Concrete silo collapse: emergency medical services response to a mass casualty incident.

BACKGROUND: Major incidents evolving from occupational accidents are very infrequent in Scandinavia and therefore, case reports are called for. On 26 November, 2024, a fatal occupational accident took place during the construction of a concrete silo in the small rural town of Flemløse (population 574), Denmark. Three people died and six were injured as the result of a collapsing concrete roof during construction. We aim to describe the incident response by the emergency medical services (EMS), to identify areas of improvement, and to evaluate the adherence to current national major incident gu

Injuries Due to In-Flight Turbulence During United States Commercial Airline Flights (2008-2023).

IntroductionAlthough often harmless, unexpected turbulence can occasionally cause injuries and even mortality for passengers on commercial airline flights. Studies of the mechanisms of injury associated with turbulence-related accidents can help airlines and regulatory agencies develop strategies to improve safety during encounters with turbulence.MethodsWe examined accident reports from the National Transportation Safety Board (NTSB), a federal agency tasked with investigating aviation accidents. Reports for accidents with at least one serious injury between January 1, 2008, and December 31,

Survival After Ditching in Motorized Aircraft, 1989-2022.

INTRODUCTION: Although an unintended aircraft landing on water (referred to as ditching) is a rare event, the potential for occupant injury/fatality increases immediately following the event due to adverse conditions. However, to date, few studies have addressed the subject. Herein, ditching events and post-ditching survival were investigated.METHODS: Ditchings (1982-2022) in the United States were identified from the National Transportation Safety Board database. Occupant injury severity, aircraft type, pilot experience, flight conditions, and number of occupants were extracted. Poisson distr

Circumstances and Pathological Findings in Civilian Helicopter-Related Fatalities.

Helicopters are used worldwide in a range of commercial and private industries and are particularly useful in northern Australia due to the sparse population scattered over large distances with remote regions difficult to access by any other means of transport. Nine civilian helicopter-related fatalities were reported to the Northern Territory coroner between January 2004 and December 2023. The victims were all male (age range, 34-74 years; mean, 52 years). All fatal crashes occurred in remote areas, the majority on cattle stations. Pilots were the sole occupants in 5/9 incidents; in 4 inciden

Occupant injury and fatality in general aviation aircraft for which dynamic crash testing is certification-mandated.

Towards further improving general aviation aircraft crashworthiness, multi-axis dynamic tests have been required for aircraft certification (14CFR23.562) since 1985. The objective of this study was to determine if occupants in aircraft certified to these higher crashworthiness standards show a mitigated fraction of fatal accidents and/or injury severity. The NTSB aviation database was queried for accidents occurring between 2002 and 2012 involving aircraft certified to, or immune from, dynamic crash testing and manufactured after 1999. Only operations conducted under 14CFR Part 91 were conside

Complex alpine extrication: case report of mountain and speleological rescue cooperation.

Mountain sporting activities are an increasingly popular practice that exposes mountaineers to a high risk of adverse events. This report describes a unique case of recovery in an austere environment that involved explosives. In June 2012, a 52-year-old man ascended a cliff tower in the Eastern Alps, Italy. A landslide occurred, and a boulder crushed the climber against a large stone located farther down the cliff, causing compression of the lower limbs and the pelvis with consequent severe musculoskeletal trauma. The National Alpine and Cave Rescue Unit (NACRU) arrived and proceeded with stab

[Rescue procedures in the major trauma of upper extremities - the role of the Polish medical air rescue in the therapeutic process].

BACKGROUND: Due to the growing use of various types of industrial and agricultural machinery, occupational accidents are among the most serious ones and quite frequently result in the permanent posttraumnatic disability of the injured person. In Poland, a replantation service has been operating since 2010. Each day, one out of six centres provides emergency replantation service accepting amputation calls from across the country. Patients qualified for replantation often need to be transported from places located even several hundred kilometres from the target hospital. MATERIAL AND METHODS: T

Helicopter crashes into water: warning time, final position, and other factors affecting survival.

INTRODUCTION: According to 40 yr of data, the fatality rate for a helicopter crash into water is approximately 25%. Does warning time and the final position of the helicopter in the water influence the survival rate? METHODS: The National Transportation Safety Board (NTSB) database was queried to identify helicopter crashes into water between 1981 and 2011 in the Gulf of Mexico and Hawaii. Fatality rate, amount of warning time prior to the crash, and final position of the helicopter were identified. RESULTS: There were 133 helicopters that crashed into water with 456 crew and passengers. Of

Probable cause in helicopter emergency medical services crashes: what role does ownership play?

BACKGROUND: The National Transportation Safety Board (NTSB) ranks helicopter emergency medical services (HEMS) as one of the most perilous occupations in the United States, with improvements in its safety of highest priority. As many injured patients are transported by helicopter, this is of particular concern to the trauma community. The use of HEMS is associated with a heightened degree of inherent risk. We hypothesized that this risk is not uniform and varies with the entity providing HEMS, specifically, commercial versus public safety providers. METHODS: The NTSB accident database was que

Usual seat belt practices reported by airline passengers surveyed in gate areas of a U.S. airport.

INTRODUCTION: Most people who are seriously injured or die as a result of air turbulence were not wearing seat belts, yet we find no published studies of airline passengers' seat belt use. Drawing from the traffic safety literature, we explored characteristics that might be associated with passenger restraint use on planes. METHODS: The study was conducted at a major U.S. airport. All passengers waiting to board 24 domestic flights were asked to complete a brief, anonymous survey. We solicited data on various passenger characteristics, including their alcohol consumption patterns. We then use

Analysis of 2004 German general aviation aircraft accidents according to the HFACS model.

INTRODUCTION: The number of aircraft accidents remains on a constant level since the late 1990s. Routine analysis in detail of the causative factors is not carried out in Germany. The analysis of flight mishaps has been demonstrated to be an important basis for flight safety. The Human Factors Analysis and Classification System (HFACS) model is best suitable for aircraft accident analysis. The aim of this study was to classify aircraft accidents in the General Aviation (GA) of Germany according to the HFACS model and to figure out the underlying causes. MATERIAL AND METHODS: The analysis was
