The rehabilitation of injured workers in New Zealand: a pilot study.

At the request of the local office of the Accident Compensation Corporation (ACC) we established a centre which offered a multidisciplinary rehabilitation programme consisting of a full assessment and an eight week individually planned and monitored fitness programme. The clients progressed to occupational therapy, work trials, and later to paid work. The programme included education about back care and health maintenance, and relaxation techniques. Of the 48 injured people who attended the centre, 37 were assessed as suitable to enter the programme, and 32 people took part. Outcome, measured

Identification of visual scanning deficits in adults after cerebrovascular accident.

Visual scanning deficits can result in the inefficient acquisition of information about one's environment. For adults who have had cerebrovascular accidents (CVAs), such deficits can interfere with the independent performance of many daily activities. The present study examined five tests designed to measure visual scanning deficits in post-CVA adults. Subjects with right and left hemisphere lesions were matched with control subjects by age and sex, and their performances on the tests were compared. The difference between the matched groups was statistically significant for all tests, with the

Infection after farm machine-related injuries in children and adolescents.

Infection played an important role in prolonging hospitalization and increasing morbidity in 68 children injured in farm settings. Predominantly gram-negative enteric organisms, group D streptococci, and anaerobic organisms were isolated in cultures of specimens obtained from wounds. Infection was more often associated with severe injuries of the large bones of the extremities than in amputation injuries of the digits and crush or roll-over injuries when the skin barrier was intact. The occurrence of infection in farm injuries was associated with prolonged hospitalization for parenteral antibi

Successful return to work following a musculoskeletal injury.

Successful return to work following a musculoskeletal injury is facilitated by early intervention and rehabilitation that addresses the physical, psychosocial, and environmental factors in recovery. Job simulation and work hardening programs that involve the employee, the employer, and the health care providers are an effective approach to injured worker rehabilitation. The occupational health nurse's role can include early identification of injury, treatment coordination and follow up, matching worker abilities and restrictions to the job, and implementation of an injury prevention program. [Ophthalmologic care of patients injured in accidents].

The authors analyze the results of treatment of patients whose organ of vision was injured in accident. They point out a complex of measures promoting a relatively favorable optic and cosmetic outcome of injuries and discuss the shortcomings of ophthalmologic service. Various aspects of the problem are considered to develop practical recommendations on effective care of the injured patients.

Evaluation of the injury profile of personnel in a busy urban EMS system.

The occupational injury profile of emergency medical technicians (EMTs) and paramedics is not well described. We retrospectively studied 254 injuries over a 3.5-year period in a busy urban EMS system. Low back strain was the most common injury (93/254, 36%), with EMTs suffering a significantly higher injury rate than paramedics (0.33 v 0.17 injuries/person-years at risk, P = .03). Lifting caused 58/93 (62.4%) of back injuries, and most occurred at the scene to which personnel were dispatched (58/93, 62.4%). The back injuries were recurrent in 31% of personnel. The data showed trends toward hig

Disability, direct cost, and payment issues in injuries involving woodworking and wood-related construction.

Treatment cost and payment patterns and disability for work, home, and recreation activities were examined for 495 consecutive persons seen as primary care patients over one year in Northern Vermont with injuries associated with woodworking, wood related construction, and home repair activities and materials. These primary care patients were from a larger sample of 601 persons with such injuries who received either primary or tertiary care. Mean total charges were $530 and $342 respectively for work and nonwork related injuries, with highest costs for back and arm injuries, and injuries involv

Maxillofacial fractures related to work accidents.

Work-related maxillofacial fractures were studied retrospectively over a six-year period. There were 98 cases accounting for 4.5% of all facial bone fractures treated in our hospital between 1981 and 1986. Of the patients, 89.8% were male. The mean age of the injured was 36.4 years. The incidence of work-related maxillofacial fractures was 0.37 per 1000 workers. Most of the injuries (66%) occurred in factories and construction work. Such work was associated with an to 15 times higher risk of maxillofacial fracture than service and office work. Of the fractures, 20.4% were sustained on the way

Pediatric injuries in the back of pickup trucks.

Travel in the back of pickup trucks has not been adequately addressed as an occupant protection issue. This study compares injuries sustained by children riding in the back of pickup trucks with those of children riding in the cab. Data were obtained from a multihospital monitoring system and the coroner in a single urban county. The series of injured children consisted of 290 children 0 through 14 years of age, 201 of whom had been riding in the cab and 89 in the back. Age distribution of the children demonstrated that it is most frequently the 10- to 14-year-olds who travel in the back. Chil

Identification of the driver in an automobile collision.

