Surgical treatment of first branchial cleft anomalies using retrograde facial nerve dissection technique.

PURPOSE: First branchial cleft anomalies (FBCAs) are infrequent congenital malformations. In FBCAs removal surgery, due to the previous infection history and the anatomical proximity of the FBCAs tract to the facial nerve, postoperative recurrence and facial paralysis are not uncommon. This study aimed to assess the clinical feasibility and outcomes of FBCAs resection using the retrograde facial nerve dissection technique. METHODS: This retrospective study included 19 patients (mean age, 6.3 ± 4.4 years) who underwent FBCAs excision via retrograde facial nerve dissection between 2017 and 2023

Quality of life (QoL) following transoral robotic surgery (TORS) resection of parapharyngeal space tumours.

INTRODUCTION: Parapharyngeal space tumours (PSTs) can be challenging to manage in view of their distinct location and proximity to vital neurovascular structures. Numerous surgical approaches have been described including minimally invasive techniques utilising transoral robotic surgery (TORS). This study aims to evaluate the role of TORS in the management of PSTs with a special focus on quality of life (QoL) outcomes. METHODS: A systematic search was performed to evaluate the existing literature surrounding the management of PSTs with TORS. In addition, all patients at our tertiary unit were

Right-Sided Type IV Branchial Cleft Anomaly in a Seven-Year-Old Boy: A Case Report.

Type IV branchial cleft anomalies, which can present as a cyst, sinus, or fistula, are the rarest type of branchial cleft anomalies. Although they can present at any age, they typically present during childhood with a history of frequent neck abscesses. Type IV branchial cleft anomalies usually occur on the left side near the medial lower border of the sternocleidomastoid (SCM) muscle and can extend to the pyriform sinus in the larynx. They can also present with thyroid involvement. Imaging with ultrasound, computed tomography (CT), magnetic resonance imaging (MRI), sinogram, and direct laryng

Management of a First Branchial Cleft Fistula in an Adult Patient: A Case Report and Literature Review.

First branchial cleft anomalies (FBCAs) are uncommon congenital malformations that develop due to incomplete closure of the first branchial cleft during embryonic development. They represent a small proportion of all branchial cleft anomalies. These lesions, which may present as cysts, sinuses, or fistulas, pose significant diagnostic and therapeutic challenges due to their nonspecific clinical manifestations and complex anatomical relationships, particularly with the facial nerve and parotid gland. This case report describes a 47-year-old male patient presenting with recurrent infections and

Branchial Cleft Cyst Carcinoma Remains Grossly Over Diagnosed: A Large Database Analysis.

INTRODUCTION: Branchial cleft cyst carcinomas (BCCC) are rare lateral neck malignancies thought to originate from branchial cleft remnants. Diagnosing primary BCCC should be approached with skepticism; cystic nodal metastasis must be excluded. Due to its rarity, we performed a comprehensive database study to better characterize the ongoing diagnosis of BCCC. METHODS: Single-arm retrospective cohort study using the TriNetX Research database. Patients ≥ 18 years of age with a diagnosis of BCCC identified by ICD-10 C10.4 between 2008 and 2018 were included, allowing for up to 5 years of follow-u

[Novel PAX1 mutation identified in autosomal dominant otofaciocervical syndrome 2 with new phenotypes].

Objective: To determine the diagnosis of microtia-associated syndrome through genetic testing. Methods: Peripheral venous blood samples were collected from members of a two-generation family with a syndrome associated with ear malformations (3 patients and 1 normal control). Pathogenic mutations were identified using whole exome sequencing analysis, Sanger sequencing validation, and bioinformatics analysis. Based on the genetic diagnosis and a review of the literatures, the patients' clinical phenotypes were thoroughly evaluated to confirm the clinical diagnosis. Results: All three patients ca

A Misleading Neck Mass: Branchial Cleft Cyst Mimicking Anaplastic Thyroid Carcinoma.

A 33-year-old female individual had a history of a lymph node excision positive for metastatic papillary thyroid carcinoma (PTC) in 2016. She was treated with a total thyroidectomy and central neck dissection showing 0.2-cm micro-PTC without paratracheal lymph node involvement, followed by adjuvant radioactive iodine treatment with 104 mCi of iodine-131. After 9 years without evidence of disease, she presented with a mildly tender, firm, left neck level II mass rapidly enlarging over 3 weeks. Neck ultrasound revealed a 3.5 cm irregular hypoechoic mass with internal vascularity and punctate ech

Case report of a novel GREB1L gene mutation in a patient with branchio-oto-renal syndrome.

Branchio-oto-renal (BOR) syndrome is an uncommon disorder inherited in an autosomal dominant manner. Its main clinical manifestations include branchial cleft cysts, anterior auricular fistula, hearing impairment, and kidney malformations. BOR syndrome is associated with heterozygous pathogenic variants including EYA1, SIX1, and SIX5. The study focused on a 13-year-old Chinese boy who presented with hearing impairment, renal malformations, and bony atresia of the right external auditory canal with microtia. The boy's clinical manifestations met the diagnostic criteria for BOR syndrome. Two of t

Papillary Thyroid Carcinoma in a Branchial Cleft Cyst: A Two-Case Single-Center Experience.

