Repurposed small molecule toxin inhibitors neutralise a diversity of venoms from the Neotropical viperid snake genus Bothrops.

Snakebite globally claims more than 100,000 lives per year and results in morbidity for 400,000 survivors. Current treatment uses antibody-based antivenoms which are constrained by their efficacy, safety, and cost. In this study we evaluated the efficacy of previously described repurposed drugs against viperid snakes of the medically important Bothrops genus. Despite variable toxin representation and bioactivity across this central and south American genus, we found that the lead inhibitors targeting metalloproteinases (marimastat and DMPS) and phospholipases (varespladib) demonstrated pan-spe

Optimizing prostate biopsy decision making - (MUSIC-screen): A 1:1 randomized controlled trial comparing micro-ultrasound versus multiparametric magnetic resonance imaging for prostate cancer diagnosis.

BACKGROUND: Micro-ultrasound (microUS) represents a potential alternative to multiparametric magnetic resonance (mpMRI) in guiding prostate biopsy, with level 1 evidence demonstrating non-inferiority to detect Grade Group ≥2 (GG ≥ 2) prostate cancer in biopsy-naïve men. However, a critical gap remains in the screening pathway, in which imaging is needed to identify men at risk and determine whether biopsy is warranted. METHODS: MUSIC-Screen is a phase 3, open-label, noninferiority 1:1 randomized controlled trial evaluating microUS as an alternative imaging compared to mpMRI for determining th

A systematic evaluation of musculoskeletal injuries in mixed martial arts athletes: a meta-analysis.

INTRODUCTION: Mixed martial arts (MMA) is a popular combat sport including wrestling, boxing, and martial arts, which involves danger and violence. It is reported to have one of the greatest injury rates among different sports, limits participation, and often consists of considerable medical costs. The available literature on MMA injuries is sparse and requires a comprehensive review of injuries in this popular sport. This systematic review and meta-analysis was conducted with the aim of critically evaluating the epidemiological literature on injury patterns, mechanisms, and associated risk fa

India's Stray Dogs as a Public Health Risk: An Autopsy Case Report and Comprehensive Review.

A major public health issue is stray dogs, particularly in urban and peri-urban areas. Particularly for small children, who are more vulnerable to serious injury and deadly assaults, stray dogs can present serious risks. While playing outside his house, a 5-year-old kid was fatally bitten by a stray dog. Severe head, neck, and back injuries, including deep cuts, exposed occipital bone, abrasions, and bruises, were discovered during an autopsy. The internal findings were a jugular vein tear, laryngeal perforations, vertebral avulsion, and neck muscle lacerations. The growing number of stray dog

Local immune responses in tick-bite skin during SFTSV infection are associated with type I interferon signaling and monocyte-lineage cell recruitment.

The skin harbors a complex immune microenvironment that integrates innate and adaptive components and plays a critical role in vector-borne pathogen transmission. To investigate immune responses during tick-mediated transmission of Severe Fever with Thrombocytopenia Syndrome Virus (SFTSV), we analyzed skin from BALB/c mice bitten by artificially infected Amblyomma testudinarium nymphs. Skin samples were collected at time points corresponding to peak viral RNA levels (days 6-7 post-attachment) and analyzed by transcriptomics, followed by real-time PCR and immunohistochemical (IHC) analyses. Severe venom-induced consumption coagulopathy, snakebite-associated thrombotic microangiopathy, and local necrosis following Western bush viper (Atheris chlorechis) envenoming in France.

BACKGROUND: Atheris spp. are small African vipers whose bites are rarely reported but can result in significant envenoming. There is no specific antivenom available. We describe a case of systemic A. chlorechis envenoming presenting with venom-induced consumption coagulopathy (VICC), snakebite-associated thrombotic microangiopathy (TMA), and local necrosis. CASE PRESENTATION: A 33-year-old man was bitten on the index finger by a captive A. chlorechis. Within 6 hours, early laboratory abnormalities progressed to VICC with markedly prolonged PT/aPTT, undetectable fibrinogen, elevated fibrin mon

Promoting the utilization of rheumatologic musculoskeletal anatomy: a 50-year travel log.