We report on a three-occupant automobile collision in which one of the two survivors claimed that the deceased had operated the vehicle. We attempted to identify the driver. The left face and neck of the deceased appeared to have struck the vehicle's interior. The survivors were slightly injured on the right side of their faces. In Japan, the passenger side is on the left, and in this case, the right side of the vehicle had been damaged. By comparing the injuries of the deceased and the survivors in relation to the vehicle damages, we concluded that the deceased occupant had probably been a pa

Simply osteoporosis--or multifactorial genesis for the increasing incidence of fall injuries in the elderly? The need for a scientific approach to reducing injuries.

Both the high and rising incidence of injuries in old age and a situation where accidents are looked upon as "super-human" events, impossible to prevent, call for a scientific approach to the problem. Another strong reason for stressing the scientific approach is the complex aetiological origin of injuries in the elderly. Individual factors as well as factors overrepresented in the injured population must find a developed model as site for their importance compared to agent factors and environmental factors. If not, there will be an everlasting contradiction between the discussion on monofacto

Head injury in the workplace.

Work related head injuries do not occur frequently in comparison to other injuries, but their importance lies in their relative severity, both in terms of work days lost and long term morbidity, making it important for the occupational health nurse to have knowledge of how to deal with the initial injury and residual problems. In head injury, most of the treatment is aimed toward prevention of hypoxia, airway obstruction, hypercapnia, hypotension, and bleeding. The Glasgow Coma Scale is used for the severely head injured person. The residual cognitive impairment, emotional disturbances, and be

The fraction of traffic fatalities attributable to alcohol.

The vast literature on alcohol's effect on traffic safety does not contain even a moderately satisfactory answer to one of the most basic questions, namely "What is the fraction of all traffic fatalities attributable to alcohol use?" A published estimate of 23.7% based on an erroneous calculation has been widely quoted. This paper combines 1987 Fatal Accident Reporting System (FARS) data from 26 states that recorded blood alcohol concentrations for over 84% of fatally injured drivers with published estimates on how alcohol affects crash risk. By categorizing all traffic fatalities as either no

[Abdominal injuries after blunt trauma].

We present a series of 331 patients admitted to hospital in 1980-87 with abdominal injuries after blunt trauma. The patients included 230 males and 101 females. The median age was 29 years. More than half of the patients were injured in traffic accidents. 11% were transferred to our Trauma Center from other hospitals, median five hours after the accident. A doctor-manned helicopter transported 52 patients (18%) directly to our hospital. 70% had extra-abdominal injuries as well. A minimum of 20% were intoxicated by alcohol and/or drugs. Severe injuries (AIS greater than 3) were present in 46%.

Safety at work for railway staff in Britain.

The recent series of railway accidents raised questions about the safety of British Railways. We have studied trends in accidents to railway staff over a 33-year period, 1954-86, which time has covered the pre- and post-modernization of the railways, in conjunction with large reductions in both staffing and traffic volume. The Railway Inspectorate, which has the duty of investigating and reporting on accidents occurring on railway premises, defines three kinds of accident: train, movement and non-movement. The numbers of railway staff killed and injured in each kind of accident have been studi

Manufactured housing plant injuries in a rural family practice.

The manufacture of mobile homes and prefabricated houses results in the second highest occupational injury and illness rate of any industry. The types of worker injuries sustained have never previously been characterized. This series of 138 injuries from a rural family practice categorizes the injuries sustained by workers in a manufactured housing plant. Most injuries were not judged to be serious, but did result in lost work time and morbidity. Forty-nine percent of all injuries involved lacerations or puncture wounds. Seventy-six percent of puncture wounds involved staple gun use. Two third

[Thoracic injuries in traffic accidents and non-traffic accidents].

Traffic accidents differ from non-traffic accidents where the injuries are concerned. With the males two thirds of the thoracic injuries happened in non-traffic accidents and one third in road accidents; with the females, the relation was reversed. In thirteen per cent of the traffic accidents and in twenty-five per cent of the non-traffic accidents there were only thoracic injuries, in the other cases there were multiple injuries. Blunt damages of the thorax occurred in the majority of both kinds of accidents, penetrating injuries were less common. Closed rib fractures happened twice as often

Impact of acute alcohol intoxication on the severity of injury: a cause-specific analysis of non-fatal trauma.

The impact of alcohol on the severity of injury was studied by using injured patients from an emergency room in Helsinki, Finland. Blood alcohol content was estimated either clinically or with a breath test and then was recorded using a three-grade intoxication code (Ethyl sign). The study population was comprised of 14,995 men aged 15-64 years. The Ethyl sign was positive in 19.7 per cent. Hospitalization was used as an indicator of the severity of injury. Hospitalization was almost as common among sober (9.3 per cent) as among intoxicated patients (9.6 per cent) and the odds of being hospita
