoma (PTC) in a branchial cleft cyst (BCC) is exceedingly rare. This case report describes a 53-year-old male with a BCC containing PTC. Despite normal preoperative thyroid imaging, total thyroidectomy revealed multifocal bilateral micropapillary thyroid carcinoma with a contralateral metastatic lymph node, suggesting an aggressive disease course. This finding supports the likelihood that thyroid gland carcinoma metastasized to the BCC. However, the possibility of a primary tumor arising from ectopic thyroid tissue within the cyst cannot be excluded. To our know
