The rise and fall (and rise) of the Affordable Care Act: Varying impacts on coverage over time and place.

The Affordable Care Act (ACA) significantly expanded health insurance in the United States, but its impact has varied across time and states. We assess the law's heterogeneous impacts over the three presidential administrations since its enactment (between 2013 and 2023), as well as across states with different levels of implementation of the law. We focus on the law's Medicaid expansion and Marketplace subsidies, including the enhanced subsidies under the American Rescue Plan of 2021 (ARP). We use national household survey data and a triple-difference design-leveraging variation by time, plac

Dog-related injuries requiring medical attention in Skaraborg district, Sweden.

AIM: The aim of this study was to identify factors associated with dog-related injuries by analysing data collected from 5908 patients seeking treatment at healthcare facilities in Skaraborg district, Sweden, over the years 2001-2016. METHODS: To describe factors associated with dog-related injuries, we analysed variables from four categories: people, that is, characteristics (gender and ages) of the patients who were injured; the circumstances in which the injuries occurred; the causes and types of injuries caused by dogs (injury mechanisms); and whether the injuries resulted in hospitalisat

Dynamics of culling risk of Holstein cows in US organic dairy herds.

The study aimed to analyze the culling dynamics of cows in US organic dairy herds based DHI-reported reasons. In addition, the overall cow longevity in these herds was evaluated. A total of 287,927 lactation records from cows calving between 2018 and 2022 across 434 organic dairy herds from 31 US states were evaluated. Annualized rates of live culling and death were calculated, and the distribution of culling reasons was assessed by parity, days after calving, season, year, and geographic region. The average age at culling and its association with lifetime genetic merit were also examined. The

Factors Influencing Interest in Urology Among Osteopathic and Allopathic Medical Students: A Single-Institution Cross-Sectional Survey.

Background  Following the transition to a single-accreditation residency system, Doctor of Osteopathic Medicine (DO) and Doctor of Allopathic Medicine (MD) students now apply for the same urology residency positions. Despite this unification, disparities in match rates persist, and factors influencing student interest in and the perceived feasibility of pursuing urology remain unclear. Objective  To compare and explore differences in perceptions of pursuing urology between DO and MD medical students at a single institution, with primary outcomes including perceived attainability of a urology c

Risk perception of falls on the same level at workplaces among social welfare workers in Japan.

Falls on the same level (FSLs) are a major cause of occupational injuries in the social welfare industry, especially in aging societies. Here, we conducted a questionnaire survey to investigate how social welfare workers perceive the risk of occupational accidents, with a focus on FSLs, and to examine the effects of age on risk perception. Respondents (n=294) rated their estimated likelihood of experiencing five major types of occupational accidents across six levels of injury severity. The results indicated that the FSL risk was not consistently underestimated relative to other accident types

From GPT-4 to Expert-Endorsed Athlete Guidance: A Delphi Consensus on Sleep and Jet Lag.

BACKGROUND: Large language models (LLMs) including GPT-4 are increasingly used to generate health information, but concerns persist about their accuracy and relevance, particularly for elite athletes. OBJECTIVE: This study used GPT-4-generated frequently asked question (FAQ) responses on sleep and jet lag as the starting material for expert evaluation and consensus development, with the goal of producing consensus-based, athlete-specific guidance while identifying the limitations of AI-generated content. METHODS: Between November 2024 and March 2025, n = 17 international sleep and circadian

Seatbelt D-ring anchor location moderates forward excursion in reclined seating: a scoping review and meta-regression of low-speed human volunteer studies.

OBJECTIVES: The emergence of autonomous vehicles is expected to introduce nonstandard seating configurations, including reclined postures, that fall outside the design basis of current restraint systems. A growing body of research has investigated occupant kinematics in these configurations, but most of this work relies on anthropomorphic test devices or postmortem human subjects. Human volunteer data, which captures active musculoskeletal responses and demographic variability, remains limited and has not been synthesized across studies. METHODS: This scoping review identified 10 human volunt

Bedtime Audiobook Listening Motives and Insomnia Status: Survey Study.

BACKGROUND: Many individuals use audiobooks at bedtime to unwind and facilitate sleep, yet little is known about how this practice relates to sleep health. Understanding these patterns may inform behavioral strategies to support sleep. OBJECTIVE: This study aimed to examine associations between bedtime audiobook listening, perceived sleep benefits, and sleep-onset insomnia symptoms in a large adult sample. METHODS: A cross-sectional online survey was conducted among 1136 Swedish adults (aged ≥50 years: n=600, 53%; women: n=1027, 90%). Recruitment took place in Sweden between January and Febr

Capability, Opportunity, and Motivation for Healthy Eating: Exploring Gender-Specific Pathways.

Women generally consume a healthier diet than men; however, both fall short of dietary guidelines. Health behaviour theories are often applied in a gender-neutral manner, implicitly assuming that explanatory models operate similarly across groups. This conceptual replication tested the Capability, Opportunity, Motivation and Behaviour (COM-B) model for dietary behaviour, originally evaluated in a female-dominant sample, in a male-dominant sample (N = 1,358; 67% male). Using the same measurement battery and a closely aligned analytic approach, structural equation modelling indicated acceptable

Rule-based labels versus experimental endpoints: A systematic assessment of generalisation in QSAR models for ADMET prediction using fruit phytochemicals.

