Integrated analysis of extrachromosomal circular DNA and transcriptome reveals associations between microbial profiles and Immunoinflammatory markers in hypospadias.

PURPOSE: Hypospadias, a prevalent congenital anomaly characterized by abnormal urethral opening formation, poses significant psychological and physical challenges to patients. This study aimed to elucidate the function of extrachromosomal circular DNA (eccDNA) found in foreskin tissues of hypospadias patients as well as its association with immunoinflammatory responses and microbial dynamics. METHODS: We used Circle-seq to quantify eccDNA abundance and RNA sequencing for gene expression profiling. Foreskin samples were carefully collected from 54 patients with hypospadias and compared against

When seeing is not believing: a scope mistaken for a foreign body.

OBJECTIVES: A convincing endoscopic image can prove false when the observer's expectation supplies a conclusion before evidence is tested. We describe how a flexible ureteroscope can mimic a retained intraluminal foreign body, and how anchoring and diagnostic momentum can drive an operator to an unnecessary and hazardous retrieval. CASE PRESENTATION: During an elective flexible ureteroscopy as a planned second-look procedure, a linear, smooth, metallic-appearing structure within a dilated calyx was taken for a retained intraluminal foreign body, consistent with the residual hardware anticipat

Is a Commensal Here, a Pathogen There? Genotype Specific Responses by the Sea Anemone Nematostella vectensis to Vibrio Species.

Animals rely on diverse molecular mechanisms to maintain beneficial microbial associations while also defending against pathogens, yet the extent to which these responses vary among genotypes of a single species remains poorly understood. Using individuals from different locations of the sea anemone Nematostella vectensis, we compared transcriptional responses to a commensal (Vibrio diabolicus) and pathogen (Vibrio coralliilyticus) bacterium. We find striking genotype-specific divergence: individuals from Nova Scotia mounted a strong transcriptional response to V. diabolicus, whereas the North

A Rare Pediatric Cause of Lateral Foot Pain: Symptomatic Os Vesalianum Pedis Requiring Excision and Peroneus Brevis Repair.

INTRODUCTION: Os vesalianum pedis (OV) is a rare accessory ossicle located posterior to the proximal base of the fifth metatarsal, with an estimated prevalence of 0.6%. It results from the failure of fusion between the secondary ossification site and the primary ossification center. Most cases are asymptomatic and identified incidentally, but symptomatic presentations can occur following acute trauma or repetitive stress. Reported adolescent cases are limited, with prior literature documenting ages 13-19. We present the youngest reported case of a skeletally immature symptomatic OV in our find

Electronic Health Record-Linked Biobank Expansion Reveals Global Health Inequities.

Electronic health record (EHR)-linked biobanks are transforming biomedical research, enabling population-scale studies that integrate genomic, clinical, and phenotypic data. Yet as these resources proliferate, it remains unclear how their research outputs reflect global health priorities. This article presents a comprehensive review of five globally established EHR-linked biobanks: UK Biobank, the Million Veteran Program, FinnGen, the All of Us Research Program, and the Estonian Biobank. Drawing on 14,142 peer-reviewed publications from 2000 to 2024, we show how each biobank displays a distinc

Mutations in the Txnip PPXY motifs protect against myocardial infarction despite enhanced protein stability.

Thioredoxin-interacting protein (Txnip), a member of α-arrestin superfamily, is best known for inhibiting thioredoxins and glucose transporters, increasing oxidative and metabolic stress. Through these functions, Txnip has emerged as a key contributor to the pathogenesis of heart diseases. Txnip contains C-terminal PPXY motifs that are conserved among α-arrestins across diverse species. Nevertheless, the physiological significance of these motifs remains unknown. We demonstrate that mutation of Txnip PPXY motifs to AAXA uncouples Txnip's canonical functions from its cytotoxic effects. Although

MAIT cell enrichment in Lynch syndrome is associated with immune surveillance and colorectal cancer risk.

BACKGROUND: Lynch syndrome (LS) is the most common hereditary cause of colorectal cancer (CRC) and results from pathogenic germline variants affecting mismatch repair. The tissue microenvironment that contributes to this elevated risk of CRC is poorly characterised particularly during the early precancerous stages. OBJECTIVE: To define features of the colonic microenvironment that distinguish LS carriers with and without a history of CRC from one another and from the general population. DESIGN: We applied Expanded Cellular Indexing of Transcriptomes and Epitopes by sequencing, a multimodal s

Intrinsic T-cell programming and immune spatial organization govern sex-biased tuberculosis immunity.

Biological sex can profoundly influence the susceptibility to infectious diseases, yet the mechanisms behind the sex-dependent protective immunity against tuberculosis (TB) remain poorly understood. Here we show that sexually divergent immunity during chronic Mycobacterium tuberculosis (Mtb) infection is governed by both intrinsic T cell programming and pulmonary immune spatial organization. Using the Four Core Genotype (FCG) mouse model, adoptive cell transfer, pathway-specific blockade and B cell depletion, we demonstrate that CD4⁺ T cells from gonadal females (XXF), but not XX males (XXM),

Pharmacological characterization and nephroprotective potential of venoms from three egyptian scorpion species.

INTRODUCTION: Scorpion venoms are complex natural mixtures rich in bioactive molecules with emerging pharmacological relevance. Drug-induced kidney injury, particularly from chemotherapy agents like Taxol® (paclitaxel), remains a significant clinical challenge. This study investigates the molecular composition, enzymatic activities, and potential nephroprotective effects of venoms from three Egyptian scorpion species (Leiurus quinquestriatus, Buthacus leptochelys, and Scorpio maurus palmatus) against Taxol®-induced kidney injury. METHODS: Comprehensive molecular mass profiling of the three ve

Cirevetmab, a novel canine anti-TGFβ monoclonal antibody, exerts antifibrotic effects on renal cells in vitro.

