A Clinical Audit of the Use of CT Head Scan Post-Inpatient Falls in Hospitalised Older Adults Utilising a Post-Fall Clinical Pathway.

Background: Older adults are at high risk of falls, and head injuries following these events can have devastating consequences. The In-Patient Post Fall Clinical Pathway is a tool utilised in many hospitals in Queensland, Australia, to guide the need for CT brain imaging post-inpatient fall. This audit aimed to assess the use of CT imaging following inpatient falls in older adults, evaluate adherence to the In-Patient Post Fall Clinical Pathway, and explore factors associated with serious head injury. Methods: A retrospective audit was conducted across two regional Queensland hospitals over 2.

Biological Skin Substitutes in Pressure Ulcers: High-Purity Type I Collagen-Based Versus Amnion/Chorion Membrane.

BACKGROUND: Pressure ulcers (PUs) present a therapeutic challenge in bedridden or neurologically impaired patients and represent a significant healthcare burden, affecting millions of patients worldwide. Current standard treatment approaches often fall short in achieving optimal healing outcomes, necessitating the development of advanced therapeutic interventions. Biological skin substitutes - high-purity type I collagen (HPTC) and dehydrated human amnion/chorion membrane (dHACM) - have emerged as promising therapeutic alternatives to traditional wound care approaches. This study compares thei

Congenital Absence of Bilateral Patella in an Active Military Personnel Case Report.

INTRODUCTION: Bilateral anterior hip dislocations are exceedingly rare, accounting for only 2-5% of all traumatic hip dislocations. They typically result from high-energy trauma, with most cases reported in road traffic accidents. Early reduction is crucial to minimize complications such as avascular necrosis, irreducibility, and post-traumatic osteoarthritis. CASE REPORT: We report the case of a 32-year-old male who sustained bilateral anterior hip dislocations, pubic type on the right and obturator type on the left, following a high-energy trauma involving a fall astride a roadside ravine.

A potential role for trace amines in the treatment of septic shock.

Sepsis involves an unregulated response to bacterial infection, the inflammatory response leading to persistent hypotension, leading to a precipitous fall in arterial blood pressure. The major trigger for the hypotension in sepsis and septic shock is the release of lipopolysaccharide (LPS) from the cell wall of gram-negative bacteria, which generates a major inflammatory response. LPS activates toll-like receptors (TLR4), leading to the release of an array of inflammatory mediators, which most notably activate inducible nitric oxide (NO) synthase and release copious amounts of NO. This is resp

Models for delivery and co-ordination of primary or secondary health care (or both) to older adults living in aged care facilities.

BACKGROUND: The number of older people is increasing worldwide and public expenditure on residential aged care facilities (ACFs) is expected to at least double, and possibly triple, by 2050. Co-ordinated and timely care in residential ACFs that reduces unnecessary hospital transfers may improve residents' health outcomes and increase satisfaction with care among ACF residents, their families and staff. These benefits may outweigh the resources needed to sustain the changes in care delivery and potentially lead to cost savings. Our systematic review comprehensively and systematically presents t

Assessing patient transport conditions during ambulance transit.

Emergency ambulances play a vital role in medical rescue and patient transportation, but their transit can impact patient health due to vehicle dynamic forces and vibrations. This study evaluates patient transport conditions on a stretcher subjected to vertical vibration excitation from road unevenness. Using an eight-degree-of-freedom numerical model, we analyze the construction parameters of a medical stretcher's support and vehicle suspension. Actual experimental data from an emergency vehicle were utilized to assess the vibration conditions experienced by both the stretcher and the ambulan

TAILR (Nursing-Sensitive Events and Their Association With Individual Nurse Staffing Levels) Project: Protocol for an International Longitudinal Multicenter Study.

BACKGROUND: Nursing-sensitive events (NSEs) are common, accounting for up to 77% of adverse events in hospitalized patients (eg, fall-related harm, pressure ulcers, and health care-associated infections). NSEs lead to adverse patient outcomes and impose an economic burden on hospitals due to increased medical costs through a prolonged hospital stay and additional medical procedures. To reduce NSEs and ensure high-quality nursing care, appropriate nurse staffing levels are needed. Although the link between nurse staffing and NSEs has been described in many studies, appropriate nurse staffing le

Dramatic Wound Closing Effect of a Single Application of an iBTA-Induced Autologous Biosheet on Severe Diabetic Foot Ulcers Involving the Heel Area.

INTRODUCTION: Chronic wounds caused by diabetes or lower-extremity artery disease are intractable because the wound healing mechanism becomes ineffective due to the poor environment of the wound bed. Biosheets obtained using in-body tissue architecture (iBTA) are collagen-based membranous tissue created within the body and which autologously contain various growth factors and somatic stem cells including SSEA4-posituve cells. When applied to a wound, granulation formation can be promoted and epithelialization may even be achieved. Herein, we report our clinical treatment experience with seven

The characteristics of falls in an institutionalized elderly cohort in China - A Retrospective observational study.

Falls are classified by WHO as the second highest cause of accidental injuries in older adults. It remains unclear what characteristics of falls are in elderly individuals that do not present definite causes. A total of 304 institutionalized older adults presented at least one fall were recruited in this retrospective observational study. The fall incidence was significantly higher at 4am-8am, at locations involving sitting down or standing up and involving flat ground, and when individuals going to toilets, getting off bed or walking. The most frequently injured sites were head and lower limb

Infant death from accidental suffocation and strangulation in bed in England and Wales: rare or unrecognised events?

BACKGROUND: Mandatory joint police and healthcare investigations of sudden unexpected death in infancy (SUDI) have been in place since 2008 in England. These include death scene examination with cause of death determined at multiprofessional case conference. Detailed evidence on sleep arrangements is available for most cases potentially leading to more being identified as due to accidental suffocation. SUDI remaining unexplained following investigation are classified as SIDS (sudden infant death syndrome) or unspecified deaths. Our objective was to determine whether detailed SUDI investigation

Transfer learning on physics-informed neural networks for tracking the hemodynamics in the evolving false lumen of dissected aorta.

Hyperconnectivity in the cerebello-thalamo-cortical (CTC) circuit, a key component of predictive coding, has been linked to schizophrenia and psychosis risk among youth at clinical high-risk for psychosis (CHR-P). The cerebellum monitors prediction errors, and its dysfunction may lead to misattributions of internal experiences and, consequently, hallucinations and delusions. Neighborhood social fragmentation, characterized by disrupted social ties and unpredictable norms, may compromise the brain's ability to form stable models of the social world. Within predictive coding frameworks, such env
