National Database Study of the effect of mode of transportation on trauma patients injured in snowmobile accidents.

BACKGROUND: Snowmobile accidents occur in areas that may be difficult to reach by ground Emergency Medical Services (GEMS) vehicles. Helicopter EMS (HEMS) can potentially be useful in these situations, but there are few available data assessing HEMS-associated survival improvement in patients injured in snowmobile accidents. This study evaluates the national trauma database (NTDB) to identify snowmobile-related injury cases to determine if HEMS transport was associated with improved outcome, and whether any improvement in outcome is associated with worse functional status as indicated by dispo

Independence of the mortality of severely injured patients from types of transport and hospital level in a well-developed trauma network.

INTRODUCTION: Despite over 50 years of research, trauma remains a significant global health issue. In addition to medical advancements, establishing trauma networks appears to positively impact the survival rates of severely injured patients. The influence of the type of transport (helicopter emergency medical service [HEMS] vs. ground emergency medical service [GEMS]) on mortality depending on the destination hospital in an established trauma network is currently unclear. The objective of the study was to evaluate this line of questioning within the context of an entire trauma network. MATER

Why do some trauma patients die while others survive? A matched-pair analysis based on data from Trauma Register DGU®.

PURPOSE: The mortality rate for severely injured patients with the injury severity score (ISS) ≥16 has decreased in Germany. There is robust evidence that mortality is influenced not only by the acute trauma itself but also by physical health, age and sex. The aim of this study was to identify other possible influences on the mortality of severely injured patients. METHODS: In a matched-pair analysis of data from Trauma Register DGU®, non-surviving patients from Germany between 2009 and 2014 with an ISS≥16 were compared with surviving matching partners. Matching was performed on the basis of

Mortality and hospitalisation in the Danish Helicopter Emergency Medical Service (HEMS) population from 2014 to 2018: a national population-based study of HEMS triage.

OBJECTIVE: To describe characteristics and outcomes for patients where the Danish Helicopter Emergency Medical Service (HEMS) either transported the patient to hospital, treated the patient on scene but did not transport the patient or was dispatched but cancelled en route to the patient (aborted mission), and to assess the field triage by comparing these outcomes. DESIGN: National population-based study. SETTING AND PARTICIPANTS: HEMS dispatches are undertaken from the five Danish emergency dispatch medical centres according to national guidelines. The study analysed all primary missions wi

Reduced Mortality in Severely Injured Patients Using Hospital-based Helicopter Emergency Medical Services in Interhospital Transport.

Recent evidence has demonstrated the survival benefits of helicopter transport for trauma patients. The purpose of this study was to evaluate the effectiveness of hospital-based helicopter emergency medical services (H-HEMS) in comparison with ground ambulance transport in improving mortality outcomes in patients with major trauma. Study participants were divided into 2 groups according to type of transport to the trauma center; that is, either via ground emergency medical services (GEMS) or via H-HEMS. The study was conducted from October 2013 to July 2015. Mortality outcomes in the H-HEMS gr

Pediatric Blunt Neck Trauma Causing Esophageal and Complete Tracheal Transection.

BACKGROUND: Blunt injuries to the cervical trachea remain rare but present unique and challenging clinical scenarios for prehospital providers. These injuries depend on prehospital providers either definitively securing the injured airway or bridging the patient to a treatment facility that can mobilize the necessary resources. CASE SUMMARY: The case presented here involves a clothesline injury to a pediatric patient that resulted in complete tracheal transection and partial esophageal transection. Ground and air prehospital providers utilized a stepwise approach to this airway injury and ach

[Rescue procedures in the major trauma of upper extremities - the role of the Polish medical air rescue in the therapeutic process].

BACKGROUND: Due to the growing use of various types of industrial and agricultural machinery, occupational accidents are among the most serious ones and quite frequently result in the permanent posttraumnatic disability of the injured person. In Poland, a replantation service has been operating since 2010. Each day, one out of six centres provides emergency replantation service accepting amputation calls from across the country. Patients qualified for replantation often need to be transported from places located even several hundred kilometres from the target hospital. MATERIAL AND METHODS: T

Experience of pre-hospital treatment of survivors of falls-related trauma by an Australian helicopter emergency medical service.

BACKGROUND: Greater Sydney Area Helicopter Emergency Medical Service (GSA-HEMS) operates a doctor and paramedic team providing pre-hospital and inter-hospital retrieval. Falls are an important cause of morbidity and mortality among trauma patients. In NSW, patients injured by falling comprise 38% of those with serious to critical injuries (ISS>15). The mortality of falls in this group is 15.2%, higher than the mortality rate for other common injury mechanisms. Mortality rate for high falls (>5m) is similar to that of low/medium falls. AIMS: The primary aim was describe the basic demographics,

Blast lung injury in a 20-year-old man after a home explosion.

A large family home exploded after a propane leak ignited. Initial reports from the scene noted that 11 people were injured, with many sustaining critical injuries. Immediately, multiple helicopter emergency medical services aircraft were dispatched to respond to the scene, and ground emergency medical services (EMS) providers were en route. Of the five aircraft requested, only two were available to respond; one aircraft was out for maintenance, and two others were committed to other missions.