INTRODUCTION: Transforming Growth Factor-β (TGF-β) has been implicated for its role in fibrosis and progression of chronic kidney disease (CKD) in both humans and rodents. Additionally, CKD is a common and progressive condition in canines, with striking pathological similarities to human disease. However, targeted therapies addressing fibrogenic mediators in dogs remain underexplored. METHODS: We investigated the role of all three TGF-β isoforms (TGF-β1, TGF-β2, and TGF-β3) in CKD and renal fibrosis with in vitro model systems using human and canine primary renal proximal tubular epithelial c

Recent Advances in the Molecular Mechanisms of Bacterial Anti-Phage Defence Systems.

Deep learning models based on both distant protein sequence homology and genetic neighbourhood context searches predicted that 1.5% of bacterial genes or 30 genes per E. coli genome represent anti-phage systems (APS); many are colocalized on defence islands or are found on mobile DNA elements. Thousands of APS thus remain to be defined molecularly. Selected recent examples highlighting an astonishing molecular diversity of these defence systems are described in this editorial. The mechanisms include allosterically regulated dGTPase responding to competing nucleotide signals (Clover); bacterial

Genome organization and packaging, mutation impacts and quasi-periodic patterns in viral genomes.

A method described in this publication is able (i) to display features related to the general genome organization and packaging; (ii) to identify explicitly motifs responsible for these features and the distribution of motifs over the genome; the most pronounced motifs can be screened against available databases for further studies; (iii) to assess the impact of mutations; the salient mutations can be tried experimentally and used for genome editing; and (iv) to quantify the information that can be insightful for general studies of molecular evolution of living organisms. The method is based o

Avulsion Fracture of the Lesser Trochanter and the Use of Conservative Treatment.

Avulsion fractures of the lesser trochanter of the femur are extremely rare injuries in the pediatric population, most commonly resulting from physical activity. The typical mechanism involves a sudden contraction of the iliopsoas muscle, which inserts on the lesser trochanter. We present a rare case of such an injury in a pediatric patient, emphasizing the importance of appropriate imaging and the effectiveness of conservative management in achieving full recovery. An 11-year-old boy presented to the emergency department with perineal trauma sustained after losing balance while riding a bicyc

Incidence of Gonadal and Extragonadal Germ Cell Tumours in Patients With Klinefelter Syndrome.

BACKGROUND: Klinefelter's syndrome (KS; 47, XXY) is associated with an altered risk profile for malignancies compared with non-KS males. In particular, several reports have noted a striking association between KS and extragonadal germ cell tumours (EGCTs), especially in the mediastinum, whereas the risk of testicular germ cell tumours (TGCTs) in KS remains unclear. KS patients also have a higher prevalence of cryptorchidism (undescended testes)-a known risk factor for TGCT in the general population-yet it is uncertain if cryptorchidism confers the same cancer risk in the KS population. This st

Hypercontractile Esophagus: An Underrecognized Cause of Dysphagia with a "Nutcracker" Appearance on Barium Esophagram.

Esophageal motility disorders are an important and uncommon cause of dysphagia when structural abnormalities are absent. We present a case of a 77-year-old woman with dysphagia and regurgitation whose barium esophagram demonstrated a characteristic "nutcracker" appearance of the distal esophagus. Despite this striking radiographic finding, peristalsis was preserved, and a barium tablet traversed the segment without obstruction. These features argue against classic diffuse esophageal spasm and instead suggest a hypercontractile motility disorder. This case highlights the role of barium esophagr

Metastatic Crohn's Disease and Acute Myeloid Leukemia: A Diagnostic Paradox in a Severe Hyperinflammatory State.

Crohn's disease (CD) is a chronic immune-mediated inflammatory disorder in which extraintestinal manifestations occur in up to 40% of patients. Metastatic Crohn's disease (MCD), characterized by noncaseating cutaneous granulomas anatomically remote from the gastrointestinal tract, is among the rarest of these manifestations. Its coexistence with occult acute myeloid leukemia (AML) and secondary hemophagocytic lymphohistiocytosis (sHLH) in a treatment-naïve patient constitutes an exceptional clinical triad with profound diagnostic and therapeutic implications. We report a 56-year-old woman with

Delayed Descending Thoracic Aortic Laceration Caused by a Posterior Rib Fracture Fragment: A Case Report.

INTRODUCTION: Delayed descending thoracic aortic laceration caused by a rib fracture fragment after blunt chest trauma is rare but potentially fatal. Because initial findings may appear limited, diagnosis and definitive treatment can be delayed. We report a case in which rapid progression to massive hemothorax and traumatic cardiac arrest was successfully managed by emergency thoracotomy followed by definitive aortic repair. CASE PRESENTATION: A 78-year-old woman sustained extensive bilateral rib fractures after striking her chest against a tree. She was initially managed conservatively becau

Endothelial cell-specific knockout of ATG5 ameliorates inflammation and renal fibrosis by regulating pyroptosis.

OBJECTIVES: ATG5, a key regulator of autophagy-associated inflammation, is markedly upregulated under inflammatory conditions. In this study, we observed significantly elevated ATG5 expression in the kidneys of patients with renal fibrosis and in mouse kidneys after unilateral ureteral obstruction (UUO), particularly in renal tubules and glomeruli. Recent studies suggest that enhanced ATG5-mediated autophagy in tubular epithelial cells targets the NF-κB pathway to alleviate renal injury. The glomerulus is a dense capillary network primarily composed of endothelial cells, which represent a key
