Over a period of 26 months, emergency medical contact was made with 63 sick or injured trawlermen from Hull and Grimsby who had been fishing in the fishing grounds around the Orkney Islands. The majority of accidents were due either to falls in heavy weather, entanglement with winching or fishing gear, or burns and scalds in the galley. Nearly 80 per cent of the episodes of illness which were seen might not have required the emergency service had the trawler crews been properly screened before going to sea, or trained in elementary first aid.

This article deals with some of the unique problems encountered when people, injured in road traffic or industrial accidents, are trapped as a result of the accident. Among nearly 2000 patients of a traumatological ward 82 patients were treated with chemical injury during 6 years. On the basis of their experiences the tissue damaging effect of the chemical substances as well as the methods of treatment are discussed by the authors. Blood alcohol was determined in 1012 injury victims. It was found that the alcohol involvement (AI) rate was 37% in the total number of patients, 19% in industrial, 38% in traffic, 36% in home, 45% in other freetime accidents, and 69% in the victims of fights, assaults, and suicide attempts. The article presents the results of re-examinations performed on 120 persons with spinal cord injuries due to industrial accidents. Comparisons were made between paraplegis and paraparetics with varyingly long histories of spinal cord damage. This paper reports an intensive follow-up study of persons injured while wearing seat belts in crashes. Examinations of both vehicles and persons were carried out after the crash, and 30 out of 54 (55.5%) persons were judged to have been wearing seat belts at impact. Eight received injuries from the seat belt, only four of these injuries being severe, and none was fatal. A roadside survey showed that half of the occupants wearing seat belts had them adjusted incorrectly. This compared with nearly 90% of the crash cases having incorrectly adjusted belts. World airline accident statistics for the period 1960 to 1970 indicate the number of seriously, but nonfatally injured is unlikely to exceed 25% of the occupant capacity of the largest aircraft using an airport. In only 1 accident out of 20 will this number be an underestimate. This estimate may be of value in medical planning for an airport disaster and in the assessment of loads on hospitals.

The author reports that relatively frequent injuries to the thorax and to the heart are established in autopsies of aircraft fatalities. A review of 128 autopsy protocols of aircraft accident fatalities revealed 111 cases of severe contact injuries to the thorax and 98 cases of traumatic injuries to the pericardium and the heart. Two cases of blunt trauma of the abdominal aorta are reported. In both there was lacreation of the intima and development of intimal flaps with only partial occlusion of the aorta.