Papillary thyroid carcinoma (PTC) arising within a branchial cleft cyst (BCC) is an exceptionally rare condition that presents diagnostic and therapeutic challenges. This study aims to describe two such cases identified in our institution and to review the relevant literature. A retrospective review of the pathology database of Unidade Local de Saúde (ULS) de Coimbra in Coimbra, Portugal, from 2015 to the present identified two such cases. Clinical, pathological, and follow-up data were analyzed, and a brief literature review was performed. The first patient, a 63-year-old man, presented with

Adult-Onset Cervical Lymphatic Malformation: A Case Report.

Lymphatic malformations (LMs) are low-flow vascular malformations of lymphatic origin that are typically diagnosed in childhood, with cervicofacial LMs posing particular risks for functional and cosmetic morbidity. Fewer than 10% of LMs present after adolescence, and adult-onset cervical cases are exceedingly rare, with only isolated reports in the literature. Adult-onset cervical LMs often mimic other cystic neck lesions, complicating diagnosis and delaying management. We report the case of a 20-year-old male who presented with a painless, slowly enlarging left upper neck mass. Ultrasound dem

A case of branchial cleft cyst in the esophagus: A case report.

Branchial cleft cyst, also known as a benign lymphoepithelial cyst, is a congenital disorder with an unclear pathogenesis. It most commonly occurs in the anterior triangle of the neck along the upper third of the sternocleidomastoid muscle, while occurrences at other sites are exceedingly rare. Diagnosis through ultrasound, computed tomography, or magnetic resonance imaging is challenging. Endoscopic submucosal dissection (ESD) enables the acquisition of complete specimens, aiding in the diagnosis and treatment of such lesions. This study presents a case of an esophageal branchial cleft cyst t

Intrathyroidal Branchial Cleft Cyst Mimicking Malignancy: A Cytologic Pitfall in the Era of Incidental Imaging.

Branchial cleft cysts are congenital anomalies derived from the branchial apparatus, most commonly presenting as lateral neck masses. Intrathyroidal localization is exceedingly rare and may mimic malignancy both radiologically and cytologically-particularly when squamous cells are identified and misinterpreted as malignant. Surgical resection is often required to achieve a definitive diagnosis. Our report highlights the diagnostic challenges of such lesions and underscores the importance of integrating clinical, cytological, and imaging data.

A case report of tracheal duplication cyst with steatocystomatous histology: a rare embryological overlap.

BACKGROUND: Tracheal duplication cysts (TDCs) are rare congenital anomalies caused by defects in the duplication of the foregut, that is of endodermal origin. These cysts are usually present during childhood and cases in adults are seen very rarely. Steatocystoma multiplex is a disorder of multiple sebaceous cysts, most commonly from skin appendages, that has ectodermal origin. The presence of the two coexisting in the same place has never been reported in literature. We report the first case of a unique embryological overlap between ectoderm and endoderm in a TDC. CASE PRESENTATION: A 34-yea

Cervical Schwannoma: A Rare Case Presentation - Neural Tissue Tumour.

Cervical schwannomas are rare benign tumors that can present a significant diagnostic challenge, particularly when they exhibit cystic changes that mimic more common lesions such as second-branchial cleft cysts. We report the case of a 36-year-old male who presented with a gradually enlarging, firm, nontender neck mass. Imaging studies, including ultrasound and contrast-enhanced computed tomography, suggested a cystic lesion, most likely a second branchial cleft cyst. Fine-needle aspiration cytology was inconclusive. Surgical excision was undertaken, and histopathological examination revealed

Diving Into the Rarity: A Case Report of Nonsyndromic Bilateral Branchial Cleft Fistula.

Bilateral branchial cleft fistulas are extremely rare congenital anomalies that may be part of a larger syndromic association. Prompt recognition and treatment are crucial in syndromic patients to halt disease progression, improve outcomes, and reduce the risk of long-term complications. Complete surgical excision is the standard treatment for fistulas.

Isolated facial pain as the sole presentation of a type I branchial cleft cyst in an adult male.

First branchial cleft cysts (FBCCs) are rare developmental anomalies that typically present in childhood. We report a unique case of a Type I FBCC in a 43-year-old male whose only symptom was isolated facial pain without infection or mass. Imaging and biopsies were inconclusive, prompting surgical excision for definitive diagnosis and symptomatic relief. Histopathology confirmed a branchial cleft anomaly. This case highlights the importance of including congenital anomalies in the differential diagnosis for atypical facial pain even in the adult population and underscores the value of a thorou

Branchial cleft carcinoma with cervical lymph node metastasis: a Case Report.

Branchial cleft carcinoma is a rare malignant tumor originating from the epithelium of branchial cleft cysts, which are highly susceptible to misdiagnosis. There is a lack of systematic consensus on its lymph node metastasis pattern and standardized treatment protocol. We report a case of branchial cleft carcinoma with cervical lymph node metastasis and discuss its diagnostic criteria, clinicopathological features, lymph node metastatic characteristics, and treatment strategies. This case report supports the notion that branchial cleft carcinoma should be diagnosed according to Khafif's criter
