On December 14, 1985, the Teller chairlift at the Keystone, Colorado, ski area collapsed, throwing 60 of the 372 people aboard to the ground from heights up to 50 feet. Initial triage and management of the victims was carried out by the local ski patrol, the on-duty physician at the area's Snake River Health Services Clinic, and by volunteer physicians and nurses present at the scene. Thirty-three people required immediate evacuation to hospitals, most of them being transported 75 miles by helicopter air ambulance to level I and II trauma centers in the Denver metropolitan area. Eighteen of th

Motor vehicle accidents are a common cause of death for all age groups but are the most serious threat to life for patients aged one to 34 years. Major progress has been made in reducing these losses through improved highway design, improved laws, adjudication of laws controlling the drunk driver, and the advent of vehicle safety design standards. A federal law enacted in 1966 established the National Highway Traffic Safety Administration (NHTSA) and the Federal Motor Vehicle Safety Standards (FMVSS). Motor vehicles, particularly passenger cars, now provide effective protection for occupants b

Motor vehicle crashes account for approximately one-half of all hospitalized patients with brain injury. Therefore, measures to reduce the frequency and severity of injuries from motor vehicle crashes have the potential for making a substantial impact on the incidence and severity of brain trauma. Occupant restraints, including seat belts and air bags, have been proven highly effective in preventing injuries, yet the specific benefits for the brain, the face, and the cervical spine provided by air bags have not been widely publicized. Air bags prevent the violent whiplash motion of the head in

EDT can be successfully performed with the proper system in place. This includes an established thoracotomy protocol, a well-integrated EMS system, and an in-house team. Time seems to be critical, and the time between injury and EDT may be the single most important factor affecting survival other than the mechanism of injury. Cardiac penetrations, especially stab wounds, were found to have a 93 per cent survival while subdiaphragmatic penetrations had only one survivor from a group of 18 patients (5.5%). The high rate of salvage in the heart wound group probably reflects the speed of prehospit

Data on hazardous material releases that occurred between January 1, 1982 and September 30, 1983 in California were obtained from the California Highway Patrol (CHP) and the US Department of Transportation (DOT). The majority of incidents involved highway transport of hazardous materials, although some information was available on air, rail, and stationary facility releases. Vehicle accidents and failure of or damage to the container were the most frequent causes of releases. Proportionately more hazardous materials incidents occurred in early summer than at other times of the year, during wee

Over one half of all neurotrauma occurs in or by motor vehicles. The other causes of neurotrauma are falls, violent assaults, sports and recreation, and firearms. The victims who survive are left with paralysis, disfigurement, loss of sight and hearing, seizures, epilepsy, psychiatric disorders, amnesia--in short, significant and often permanent impairment. To permanently prevent this annual human destruction, the prevention agenda should include advocating for air bags in all cars, helmet use for all motorcyclists, the elimination of handguns, and the regionalization of all trauma care. After

Personnel records of over 1000 welders and electricians but only 235 caulkers and 557 platers employed at a shipyard in NE England between 1940 and 1968 were obtained and the mortality followed up to December 1982. The observed number of deaths (13 from mesothelial tumours, nine among the electricians) were compared with the number to be expected in the Newcastle connurbation. Welders and caulkers were most exposed to welding fumes, electricians to asbestos. The study was limited by the lack of accurate job exposure details, and there was no record of smoking habits, but welders and caulkers s

During the course of dockside ship maintenance, a compartment was partially flooded with tricholotrifluoroethane gas. One sailor entered the compartment, collapsed, and was then rescued by two other men. All three victims then climbed a 11-m (36-ft) ladder and collapsed. They all experienced a rapid development of cardiac arrest. We report on the pathologic, toxicologic, and pathophysiologic aspects of the incident.

Human error causes or contributes to considerably over half of all aviation mishaps. This report describes a 7-year study of aircraft incident data conducted in an attempt to further our understanding of the phenomenon of human error. The study of incidents as a surrogate for aircraft accidents is relevant only if incidents constitute a population or universe of which accidents are a subset. This assumption has been examined in a study of over 35,000 reports of aviation incidents collected from 1976-83 by the NASA Aviation Safety Reporting System. One-third of the reports involve conflicts amo

Army Air Corps accident and fatality rates have now reached levels which compare favourably with data from other civilian and military sources. This improvement is the result of enhanced helicopter design and parallel progress in aircrew training. The introduction of new generations of turbine powered rotor craft has largely eliminated mechanical failure as the cause of accident. As a result 75% of Army Air Corps accidents are due to pilot error. This contribution is likely to increase in the future as the pilot's task is made more difficult by the incumberance of personal equipment. Methods w

Aviation medicine has researched many important facts on pilots, but little on direct relationships between controllers, aviation medicine and air safety. The unsuspecting flying public accepts a 'blind faith' in aircraft and pilots, unaware that aircraft are controlled within 'suspect' ATC systems. The deceptive simplicity of controlling air traffic in apparently limitless skies belies the complexity of man-machine ATC systems operated in ever-crowded airspace, sometimes with antiquated equipment and indifferent communications. The indivisible operational controller/pilot team strives to meet

Several risks can be posed by travel, even when the stay is not protracted nor the destination exotic. Air travelers should consider the potential problems associated with decreased cabin pressure, hypoxia, and circadian dysrhythmia, problems that can contraindicate flying for some. Motion sickness can affect those traveling by air, sea, or land; medications are available to combat the symptoms, and several other remedies such as changing visual stimuli are also worth trying. Persons staying in hotels, particularly high rises, should review several general guidelines for defensive action in ca

Automobile accidents are the most frequent cause of injury and death among young children. The number of injuries or deaths could be decreased by the use of child restraints. Counseling parents about car safety has a positive effect on increasing the use of restraints. Other important factors include passive protection, such as interior design and air bags, appropriate laws for enforcement of proper use and adequate safety testing of existing restraints.

Ricardo T., aged 20 years, was a passenger in a Varig air-craft which suffered an accident on July 11th 1973. A fire developed on board whilst the plane was in the air, at about 3 p.m. After emergency landing, rescue workers (firemen and emergency ambulance teams) discovered only two survivors amongst the 122 passengers. One was in his forties and died a few moments later. The other (Ricardo) was rapidly transported to the surgical and traumatological intensive care unit of the Henri Mondor Hospital. At the time of arrival, he was in stage 1 coma with skin burns (3rd degree), burns of the airw

The dynamic impact tests at Southwest Research Institute for the first time exposed human volunteers to production-like driver air cushion system depolyments at impact levels equivalent to a 30 mph barrier crash (48 kph). No significant injury was produced. At no time was it necessary for the secondary restraint systems to be utilized and the entire program schedule was carried out as planned. All tests were conducted under the most safe and controlled conditions possible, and typify only what the air cushions might accomplish in direct, head-on impacts up to 30 mph (48 kph). There is a need for a more effective method for the disinsection of intercontinental aircraft. A study was made of the possible toxic hazard associated with a new method of disinsection using DDVP vapour (O,O-dimethyl-2,2-dichlorovinyl phosphate) as the insecticidal agent. In these experiments, men and monkeys were exposed four times over one- or two-hour periods for a total of 4-8 hours to DDVP vapour in a simulated aircraft cabin. The concentration of DDVP was higher and the exposure periods were longer than those planned for use in disinsection. Concentrations up to 0.7 mug per litre of air