Computational ADMET assessment is now a cornerstone of early drug discovery, yet QSAR classifiers are sometimes trained on physicochemical rule-derived labels rather than measured biological data. This practice raises a fundamental question: do strong internal performance statistics actually reflect predictive accuracy against experimental endpoints? To address this directly, we built Random Forest (RF) and XGBoost (XGB) classifiers for gastrointestinal (GI) absorption and blood-brain barrier (BBB) permeation using 995 fruit phytochemicals from 29 species, annotated with binary labels derived

Multicomponent Behavior Change Technique Intervention for Caregivers of People With Alzheimer Disease and Related Dementias: Protocol for a Single-Arm, Personalized Behavioral Trial to Disrupt Sedentary Time.

BACKGROUND: Sedentary behavior is associated with negative health outcomes. High levels of sedentary behavior are common among Alzheimer disease and related dementias (ADRD) caregivers already at risk of other adverse health effects, yet few interventions target sedentary behavior within this population. There is a need for trials intended to reduce time spent sedentary, which may be achievable by increasing the frequency of disruptions to sedentary time. Remotely delivered behavior change techniques (BCTs) may be effective for disrupting sedentary behavior in this population through short bur

Cannabis hyperemesis syndrome: the rare case of a valid diagnosis plus hypersensitization hypothesis for development and chronification of this severe somatic cannabis use disorder.

Cannabis hyperemesis syndrome (CHS) is a serious physical consequence of long-term, high-dose cannabis use, and a regional increase in incidence appears to be an indicator of an increase in cannabis use there. In addition to gradually increasing nausea, which escalates into cyclical vomiting attacks, and resistance to classic antiemetics, relief by hot showers is one of the diagnostic criteria. However, the same symptoms are also found in cyclic vomiting syndrome (CVS), which in turn can also be associated with cannabis use, but unlike CHS, cannabis is not causal for CVS. The only diagnostic c

The Role of Fall Rate From Transdermal Alcohol Concentration on Alcohol-Related Consequences in College Students.

BACKGROUND: Alcohol elimination rate is associated with acute alcohol consequences, yet it remains difficult to measure reliably in real-world settings. Wearable transdermal alcohol concentration (TAC) sensors provide a feasible option through passive, continuous monitoring of biological alcohol exposure. This facilitates precise testing of whether alcohol elimination rate predicts alcohol-related consequences in naturalistic environments. Additionally, a faster alcohol elimination rate may buffer or reduce the risk of alcohol-related consequences following high and/or rapid consumption. METH

A Tripartite Framework of Conflict-Induced Stress, Acoustic Hypervigilance, and Ramadan Observance as Convergent Determinants of Circadian Decoherence in Athletes.

Athletes living and training in active conflict zones face a combination of distinct biological pressures that disrupt the body's internal 24-hour clock and stress-hormone systems. The constant psychological threat of conflict keeps the body's stress-hormone axis chronically active, raising cortisol levels, heightening alertness, and making it harder to fall and stay asleep. A key problem in the existing research is that nighttime sleep disruption in conflict zones has often been incorrectly described as ordinary screen-related sleep loss. In reality, government-issued missile warning sirens a

Everolimus Related Acute Steatohepatitis Detected on 68Ga-Dotatate PET/CT in a Patient with Metastatic Duodenal Neuroendocrine Tumor.

UNLABELLED: Hepatocellular carcinoma (HCC) is the most common form of primary liver cancer worldwide, with increasing incidence and mortality rates. Although several targeted therapies are currently available, the therapeutic outcomes remain unsatisfactory due to the high heterogeneity and drug resistance of HCC. Therefore, novel molecular mechanisms and therapeutic strategies urgently need to be explored. In this study, we obtained the GSE39791 dataset from the GEO database and identified 1,186 differentially expressed genes (DEGs). Weighted gene co-expression network analysis (WGCNA) was con

Residential radon in Peru: challenges for health surveillance and lung cancer prevention.

The Journal of Genetics, started by William Bateson in 1910, played a distinguished role in the early years of genetics. However, it stopped publishing in 1978. The Indian Academy of Sciences revived it in 1985, and has published it regularly since then. To commemorate this landmark, I highlight one of the 17 articles published that year. 'The isolation and genetic analysis of a Caenorhabditis elegans… X-chromosome balancer' by András Fodor and Péter Deak, of the Institute of Genetics, Biological Research Centre, Hungarian Academy of Sciences, Szeged, Hungary. Structural fragility in digital public health communication.

Inherited genetic disorders are progressive in nature and lead to organ dysfunction or death in severe cases. At present, there are no permanent treatment options for >95% of inherited disorders. Different modes of inheritance, type of gene(s) involved, and population-based variations add further complexity to finding suitable cures for approximately 400 million patients worldwide. Gene therapy is a very promising molecular technique for the treatment of rare genetic disorders. Gene therapy functions on the basis of restoration, replacement, inhibition, and, most recently, editing of gene(s) t

Peer discrimination and diurnal cortisol output in a rural boarding school in China: Empirical findings and methodological considerations.

Adolescents' experiences of discrimination and their health consequences are understudied in non-Western cultures. Using data from 90 rural Chinese adolescents (Mage = 13.70; 49% female), this study examined cumulative and daily experiences of peer discrimination (based on socioeconomic status, gender, parental migration status, appearance, grades) and their associations with diurnal cortisol output. Data highlighted a high prevalence of peer discrimination, with 85% of the sample reporting any type of cumulative discrimination in the current semester and 56% of the sample reporting any type o
