Interstep Variations of Stairways and Associations of High-Contrast Striping and Fall-Related Events: Observational Study.

BACKGROUND: Interstep variations in step riser height and tread depth within a stairway could negatively impact safe stair negotiation by decreasing step riser height predictability and, consequently, increasing stair users' fall risk. Unfortunately, interstep variations in riser height and depth are common, particularly in older stairways, but its impact may be lessened by highlighting steps' edges using a high-contrast stripe on the top front edge of each step. OBJECTIVE: This study aimed to determine (1) if fall-related events are associated with greater interstep riser height and depth va

Circumstances of clinical fracture events with advancing age in older men.

To characterize fracture circumstances by age at time of fracture among community-dwelling older men, we analyzed incident clinical fractures in the Osteoporotic Fractures in Men study. Participants were contacted every 4 mo to identify fractures confirmed by radiographic reports. Fractures were classified as fall- or non-fall-related and further categorized by degree of trauma: minimal (fall from ≤ standing height), moderate (fall on stairs, steps or curb), or severe (fall from > standing height) for fall-related fractures; and minimal (eg, coughing), moderate (eg, collisions with objects dur

A top-of-flight stairway defect affects foot overhang on lower steps during stair descent.

Falls during stair descent have been associated with non-uniformities in stair dimensions. A common stairway non-uniformity is the top-of-flight defect, where the rise and run dimensions of the uppermost step in a stairway differ from the subsequent lower steps. This defect has been described as hazardous, but its effect on foot placement and misstep risk in stair descent has not been studied. In this experimental study, we quantified foot overhang for adults (n = 27) descending a seven-step staircase with a top-of-flight defect compared to descending a uniform staircase. When the defect was p

Role of the Functional Gait Assessment in Validating Item Difficulty Hierarchy and Fall Risk for Idiopathic Normal Pressure Hydrocephalus.

BACKGROUND: Falls along with gait-balance disturbances are important symptoms of idiopathic normal pressure hydrocephalus (iNPH). OBJECTIVES: This study aimed to validate the Functional Gait Assessment (FGA) item-difficulty hierarchy, determine its optimal fall cutoff value, and identify key items for assessing fall risk in iNPH. METHODS: One hundred eighty-eight iNPH patients underwent pre-intervention assessments using the FGA, timed up-and-go (TUG), 10-Meter Gait Speed (10MGS) tests, and their falls history in the past 6 months. Rasch analysis confirmed the validity of the FGA item-diffic

Visual Illusion Created by a Striped Pattern Through Augmented Reality for the Prevention of Tumbling on Stairs.

A fall on stairs can be a dangerous accident. An important indicator of falling risk is the foot clearance, which is the height of the foot when ascending stairs or the distance of the foot from the step when descending. We developed an augmented reality system with a holographic lens using a visual illusion to improve the foot clearance on stairs. The system draws a vertical striped pattern on the stair riser as the participant ascends the stairs to create the illusion that the steps are higher than the actual steps, and draws a horizontal striped pattern on the stair tread as the participant

Stair-Fall Risk Parameters in a Controlled Gait Laboratory Environment and Real (Domestic) Houses: A Prospective Study in Faller and Non-Faller Groups.

BACKGROUND: Falling on stairs is a major health hazard for older people. Risk factors for stair falls have been identified, but these are mostly examined in controlled biomechanics/gait laboratory environments, on experimental stairs with a given set of step dimensions. It remains unknown whether the conclusions drawn from these controlled environments would apply to the negotiation of other domestic staircases with different dimensions in real houses where people live. OBJECTIVES: The aim of this paper is to investigate whether selected biomechanical stepping behavior determined through stai

EMG-based prediction of step direction for a better control of lower limb wearable devices.

BACKGROUND AND OBJECTIVES: Lower-limb wearable devices can significantly improve the quality of life of subjects suffering from debilitating conditions, such as amputations, neurodegenerative disorders, and stroke-related impairments. Current control approaches, limited to forward walking, fall short of replicating the complexity of human locomotion in complex environments, such as uneven terrains or crowded places. Here we propose a high-level controller based on two Support Vector Machines exploiting four surface electromyography (EMG) signals of the thigh muscles to detect the onset (Toe-of

Low Risk of Clinically Important Traumatic Brain Injury in Children Who Tumble Down Stairs.

BACKGROUND: Pediatric trauma management seeks to minimize head computed tomography (HCT) while capturing clinically important traumatic brain injuries (ciTBI). The Pediatric Emergency Care Applied Research Network (PECARN) system stratifies patients as high-, intermediate-, or low-risk for ciTBI. Although designed for free falls, we noted that PECARN criteria often are applied to tumbling down stairs (TDS), with steps estimated at 12", though TDS rarely appeared to result in ciTBI. METHODS: In a retrospective chart review of pediatric TDS patients, data was collected on mechanism of injury, c

Risky behavior during stair descent for young adults: Differences in men versus women.

Injuries commonly occur on stairs, with high injury rates in young adults, especially young women. High injury rates could result from physiological and/or behavioral differences; this study focuses on behaviors. The purposes of this observational study were (1) to quantify young adult behaviors during stair descent and (2) to identify differences in stair descent behavior for young adult men versus women. Young adult pedestrians (N = 2,400, 1,470 men and 930 women) were videotaped during descent of two indoor campus staircases, a short staircase (2 steps) and a long staircase (17 steps). Beha

A Novel Smart Shoe Instrumented with Sensors for Quantifying Foot Placement and Clearance during Stair Negotiation.

Trips and slips are significant causal perturbations leading to falls on stairs, especially in older people. The risk of a trip caused by a toe or heel catch on the step edge increases when clearance is small and variable between steps. The risk of a slip increases if the proportion of the foot area in contact with the step is reduced and variable between steps. To assess fall risk, these measurements are typically taken in a gait lab using motion-capture optoelectronic systems. The aim of this work was to develop a novel smart shoe equipped with sensors to measure foot placement and foot clea

Underlying mechanisms of fall risk on stairs with inconsistent going size.

Serious falls occur frequently on stairs with inconsistent dimensions. Inconsistent smaller goings are thought to reduce user's foot clearances and foot contact lengths since individuals do not detect and alter their behaviour prior to the inconsistency, increasing the risk of a trip, heel-catch or over-step and potential slip on the stairs. So far, these mechanisms for a stair fall remain theoretical only. The aim of this paper was to identify the underlying mechanisms by which steps with inconsistent going size increase the risk of falls. For this study twenty-seven younger adults (24 ± 3 y,