Factors at the scene of injury associated with air versus ground transport to definitive care in a state with a large rural population.

BACKGROUND: Once emergency medical services (EMS) personnel decide to transport a trauma patient directly to definitive care, the next key decision at the scene of injury is whether to transport by air or ground. OBJECTIVE: The aim of this study was to identify factors at the scene of injury that are associated with this decision. METHODS: All trauma patients transported directly to a level I or level II trauma center by either air or ground EMS over a four-year period were selected from the Oklahoma State Trauma Registry. Initial scene vital signs, Glasgow Coma Scale score (GCS), injury mec

A 6-year analysis of German emergency medical services helicopter crashes.

BACKGROUND: Annually, there are about 80,000 helicopter emergency medical services (HEMS) missions for patients in Germany. In comparison to the commercial transport of passengers, the risk of aviation crashes increases during rescue operations. The aim of this study was to evaluate helicopter crashes related to HEMS in Germany within a 6-year period (1999-2004) and to analyze contributing factors. METHODS: Flight crashes were identified in the annually published flight crash reports of the Federal Agency for Flight Accident Investigation. Data were completed by telephone interview of the ope

Helicopter rescue and prehospital care for drowning children: two summer season case studies.

BACKGROUND: Drowning is the second leading cause of unintentional injury-related death in children <14 years of age and is one of the most important causes of accidental injury between the ages of 1 and 4 years. In this study, the characteristics of non-fatal unintentional drownings in a small series of pediatric victims were examined. METHODS: We retrospectively analyzed data collected by the rescue team from May to October in two consecutive years (2006, 2007). RESULTS: Nine accidents occurred in public waters, while 5 occurred in lakes and rivers. The submersion time reported ranged from

Midair collisions in U.S. civil aviation 2000-2004: the roles of radio communications and altitude.

BACKGROUND: Midair collisions are destructive to aircraft and often fatal to occupants, with the additional possibility of death and destruction on the ground. We sought to determine the characteristics of civil aviation mid-air collisions in U.S. airspace with focus on the altitudes at which the collisions took place and radio communications prior to the collisions. METHODS: Accident reports published by the National Transportation and Safety Board for the period 2000-2004 were analyzed using descriptive statistics. Additional information was extracted from the narrative report of each accid

The Pope Air Force Base aircraft crash and burn disaster.

This report describes the initial hospital and burn center management of a mass casualty incident resulting from an aircraft crash and fire. One hundred thirty soldiers were injured, including 10 immediate fatalities. Womack Army Medical Center at Fort Bragg, North Carolina, managed the casualties and began receiving patients 15 minutes after the crash. As a result of repetitive training that included at least two mass casualty drills each year, the triage area and emergency department were cleared of all patients within 2 hours. Fifty patients were transferred to burn centers, including 43 pa

Trauma care systems in Spain.

Trauma care systems in Spain are provided by the Nacional Health Service in a decentralized way by the seventeen autonomous communities whose process of decentralization was completed in January 2002. Its organisation is similar in all of them. Public sector companies of sanitary emergencies look after the health of citizens in relation to medical and trauma emergencies with a wide range of up to date resources both technical and human. In the following piece there is a description of the emergency response teams divided into ground and air that are responsible for the on site care of the pati

[Pediatric prehospital trauma care. A retrospective comparison of air and ground transportation].

OBJECTIVE: In contrast to prehospital care of adult trauma victims, prehospital care providers have only limited clinical experience of pediatric trauma cases as these are relatively infrequent. Literature reports on prehospital pediatric trauma care given by paramedics are frequently found in the literature, but there are few publications analyzing the quality of prehospital trauma care provided by emergency physicians in the care of injured children. It was the goal of this study to analyze the prehospital care of the pediatric trauma victims transported to a trauma center by physician-staff

[Helicopter use in the Swiss air rescue service for children. Inland transport in 1992].

In 1992, the Swiss helicopter rescue service (REGA) transported 515 injured and 141 sick children (total n = 656). More than 60% of the children were boys; the age group from 10 to 16 years dominated. Primary care was provided in 415 of the flights, whereas the remaining cases were interhospital transfers to institutions with pediatric intensive care units. The main reason for primary interventions was sports accidents, followed by medical disease and traffic accidents. The majority of the sick children (70%) were severely ill with life-threatening diseases according to National Advisory Commi

The role of a regional trauma system in the management of a mass disaster: an analysis of the Keystone, Colorado, chairlift accident.

On December 14, 1985, the Teller chairlift at the Keystone, Colorado, ski area collapsed, throwing 60 of the 372 people aboard to the ground from heights up to 50 feet. Initial triage and management of the victims was carried out by the local ski patrol, the on-duty physician at the area's Snake River Health Services Clinic, and by volunteer physicians and nurses present at the scene. Thirty-three people required immediate evacuation to hospitals, most of them being transported 75 miles by helicopter air ambulance to level I and II trauma centers in the Denver metropolitan area.
