BACKGROUND: Most fall-prediction models in Parkinson's disease (PD) rely on prior falls, limiting prevention. The Waiting Room of the Future (WROTF) platform collects standardized motor and cognitive measures during routine visits, integrated into electronic health records (EHR).
OBJECTIVE: To develop a first-fall prediction model combining WROTF-EHR data and compare its performance with established cognitive- and motor-based models.
METHODS: Previously published motor (Paul) and cognitive (CYCLE-II) models were refit using logistic regression. A new regularized (LASSO) model was developed.

Age-related changes increase frailty and vulnerability to stress in older adults, and stress has been linked to poorer balance and fall risk. However, the mechanisms by which emotional stress may contribute to falls remain unclear. This study explores whether: 1) emotional stressors lead to neuromuscular changes that affect postural control in older adults; and 2) technology can assist in fall prevention by detecting increased risk. A cross-sectional, laboratory-based observational study is being conducted in a single session comparing 30 young adults (18-39 years) and 30 older adults (≥65 yea

BACKGROUND: During the multidisciplinary Definitive Surgical and Anesthetic Trauma Care (DSATC) course, trauma specialists are trained to apply damage control principles to polytrauma patients. The course was redesigned in 2020 from a three-day, traditional format to a restructured two-day format combining online and traditional learning methods to enhance course quality and learning outcomes. Considering that learning opportunities on the work floor are scarce, continuous course evaluation is crucial to ensure benefits for medical professionals and their patients.
METHODS: This retrospective

Combat sports provide a unique human model in which cerebral ischaemia-reperfusion stress, adaptive neuroprotection, and impact-induced neurodegenerative risk coexist. Across striking and grappling disciplines, exercise-induced metabolic and redox stress, repetitive head impacts and transient cerebral ischaemia-reperfusion during vascular neck restraints expose the brain to competing adaptive and injurious stimuli. Cerebral ischaemic preconditioning (cIPC) research demonstrates that brief, sublethal reductions in cerebral blood flow (CBF) activate a conserved hormetic programme involving modul

Background Patients undergoing coronary artery bypass grafting (CABG) commonly experience reduced lower-limb strength, impaired balance, decreased functional mobility, and autonomic dysfunction during the postoperative period. These impairments may increase fall risk and limit daily activities. The Otago Exercise Program (OEP) is an evidence-based exercise intervention designed to improve lower-limb strength, balance, and functional performance, while resonance frequency breathing (RFB) has been shown to enhance cardiac autonomic regulation and heart rate variability (HRV). Although both inter

Older adults with diabetes mellitus, encompassing both type 1 diabetes (T1D) and type 2 diabetes (T2D), face a substantially elevated risk of fragility fractures, contributing significantly to morbidity and mortality in this vulnerable population. The underlying pathophysiology differs between the two types: T1D is typically characterized by reduced bone mineral density (BMD) stemming from insulinopenia, whereas T2D often presents with normal or even high BMD but compromised bone quality due to factors, including altered microarchitecture, accumulation of advanced glycation end products (AGEs)

Isolated bilateral fractures of the lesser tuberosity of the humerus represent an exceptionally rare subset of proximal humerus fractures. A 66‑year‑old active woman with chronic steroid‑induced osteoporosis presented with bilateral shoulder pain after a ground‑level fall down a step. Clinical examination demonstrated marked bilateral loss of internal rotation strength with positive belly‑press and bear‑hug tests, whereas external rotation was preserved. Standard radiographs, CT, and MRI confirmed displaced bilateral lesser tuberosity avulsion fractures in porotic bone, with the subscapularis

IMPORTANCE: Gait adaptability is essential for safe walking. Impairments after non-progressive neurological conditions contribute to walking limitations and fall risk. Advances in virtual/augmented reality-based treadmills/walkways have increased interest in gait adaptability assessment and training, making a review timely.
OBJECTIVE: The objectives were as follows: (1) Explore the characteristics and outcomes of physical interventions used to improve gait adaptability for individuals with non-progressive neurological conditions; and (2) Identify the outcome measures and tasks used to track c

AIMS: Toddler's fractures are non-displaced spiral fractures of the lower third of the tibia which are usually sustained during a minor fall. These fractures are surrounded by a thick outer lining of bone (the periosteum), which typically makes them stable. Treatment approaches vary; most clinicians use casts or boots to immobilise the leg, whereas others observe recovery without immobilisation. Both approaches are thought to be safe. Whilst immobilisation protects the leg, it can cause issues with stiffness, recovery and complications. The aim of the ToTs study is to determine whether no-immo

BACKGROUND AND PURPOSE: Osteoporosis increases the risk of fragility fractures, contributes to significant morbidity, and is a psychological challenge-fear of falling (FoF)-which can limit activity and engagement in rehabilitation. These challenges are often overlooked after a fracture, despite their relevance to rehabilitation planning. Understanding these challenges is crucial for health care practitioners, particularly physical and occupational therapists, who are central to fall prevention and post-fracture care. This study explored the perspectives of older adults with osteoporosis who ha

BACKGROUND: Split hamate fractures, which most often are a complicating part of fracture dislocations of the 5th or 5th and 4th carpometacarpal joints, are rare and challenging injuries that can severely affect hand function. The injury usually requires surgical treatment. However, both short- and long-term data on the effectiveness of these interventions are limited. The objective of this study is to advance understanding of this rare hand surgical condition by investigating its epidemiology, etiopathogenesis, subsequent complications, and functional outcomes.

Background The transition from graduate medical education (GME) into independent practice is challenging for new attendings despite achieving "readiness for practice," by Accreditation Council for Graduate Medical Education (ACGME) Milestones. Most medical literature on this transition is specialty-specific and rarely involves assessment of new-to-practice attendings (NTPAs). Objective Lack of attending readiness results in compromised patient care. To inform GME leaders and managers, we sought to identify areas of struggle for NTPAs across specialties. With this information, we hope to spur n

INTRODUCTION: Patients with knee osteoarthritis are at a higher risk of falls compared to healthy individuals, thereby increasing the likelihood of accidental injury. Resistance training is an important strategy for managing knee osteoarthritis. Although some studies suggest that blood flow restriction training combined with low-load resistance training (LL-BFRT) is a beneficial treatment approach, its effect on fall risk and balance function in patients with knee osteoarthritis remains unclear. We aim to conduct a randomised controlled trial to assess the effectiveness of combined training in

Introduction: Nuclear medicine theranostics is rapidly emerging, as an interdisciplinary therapy option with multi-dimensional considerations. Healthcare Professionals do not have the time to do in depth research on every therapy option. Personalized Chatbots might help to educate them. Chatbots using Large Language Models (LLMs), such as ChatGPT, are gaining interest addressing these challenges. However, chatbot performances often fall short in specific domains, which is critical in healthcare applications. Methods: This study develops a framework to examine the use of contextual augmentation
