High-pressure injection injuries of the hand. A review.

High-pressure injection injuries to the hand are associated with severe morbidity if treated improperly. Injuries typically occur in the nondominant hand of patients using industrial equipment in the workplace. Tissue damage is produced by the delivery of both kinetic energy and foreign substances into the hand. The initial clinical course can be deceptively benign and lead to delay in instituting therapy. Early conservative management dramatically increases the likelihood that the injured part will eventually require amputation. Thorough examination for functional and structural impairment is

Injury in U.S. Army helicopter crashes October 1979-September 1985.

All U.S. Army class A and B mishaps of four types of helicopters occurring from 1 October 1979 through 30 September 1985 were reviewed. During this 6-year period, there were 298 crashes involving 303 aircraft. There were 1,060 individuals aboard the crashed aircraft and 611 were injured, 136 fatally. The most common cause of injury was the "secondary impact" caused by collapse of structure into occupied areas, by inadequate restraint of the occupants which allowed them to flail into structure, or by a combination of both mechanisms. Injury solely related to acceleration occurred infrequently.

Hazards of mountain flying: crashes in the Colorado Rockies.

Between 1964 and 1987, 232 airplanes crashed within 50 nautical miles of Aspen, CO; 90% were general aviation crashes. A total of 202 people died and 69 were seriously injured. The societal cost averaged more than $4 million annually. Most pilots were experienced and many were flight instructors, but 44% had flown less than 100 hours in the type of plane in which they crashed. Forty-one percent of the pilots were out-of-state residents. Crashes in the study area were more likely to be fatal than in the rest of Colorado. Airplanes with three or four occupants and low-powered four-seater aircraf

Invalidity of speculated injury mechanism in autopsy reports.

Postmortem and crash investigation reports were examined for 35 cases of belted automotive crash fatalities. This paper highlights those cases with speculation of the injury mechanism in the post mortem report. In two cases, the medical examiner made specific reference to a 'whiplash' mechanism of brain injury, which refers to an inertial loading of the neck without head contact. Examination of the car interior in one case indicated evidence of head contact with transfer of hair, and the other facial contact with transfer of teeth. Death was more likely due to direct head impact. In another ca

Flashbacks after traumatic hand injuries: prognostic indicators.

Flashbacks of a traumatic hand injury may compromise a patient's rehabilitation process. This study examined the nature and significance of these flashbacks in a work-injured population. We also evaluated the ability of these patients to return to work at the site of the original injury. Sixty-one patients with work-related, traumatic hand injuries received psychological evaluation and treatment. All patients experienced flashbacks. The following three types of flashbacks were identified: (1) a replaying of the events occurring just before the accident and continuing until the injury (replay f

Oil and petrol drum explosions. Injuries and casualties by exploding oil and petrol drums containing various inflammable liquids.

If ignited, an evaporated inflammable liquid remaining mixed with air in an oil or petrol drum may cause an explosion in which the top and bottom of the drum are blown off by the blast and act as projectiles causing extensive injuries to persons nearby. To analyse the occurrence of this type of accident and to study the injuries involved information was sought from all police districts in Denmark and all the departments of plastic surgery with a burns unit. The investigation revealed a total of 21 accidents caused by oil drum explosions over a period of 36 years with 16 injured within the last

Delayed primary reconstruction of subtotal nail bed loss using a split-thickness nail bed graft on decorticated bone.

Split-thickness nail bed grafting is the accepted method of treatment for injuries involving loss of nail bed tissue. The nail bed of the great toe may be used without donor-site morbidity, and nail bed grafting may be combined with other procedures for fingertip reconstruction. A case of fingertip avulsion injury with loss of the nail plate, nail bed, and periosteum over the exposed distal phalanx of the thumb was reconstructed by a split-thickness nail bed graft placed directly on granulating decorticated bone. The length, appearance, and function of the injured dominant thumb were preserved

Risk of fatality from physical trauma versus sex and age.

No large-scale epidemiologic study to investigate how sex and age affect fatal trauma risk from the same impact has been possible previously because large numbers of people are rarely subject to sufficiently similar major physical insults. This paper describes such a study, made possible by two recent developments: first, the availability of a large data file, the Fatal Accident Reporting System, which gives information on more than one third of a million people fatally injured in traffic crashes; second, a new technique, the double-pair comparison method, which, by focusing on vehicles contai

Patterns of injury in helmeted and nonhelmeted motorcyclists.

In the present study, the incidence of severe brain injury was 600 percent higher for patients riding without a helmet and the incidence of all brain injuries was nearly twice as high in the nonhelmeted riders. All surviving patients with severe brain injury sustained residual long-term disability. The incidence of injury and death was much higher for motorcyclists than for occupants of automobiles involved in accidents. Riding a motorcycle is dangerous and riding without a helmet is fool-hardy. Helmets also protect the face, as facial fractures were twice as common in the nonhelmeted riders.

