Comparison of type and severity of major injuries among undertriaged and correctly triaged older patients.

BACKGROUND: Despite the use of prehospital triage algorithms and other trauma scoring methods, a substantial proportion of older patients with life-threatening injuries are undertriaged, increasing the risk of preventable death and disability. STUDY OBJECTIVE: The primary objective of this study was to describe the injury types, injury severity, and short-term survival outcomes of undertriaged older adults injured in motor vehicle collisions, compared to a group of correctly triaged older adults. METHODS: This secondary analysis of records extracted from the National Automotive Sampling Syst

Fungus cerebri (brain fungus): a rare complication of mastoidectomy.

Transfusion‑related acute lung injury (TRALI) is a life‑threatening disease caused by blood transfusion. However, its pathogenesis is poorly understood and specific therapies are not available. Experimental and clinical studies have indicated that alveolar fibrin deposition serves a pathological role in acute lung injuries. The present study investigated whether pulmonary fibrin deposition occurs in a TRALI mouse model and the possible mechanisms underlying this deposition. The TRALI model was established by priming male Balb/c mice with lipopolysaccharide (LPS) 18 h prior to injection of an a

Searching for ski-lift injury: an uphill struggle?

Injuries arising from ski-lift malfunction are rare. Most arise from skier error when embarking or disembarking, or from improper lift operation. A search of the literature failed to uncover any studies focusing specifically on ski-lift injuries. The purpose of this study was to identify and characterise ski-lift injury resulting in hospitalisation and comment on barriers to reporting and reporting omissions. New Zealand hospitalised injury discharges 2000-2005 formed the primary dataset. To aid case identification these data were linked to ACC compensated claims for the same period and the da

Injury risks and socioeconomic groups in different settings. Differences in morbidity between men and between women at working ages.

BACKGROUND: It is unclear whether greater injury risk in lower socioeconomic groups at working ages is attributable to differences in work conditions or a reflection of a wider overall pattern of risk. The current study investigates socioeconomic differences in non-fatal injury risks in a variety of settings. METHODS: Data were taken from a community-based injury register built up over one year (November 1989 to October 1990) in a semi-urban Swedish municipality (256,510 inhabitants), and then linked by record to Sweden's National Population Register (based on the census of 1990). Injuries am

[Evaluation of the quality of mortality information by unspecified accidents and events of undetermined intent].

INTRODUCTION: Mortality data are important for monitoring violence, making it possible to assess the trends and the impact of interventions towards its reduction. The objective of the study is to assess the quality of the filling out and codification of the death certificates for unspecified accidents and events of undetermined intent in the city of S. Paulo in 1996. METHODS: Death certificates on which the underlying cause of death (UCD) given was an unspecified accident (ICD-10 X59) or an event of undetermined intent (ICD-10 Y10-Y34) were selected for investigation at the Legal Medicine Ins

Proportionate mortality among unionized construction ironworkers.

This report presents the results of proportionate mortality ratios (PMR) and proportionate cancer mortality ratios (PCMR) among 13,301 members of the International Union of Bridge, Structural, and Ornamental Ironworkers who had been members for a minimum of 1 year, were actively paying dues into the death beneficiary fund, and had died between 1984-1991. Using the United States proportionate mortality rates as the comparison population, statistically significant elevated risks, using 95% confidence intervals (CI), were observed for several types of injuries: falls (N = 259, PMR = 3.57, CI = 3.

[Medical first aid and the transportation of burn patients].

The new trends in delivering emergency medical aid and transportation of burnt patients are outlined. The successiveness of emergency measures on the site of accident is specified, namely: securing patency of the airways, spontaneous respiration, free venous return for maintaining circulation. No local treatment of wounds is done. The need of transportation to specialized centers for treatment of burns throughout the country (4 units available) is substantiated. It is emphasized that adequate treatment, both infusional and operative, may be effected only by the specialized teams in such center

Unspecified injuries on death certificates: a source of bias in injury research.

Protective gear (for example, helmets and bulletproof vests) shields certain body regions from damaging energy. Failure to specify on death certificates the body region and nature of fatal injuries compromises the utility of mortality data for epidemiologic or prevention research. Of fatally injured California motorcyclists, 41% had no specific injuries listed on their death certificates in 1988. To examine the implications of this problem, the authors abstracted 186 coroner's or medical examiner's reports from four California counties with over 60% nonspecific injuries and one county with few
