Midair collisions in U.S. civil aviation 2000-2004: the roles of radio communications and altitude.

BACKGROUND: Midair collisions are destructive to aircraft and often fatal to occupants, with the additional possibility of death and destruction on the ground. We sought to determine the characteristics of civil aviation mid-air collisions in U.S. airspace with focus on the altitudes at which the collisions took place and radio communications prior to the collisions. METHODS: Accident reports published by the National Transportation and Safety Board for the period 2000-2004 were analyzed using descriptive statistics. Additional information was extracted from the narrative report of each accid

Rupture of the pectoralis major muscle in a paratrooper.

Rupture of the pectoralis major muscle is a very rare injury. Excessive contraction of muscle fibers during certain forms of sports, such as weightlifting and bench pressing, is the most common cause. Among the 150 reported cases in the literature, in only 1 case did the injury happen during the landing phase of parachuting. Here we report a case of pectoralis major muscle rupture caused by a different mechanism than published previously. A paratrooper was injured during a tactical jump out of an aircraft after becoming entangled with the risers. The mechanism of injury was excessive traction

Medical and trauma evacuations.

Evacuating ill or injured expatriates on international assignments from globally challenging locations is a potent source of concern for individuals who are involved before, during, and after overseas assignments. It is essential to minimize the risk for such evacuations and to make the evacuations proceed as smoothly and safely as possible. The first half of this article addresses the planning and processes that are required to make inevitable medical evacuations as safe and smooth as possible. The second half reviews the non-medical consequences that medical and security evacuations can have

Injuries and diseases among commercial fishermen in the Northeast Atlantic and Barents Sea. Data from the Royal Norwegian Coast Guard.

OBJECTIVES: The working conditions in the Northeast Atlantic may be hazardous, and accidents are frequent. The objective of this study was to clarify the situation as viewed by the Royal Norwegian Coast Guard (RNCG). METHODS: Consecutive reports on medical assistance given by the three major RNCG vessels from October 1995 and March 2001 were analysed. RESULTS: There were 117 reports. Men constituted 96%, and 54% were Norwegians. The median age was 38 years. Seriousness was classified as low (46 cases), moderate (43 cases), serious (22 cases) and life threatening [five cases (two died)]. Infe

Death and injury on duty--a study of South African police officers.

OBJECTIVES: Sections of contemporary South African society are characterised by high levels of crime and interpersonal conflict. The South African Police Service (SAPS) is in the front line, with many deaths and injuries occuring among serving officers in recent years. The circumstances and nature of the injuries sustained by these officers have received little attention in the medical literature. DESIGN: A retrospective review of all injured SAPS members was undertaken at a single level one trauma centre in Johannesburg between June 1993 and June 2002. The following factors were recorded: de

Incidence, epidemiology, and occupational outcomes of thoracolumbar fractures among U.S. Army aviators.

BACKGROUND: The routine occupational hazards of flying and parachute jumping place U.S. Army aviators at risk for sustaining high-energy traumatic injuries, such as thoracolumbar fractures. METHODS: A longitudinal, prospective, epidemiologic database was used to determine the incidence, injury history, and aeromedical disposition of U.S. Army aviators who sustained thoracolumbar fractures for calendar years 1987 to 1997. RESULTS: The overall incidence rate of thoracolumbar fracture was 12.8 per 100,000 aviators per year. Thirty aviators with thoracolumbar fractures were identified, and the a

Breath-holding ability of offshore workers inadequate to ensure escape from ditched helicopters.

BACKGROUND: Following a helicopter ditching in water, the survival rate of individuals not mortally injured by the impact ranges from 50-85%. One possible cause for this low survival rate is that the crew and passengers cannot hold their breath underwater long enough to make the often difficult escape from an inverted and submerged helicopter. METHODS: We investigated pulmonary function, breath-holding times in air (BHTa) and water (BHTw) of 228 students enrolled in offshore survival courses required to work in either the offshore petroleum industry or in military marine aviation. Comparisons

Air flight disaster, posttraumatic stress, and postventive rescue and response: the aftermath of the San Diego PSA 182 plane crash recovery operation, 20 years on.

San Diego in 1978 was the scene of one of the USA's most tragic and traumatic air flight disasters, when an inbound Boeing 727 collided with a small private Cessna 172. The collision occurred in the vicinity of North Park, a suburb of San Diego. All aircraft occupants were killed or injured and many residents were injured. The wreckage was mainly concentrated in an area about the size of a city block, in a temperature of 100 degrees F or more. The whole experience was unlike anything any of the professionals involved had been prepared for, and beyond anything the civilians involved could have

Correlates of pilot fatality in general aviation crashes.

BACKGROUND: General aviation accounts for the majority of aviation crashes and casualties in the United States, and general aviation safety has not improved in the past decade. OBJECTIVE: This study identifies factors associated with pilot fatality in general aviation crashes. METHODS: We analyzed the National Transportation Safety Board's Factual Reports for all airplane and helicopter crashes of general aviation flights that occurred in North Carolina and Maryland during 1985 through 1994. Surviving pilots were compared with fatally injured pilots in relation to crash circumstances, and pi

Risk factors for injury among construction workers at Denver International Airport.

