An assistance device to help people with trunk impairment maintain posture.

People with trunk impairment cannot lean forward because of the dysfunction of the trunk resulting from events such as cervical cord injury (CCI). It is therefore difficult for such people to work at a table because they may easily fall from their wheelchair, and it is also hard for them to return to their original position. This limits the activities of daily living (ADLs) of people with trunk impairment. These problems can be solved to some extent with equipment such as a wheelchair belt or a spinal orthosis that can help the person to maintain his or her posture. However, people cannot move

Making Your Instruments Work for You.

Objective measures are alluring owing to the apparent robustness of data generated. However, there are numerous validity, reliability, cost, and logistics issues that can affect a study involving objective instruments. Using an ongoing study on patient chair design, this article highlights some of the important issues that an investigator must address, including repeatability, sensitivity and specificity, generalizability, integration, installation, budget, outcome, and time.

A systematic review of risk factors associated with accidental falls, outcome measures and interventions to manage fall risk in non-ambulatory adults.

PURPOSE: To systematically review peer-reviewed literature pertaining to risk factors, outcome measures and interventions managing fall risk in non-ambulatory adults. METHODS: Twenty-one papers were selected for inclusion from databases including PubMed/Medline, Cumulative Index to Nursing and Allied Health Literature (CINAHL), Cochrane Library, Scopus, Consumer Health Complete and Web of Science. Selected studies involved a description of fall related risk factors, outcomes to assess fall risk and intervention studies describing protocols to manage fall risk in non-ambulatory adults. Studies

An explanation for the recent increase in the fall death rate among older Americans: a subgroup analysis.

OBJECTIVE: We sought to explain the recent increase in the death rate from falls among Americans aged 65 years and older. METHODS: Using the CDC WONDER online database, a longitudinal analysis of subgroups of fall mortality based on the International Classification of Diseases, 10th Revision (ICD-10) was conducted in older adults and in younger people. We used linear regression to examine the statistical significance of trends in mortality rates during 1999-2007. RESULTS: The overall mortality rate from falls increased by 55% among older Americans (≥65 years of age) during 1999-2007, from 29

Accuracy of Timed Up and Go Test for screening risk of falls among community-dwelling elderly.

OBJECTIVE: To determine the accuracy of the Timed Up and Go Test (TUGT) for screening the risk of falls among community-dwelling elderly individuals. METHOD: This is a prospective cohort study with a randomly by lots without reposition sample stratified by proportional partition in relation to gender involving 63 community-dwelling elderly individuals. Elderly individuals who reported having Parkinson's disease, a history of transitory ischemic attack, stroke and with a Mini Mental State Exam lower than the expected for the education level, were on a wheelchair and that reported a single fall

[Total hip arthroplasty after femoral neck fracture in patients with acquired neurological deficit].

PURPOSE OF THE STUDY: To evaluate the outcome of total hip arthroplasty (THA) performed for femoral neck fracture in patients with a paralytic hip. MATERIAL AND METHODS: Between 1997 and 2004, total hip arthroplasty was performed in 301 patients with a medio-cervical or sub-capital fracture of the femoral neck. In 17 of them there was also neurological deficit due to a stroke in 11, multiple sclerosis in four, and conditions following cranial injury in two. The degree of neurological deficit was assessed pre-operatively and its effect on the outcome was evaluated, in particular that on the ov

An intervention program to reduce falls for adult in-patients following major lower limb amputation.

A qualitative and quantitative assessment was conducted regarding falls sustained by in-patients receiving rehabilitation therapy following major lower limb amputation at the Glenrose Rehabilitation Hospital. During the nine-month assessment period, 18 of 58 patients in the amputee unit experienced a fall, of which 17% resulted in a moderate injury. The majority of falls occurred during patients' use of a wheelchair (14 of 18) and involved poor balance (nine of 14). Patient wheelchair self-transfers accounted for 71% (10 of 14) of the falls, while sitting in the wheelchair and reaching represe

[Incidence, circumstances and consequences of falls in patients undergoing rehabilitation after a first stroke].

OBJECTIVES: To describe the incidence, circumstances, and consequences of falls in patients admitted in a rehabilitation ward after a stroke. METHODS: Prospective monitoring of falls over four years in a neurological rehabilitation unit. Use of fall registry. INCLUSION CRITERIA: age < 75 years, admission < 45 days after stroke onset, single stroke of 1 cerebral hemisphere (nonlacunar) or of the brain stem. Only falls due to loss of balance were considered; falls caused by a seizure or syncope were not considered. RESULTS: Of 217 consecutive patients with the inclusion criteria, 34 had falle

Technology to promote safe mobility in the elderly.

New technologies designed to help prevent adverse events related to the mobility of geriatric patients (ie, patient falls, bed-rail entrapment, patient handling, and wandering) are described. Technology offers the potential to eliminate or mitigate preventable adverse events that interfere with treatment, delay rehabilitation, potentiate impairment, and compromise patient safety. Unchecked, these adverse events can have a negative impact on patient health, functional status, and quality of life. It is not surprising that the elderly constitute the population at highest risk for adverse events,

Identifying nursing home residents at risk for falling.

OBJECTIVES: To develop a fall risk model that can be used to identify prospectively nursing home residents at risk for falling. The secondary objective was to determine whether the nursing home environment independently influenced the development of falls. DESIGN: A prospective study involving 1 year of follow-up. SETTING: Two hundred seventy-two nursing homes in the state of Washington. PARTICIPANTS: A total of 18,855 residents who had a baseline assessment in 1991 and a follow-up assessment within the subsequent year. MEASUREMENTS: Baseline Minimum Data Set items that could be potential

Patient falls in stroke rehabilitation. A challenge to rehabilitation strategies.

BACKGROUND AND PURPOSE: The risk of falls is very high among stroke patients, and falling is a major complication in stroke rehabilitation. This study aimed to investigate the incidence, characteristics, and consequences of falls in an inpatient stroke rehabilitation setting. METHODS: One hundred sixty-one patients consecutively admitted to a geriatric stroke rehabilitation unit were studied. Falls that occurred during their rehabilitation stay were prospectively registered and analyzed. RESULTS: Sixty-two of the patients (39%) suffered falls. The total number of falls was 153, which corresp

Falls in the institutionalized elderly.

The reports on 72 of 190 accidents occurring among 189 nursing home residents during a 3-month period were drawn at random and reviewed. The majority of the accidents were falls, either major or minor. Personal data on the residents included age, sex, mental status, number of diseases, and number of drugs administered. Data on environmental factors included the time of day and location of the accident, and the concomitant nursing staff patterns. Women of advanced age were more likely to be involved in a major fall, whereas younger men were more likely to be involved in a minor one. Alert, whee