This report traces the authors' long journey toward a deeper understanding of the anatomical basis of rheumatologic physical examination. Fifty years ago, one of the authors was struck by the prevalence of subcutaneous and deep bursitis and regional musculoskeletal pain syndromes at his new workplace, prompting studies of bursal swelling, lubrication, and the dynamics of distended superficial and deep bursae. Subsequently, a deficiency in anatomical knowledge was noted across multiple workshops at the American College of Rheumatology and elsewhere. Later, in a different setting, musculoskeleta

Injury severity and anatomic patterns in canine occupants involved in vehicular crashes: application of the veterinary abbreviated injury scale.

OBJECTIVE: The Veterinary Abbreviated Injury Scale (V-AIS) was created to mirror the human AIS and address a critical gap in retrospective veterinary trauma analysis. Dogs struck by a vehicle or injured inside a vehicle have the highest mortality rates among cases presenting to Veterinary Trauma Centers (VTC). Improved understanding of injury mechanisms, patterns, and severity in canine vehicular trauma is necessary to inform development of scientifically validated restraint systems and other vehicle safety features for canine occupants. METHODS: The Veterinary Committee on Trauma (VetCOT) Re

Multicentric malignant melanoma in a grey mare: A case report.

BACKGROUND: Grey coat color in horses, resulting from a 4.6 kb copy number variation (CNV) within the syntaxin 17 (STX17) gene (Grey locus), is linked to progressive hair depigmentation and a markedly increased risk of melanoma, likely to its function as a melanocyte-specific enhancer. AIMS: This report details the clinical, pathological, and genomic findings of a 26-year-old flea-bitten grey mare with multiple melanocytic tumors, progressive dyspnea, and congestive heart failure. Due to the poor prognosis the horse was euthanized and submitted for necropsy to the Long Island University - Lew

Venom with an altitude: Coagulopathy following an envenomation by the high-mountain snake Gloydius monticola (Likiang pitviper) in Yunnan Province, China.

The Likiang pitviper (Gloydius monticola) is a high-altitude species endemic to northwestern Yunnan Province, China, and its venom effects on humans have not previously been documented. We report the first confirmed case of human envenomation by G. monticola, resulting in severe and progressive coagulopathy. A 34-year-old male was bitten on the hand while photographing an adult specimen at approximately 3500 m elevation. Initial local pain and swelling were followed by delayed but catastrophic coagulopathy, characterized by profound hypofibrinogenemia, markedly prolonged clotting times, elevat

Extracellular Hemoglobin, Hypoxia, and Macrophage-Mediated Pulmonary Vascular Remodeling in Hemolytic Disease.

Pulmonary hypertension (PH) is a well-recognized complication of chronic hemolytic anemias such as sickle cell disease and thalassemia, yet the relative contributions of hypoxia and cell-free hemoglobin (Hb) to disease progression remain incompletely understood. Patients with hemolytic anemia experience a lifelong cycle of chronic and inter bitten hypoxia that compounds vascular injury driven by extracellular Hb and its degradation products, heme and iron. While the effects of hypoxia and Hb exposure have historically been studied in isolation, the combined impact of sustained, low-level plasm

Hypopituitarism following Russell's viper envenomation in Bangladesh: findings from a prospective longitudinal cohort study.

Envenomation by Russell's viper is a major cause of hemotoxic snakebite in South Asia, associated with venom-induced consumptive coagulopathy (VICC), leading to end-organ damage, including the kidneys, adrenal glands, and pituitary gland. Hypopituitarism is a rare but increasingly recognized delayed complication, reported primarily from South India, Sri Lanka, and Myanmar, but not previously reported from Bangladesh. We conducted a prospective longitudinal cohort study at a tertiary referral hospital in Rajshahi, Bangladesh (2019-2025). A total of 203 male patients with Russell's viper envenom