BACKGROUND: The Denver International Airport construction project provided a rare opportunity to identify risk factors for injury on a large construction project for which 769 contractors were hired to complete 2,843 construction contracts. Workers' compensation claims and payroll data for individual contracts were recorded in an administrative database developed by the project's Owner-Controlled Insurance Program. METHODS: From claims andy payroll data linked with employee demographic information, we calculated injury rates per 200,000 person-hours by contract and over contract characteristi

Alcohol in aviation-related fatalities: North Carolina, 1985-1994.

BACKGROUND: Previous studies examining the role of alcohol in aviation safety have been limited to pilots and the estimated degree of alcohol involvement in fatal aviation crashes was susceptible to selection bias because alcohol testing was not conducted on a routine basis. This study examines the magnitude of and factors related to alcohol involvement in both pilot and non-pilot aviation fatalities. METHODS: We analyzed medical examiner data on all victims who died in civilian aircraft crashes in North Carolina during 1985-94, complemented by crash investigation data from the National Trans

[Transportation by ambulance in a rural district in Iceland during the years 1990-1996.].

INTRODUCTION: The Health Centre at Kirkjubaejarklaustur serves a large rural district with a population a little over 600. Agriculture and various services including tourism are the main occupations. Almost 20% of the population are over 65 years of age, which is an unusually high figure compared to the rest of the country. Ambulance transports are one of the health centre's responsibilities. Most of these are surface transports to hospitals, 200-250 kilometres away. OBJECTIVE: The purpose of this study was to explore and explain the nature of all ambulance transports provided by the Kirkjuba

The effects of brace position on injuries sustained in the M1 Boeing 737/400 disaster, January 1989. NLDB Study Group.

Of the initial 87 survivors of the East Midlands Boeing 737/400 aircraft, 77 sustained head and facial trauma during the crash, 45 of whom were rendered unconscious. There were 21 who received injuries to the back of their head, including 5 of the 6 severely head-injured adults. Those passengers who adopted the fully flexed "brace" position for crash-landing achieved significant protection against head injury, concussion, and injuries from behind irrespective of local aircraft structural damage. A computer graphics simulation developed by a commercial firm (H. W. Structures, Ltd.) using the pr

Helicopter rotor blade injury: a persistent safety hazard in the U.S. Army.

Rotor blade injuries are an inherent hazard of helicopter operations. To determine the recent incidence of rotor blade injuries in the U.S. Army, a review of accident records (1972-91) was conducted. Crash-related injuries were not included. During the study period, there were 24 blade strike injuries (12 involving the main rotor), 11 (46%) of which were fatal. Comparison with previous reports indicates a lower rotor blade injury rate in the last decade than in any previous period. The head was injured most frequently (65%), followed by the chest (17%) and abdomen (7%). Protective helmets help

Crashes of commuter aircraft and air taxis. What determines pilot survival?

Pilots have been recognized as an occupational group having very high work-related death rates. Commuter aircraft and air taxis are far more likely to be involved in fatal crashes than are larger airplanes. To examine the factors related to pilot survival in commuter aircraft and air taxi crashes, we analyzed National Transportation Safety Board data for the years 1983 to 1988. Case fatality rates were calculated for the pilots-in-command and odds ratios were estimated from a multivariate logistic regression model for variables related to pilots, aircraft, and circumstances. During the 6-year

[Abdominal injuries after blunt trauma].

We present a series of 331 patients admitted to hospital in 1980-87 with abdominal injuries after blunt trauma. The patients included 230 males and 101 females. The median age was 29 years. More than half of the patients were injured in traffic accidents. 11% were transferred to our Trauma Center from other hospitals, median five hours after the accident. A doctor-manned helicopter transported 52 patients (18%) directly to our hospital. 70% had extra-abdominal injuries as well. A minimum of 20% were intoxicated by alcohol and/or drugs. Severe injuries (AIS greater than 3) were present in 46%.

Injury in U.S. Army helicopter crashes October 1979-September 1985.

All U.S. Army class A and B mishaps of four types of helicopters occurring from 1 October 1979 through 30 September 1985 were reviewed. During this 6-year period, there were 298 crashes involving 303 aircraft. There were 1,060 individuals aboard the crashed aircraft and 611 were injured, 136 fatally. The most common cause of injury was the "secondary impact" caused by collapse of structure into occupied areas, by inadequate restraint of the occupants which allowed them to flail into structure, or by a combination of both mechanisms. Injury solely related to acceleration occurred infrequently.

Hazards of mountain flying: crashes in the Colorado Rockies.

Between 1964 and 1987, 232 airplanes crashed within 50 nautical miles of Aspen, CO; 90% were general aviation crashes. A total of 202 people died and 69 were seriously injured. The societal cost averaged more than $4 million annually. Most pilots were experienced and many were flight instructors, but 44% had flown less than 100 hours in the type of plane in which they crashed. Forty-one percent of the pilots were out-of-state residents. Crashes in the study area were more likely to be fatal than in the rest of Colorado. Airplanes with three or four occupants and low-powered four-seater aircraf
