Air bags: reducing the toll of brain trauma.

Motor vehicle crashes account for approximately one-half of all hospitalized patients with brain injury. Therefore, measures to reduce the frequency and severity of injuries from motor vehicle crashes have the potential for making a substantial impact on the incidence and severity of brain trauma. Occupant restraints, including seat belts and air bags, have been proven highly effective in preventing injuries, yet the specific benefits for the brain, the face, and the cervical spine provided by air bags have not been widely publicized. Air bags prevent the violent whiplash motion of the head in

Anatomic location of spinal cord injury. Relationship to the cause of injury.

In a population based sample, 550 of 619 people with fatal and nonfatal spinal cord injuries were classified according to the cause of injury and according to the anatomic location of the highest spinal cord injury. Motor vehicle occupants accounted for 42% of the people with spinal cord injuries. Sixty percent of those injured had a cervical cord injury, 25% a thoracic cord injury, and 15% a lumbosacral cord injury. The specific anatomic locations most commonly injured were at C1 (12% of all spinal cord injuries), C6 (9%), and L2 (5%). The anatomic site of injury was significantly (P less tha

Maxillofacial trauma in fatally injured victims of motor vehicle accidents.

A survey of 84 victims of maxillofacial injuries sustained in road traffic accidents is presented. The material was based on the files of the Finnish National Board of Traffic Accident Investigation of the Insurance Companies, comprised the years 1972-1983 and covered the whole country. 86% of the victims were occupants of motor cars; only 24% had worn seat belts. Similarly only two of the eight motor-cyclists had worn a helmet. The majority of all victims also had other head injuries but only 24% had brain injuries; over 50% had a chest injury. In 20 cases the facial injury was thought to be

Data on spinal injuries--Part I. Collection and analysis of 352 consecutive admissions.

A prospective system of data collection, using computer storage, has been developed in the Spinal Injuries Unit, Austin Hospital. Three hundred and fifty-two consecutive admissions to the Unit between 1 July 1978 and 31 December 1982 are analysed, and this epidemiological data compared with other reports. Over 92% of the admissions were the result of spinal cord trauma and the balance due to a variety of non-traumatic causes. A high incidence of young males injured in road accidents, predominantly from motor cars, and lesser numbers from sporting, occupational and domestic accidents is noted.

Discrepancies in vehicular crash injury reporting: Northeastern Ohio Trauma Study. IV.

People injured in motor vehicle traffic crashes were identified from a population-representative incidence sample of hospital emergency department visits. Matched police reports of crashes were sought in official state records of motor vehicle traffic crashes. Of the emergency department cases, 55% had matched police reports. The frequency of matched reports was highest for drivers (74%), people transported to the hospital by emergency vehicle (69%), and those requiring hospital admission (74%). The frequency was lowest for people younger than 16 years (28%), people injured as occupants of veh

Burns caused by bromine and some of its compounds.

Our experience with infrequent and unfamiliar types of injury caused by various compounds of bromine are described and discussed. While bromine containing compounds share some of the characteristics of other common causes of chemical burns, they also have a specific, exclusive mode of affecting the skin. The delay in the appearance of clinical signs and symptoms is an important factor to be considered by those who treat injuries caused by bromine or some of its compounds, or by those who may be the first to see persons who are injured by these agents. As most of the injuries occur at work (92

An analysis of mooring accidents on the Polish Ocean Lines ships in 1975-80. Preventive recommendations.

The aim of the presented research was: 1. to analyse the causes and effects of accidents that occurred on the Polish Ocean Lines ships in 1975-1980 at mooring manoeuvres. 2. Issuing certain prophylactic recommendations. The material of the research was information contained in the 95 accident record cards and in other post-accident documents such as rulings of the Marine Chamber, situational sketches of the place of accident and determination of circumstances and causes of accidents. The obtained data showed, among others, that c. 81 per cent of the mooring accidents occurred at the bow manoeu

Facial injuries in automobile crashes.

In automobile accidents, the facial area is the most frequently injured body region in passenger car occupants. Laboratory studies have indicated that the tolerance of facial bones to impact is relatively low. Most of these facial injuries are rated as minor. The windshield, steering wheel, and instrument panel are the major points of contact. Restraints, lap belts, and lap-shoulder belts reduce the frequency of facial injuries at all levels of severity and also reduce the more severe and serious injuries to other body regions.

The incidence of hospital-treated facial injuries from vehicles.

The annual incidence rate of facial injuries from vehicle crashes, 278 per 100,000 residents, was determined from a population-based study involving all Dane County, Wisconsin, hospitals with emergency departments. Applying this figure to the U.S. population yields an estimated 625,000 hospital-treated facial injuries from vehicles occurring in the United States each year. Vehicle crashes were the source of a substantial proportion of facial injuries from all causes, and were found to be the single leading cause of the most severe facial lacerations and facial fractures. The majority of injuri

Grain auger injuries: the replacement of the corn picker injury?