[Presence of benzodiazepines in the serum of patients after accidents. Relation to alcoholism].

In a study of 2.021 injured persons (accidents at home or at work, road accidents), qualitative assays of serum benzodiazepines by the EMIT method were positive in 9.6 per cent of the cases, including 3.2 per cent who had blood alcohol levels in excess of 0.10 g/l. Benzodiazepines were more frequently found in accidents at home and on roads than in accidents at work. These results raise the question of the role played by benzodiazepines as a possible risk factor of accidents, notably road accidents.

Medico-economic implications of industrial hand injuries in India.

625 five consecutive cases of industrial hand injuries attending the Employee's State Insurance Hospital, Jaipur, have been studied from 1983 to October 1986. The incidence of injuries was 36 per 10,000 workers per year. 47% were due to entrapment of the hand in active machines, 25% occurred during lifting and transportation of heavy objects and 12% while handling tools. The injuries resulted in residual deficit in 55% of cases and were serious enough to require absence from work of more than four weeks in 48% of cases. On an average 35 days were lost per injured worker. The average economic l

Trauma and the veterinarian.

A survey of all American Veterinary Medical Association members in Minnesota and Wisconsin was conducted by questionnaire to document injuries resulting from animal treatment. Of 995 respondents, 64.6% had sustained a major animal-related injury. Seventeen per cent were hospitalized within the last year. Of those hospitalized, 25.3% required a surgical procedure. Hand injuries were most common in a veterinarian's career (52.6% of respondents), followed by trauma to the arms (27.6%), and the head (20.8%). The thorax (8.3%), genitalia (3.9%), and intra-abdominal viscera (2.8%) were injured less

Duration of chronic pain and the Minnesota Multiphasic Personality Inventory: profiles of industrially injured workers.

The present study explored the usefulness of the Minnesota Multiphasic Personality Inventory (MMPI) in understanding the relationship between duration of chronic pain and psychiatric difficulties. The MMPI responses of workers' compensation patients with varying levels of low back pain chronicity were compared. One hundred ninety eight patients, divided into three groups according to length of disability, underwent social history interviews and completed the MMPI. In regard to the overall profile, the sample had elevations on the Hypochondriasis, Depression, and Hysteria scales. A series of an

Occupational motor vehicle injury morbidity among municipal employees.

Injury related to motor vehicle use among Baltimore City employees was examined throughout a 34-month period. Among an average of 30,000 municipal employees; representing 7% of the total Baltimore work force, almost 2% of all city employees were injured by motor vehicles each year. Twelve percent of all injuries and 16% of the injury costs were attributable to motor vehicles. In the police department, 43% of all costs were vehicle-related. Compared with other departments, the rate of motor vehicle-related injury was more than 4 times as high in the department of public works and twice as high

Mutilating corn-picker injuries of the hand.

Our experience with 15 patients who had acute mutilating hand injuries from corn-picker machines is reviewed. Three injury patterns were defined. Type I involved severe injury to all digits of the hand, including the thumb; type II injuries involved amputation of all fingers, in whole or in part, but with sparing of the thumb; and type III injuries involved amputation of multiple fingers with sparing of the thumb and at least one other digit. Attempts at revascularization or at complex reconstruction of the injured digits in the period immediately after injury failed in all 15 patients. The mo

The long-term effects of a token economy on safety performance in open-pit mining.

A token economy that used trading stamps as tokens was instituted at two dangerous open-pit mines. Employees earned stamps for working without lost-time injuries, for being in work groups in which all other workers had no lost-time injuries, for not being involved in equipment-damaging accidents, for making adopted safety suggestions, and for unusual behavior which prevented an injury or accident. They lost stamp awards if they or other workers in their group were injured, caused equipment damage, or failed to report accidents or injuries. The stamps could be exchanged for a selection of thous

Air bags: reducing the toll of brain trauma.

Motor vehicle crashes account for approximately one-half of all hospitalized patients with brain injury. Therefore, measures to reduce the frequency and severity of injuries from motor vehicle crashes have the potential for making a substantial impact on the incidence and severity of brain trauma. Occupant restraints, including seat belts and air bags, have been proven highly effective in preventing injuries, yet the specific benefits for the brain, the face, and the cervical spine provided by air bags have not been widely publicized. Air bags prevent the violent whiplash motion of the head in

Effects of seat belts and injuries resulting from improper use.

A survey was made of car crashes in Japan of injuries to seat belt users compared to nonusers. The purpose of the study was to clarify the relationship between the types of injuries and the way the belts were used at the time of the crash. Eight hundred individuals were involved in the study. The data indicate that those using seat belts sustained less severe injury than nonusers. Of the belted occupants who were seriously injured or killed, it appears that their injuries were due to improper placement of the seat belts on the anatomy. In some situations the precrash position of the occupant a