Corn picker injuries, although more infamous, are less frequent than those produced by the grain auger. The former rarely occurs in other than adult males who are responsible for their carelessness, whereas the grain auger injury frequently involves innocent and uninformed youths. We report 4 arms, 4 forearms, 5 hands, 24 digits, 2 legs, 4 feet, and 4 toes injured by a grain auger in 24 patients. Multiple-level injuries accounted for the larger number of injuries than patients. Twelve amputations or devascularizations were reattached or revascularized successfully. Not all amputations had atte

The case of mandatory seat restraint laws.

Motor vehicle accidents are the leading cause of death and injury to children over one year of age in the United States. About 800 motor vehicle occupants less than four years old are killed each year and tens of thousands are injured severely enough to require emergency room treatment. Studies have demonstrated that the vast majority of children ride unprotected in motor vehicles. Restrained children are likely to have milder injuries or no injuries at all. One study intimates that seat belts or restraints could reduce injuries up to 70 per cent and deaths up to 90 per cent. Various education

Accidents and injuries in child vehicle occupants in New Zealand.

Records of reported child vehicle occupant fatalities and injuries were examined. Child vehicle occupants accounted for approximately 5 percent of all road injuries and deaths in 1979, with an approximate hospital cost of one million dollars per year. It is thus worth spending considerable sums of money on countermeasures. The problem of parents' perception of risk is illustrated by the observation that annually only one child in 1 000 is injured and one in 50 000 dies as an occupant of a vehicle. International comparisons show New Zealand child occupant accident rates per 100 000 population t

Distraction-fixation in the primary treatment of metacarpal bone loss.

Nine patients seen in civilian practice with severe open injuries of the hand, including loss of portions of some or all of the metacarpals, were treated by primary restoration of metacarpal length and alignment by distraction-fixation with the use of transverse intermetacarpal Kirschner wires. An external fixation device was added in two of the nine patients. Severe associated soft tissue injuries were present in all but one patient. Two injuries were caused by firearms, and the other seven by heavy machinery. Contractures of the joints were prevented by the use of a second set of wires to po

Chain saw injuries: review of 330 cases.

Three hundred thirty chain saw injury patients arriving at the emergency clinic of a community hospital in the southeast United States between 1972 and 1979 were reviewed. Forty-eight patients were admitted to the hospital with severe injuries requiring extensive repair; another was dead on arrival at the emergency clinic. The retrospective study reveals a typical pattern in chain saw injuries as injuries caused by other power tools have typical patterns: the majority of the injuries occur on the left side. Most injuries are caused by saw kickback, upward or downward; the face (not usually eye

Blood alcohol levels in a series of injured patients with special reference to accident and type of injury.

Blood alcohol was determined in 1012 injury victims. It was found that the alcohol involvement (AI) rate was 37% in the total number of patients, 19% in industrial, 38% in traffic, 36% in home, 45% in other freetime accidents, and 69% in the victims of fights, assaults, and suicide attempts. The most common external cause of injury was falling (447 cases) with an AI rate of 38%, while the 172 other nontraffic true accident cases had a rate of 24%. The road and stairs were the usual places of falls while intoxicated. Head injuries had a high (47%) and upper extremity injuries a low (25%) AI rat

Nonpenetrating trauma of the abdominal aorta with partial vessel occlusion: report of two cases.

Two cases of blunt trauma of the abdominal aorta are reported. In both there was lacreation of the intima and development of intimal flaps with only partial occlusion of the aorta. Immediate recognition in one allowed prompt repair of injured tissue. Delayed recognition in the other, while not life-threatening, did prolong disability and did necessitate vascular tissue replacement with prosthetic graft. Audible bruit and palpable thrill are diagnostic signs that, if sought for in the emergency room and operating room examinations of the traumatized abdomen, will yield early recognition of this

Emergence of occupational medicine in Victorian times.

Lee, W. R. (1973).British Journal of Industrial Medicine,30, 118-124. Emergence of occupational medicine in Victorian times. The events surrounding the establishment and development of legislation to protect the health of people at work in Victorian times are already well documented. This paper deals with some other aspects of the development of occupational medicine. Medical opinions at the time did not always see the misuse of child labour as due simply to avaricious mill owners, but in part due to the parents and in part to the workmen subcontractors. The establishment of the certifying sur

Assessment of a lower extremity training program.

The response to a lower extremity program has been studied in workmen undergoing progressive rehabilitation following injury to the lower leg or ankle region. When first seen, the isometric strength of the injured limb was substantially reduced relative to its counterpart, and aerobic power was also poor, relative to normal standards. Substantial gains of isometric strength occurred over a four- to six-week period of therapy. Measurements above the site of injury suggested a 13% gain of strength, and measurements below the injury indicated a gain of 34%. On the other hand, there were minimal c
