The factors associated with submersion events among less than 20-year-old persons that occurred in King County from 1974 to 1983 were studied to focus prevention efforts. Near-drowning (n = 103) and drowning (n = 96) victims were identified from medical examiners' reports, paramedics' reports, and hospital discharge registers. Annual incidence was 5.5; the mortality rate was 2.6 per 100,000 children. Although preschool-aged children had the largest incidence (12.8), followed by older adolescents (4.9), adolescents had the largest case fatality rate, 77%. Lake and river victims had the largest

Drowning is the second most common cause of accidental death in children. Swimming pools and natural bodies of water close to home present the greatest risk to young children. The single most important step in the treatment of submersion accident victims is the immediate institution of resuscitative measures at the earliest possible opportunity. Ice-water submersion accidents are an important subgroup of near-drowning victims, who at times can defy predictions for outcome after profound anoxic-ischemic insults. Drowning accident prevention is an important public health measure.

A total population study of childhood fresh water drowning accidents (fatalities) for the 15 year period, 1967-1981, is reported. These data are from the ongoing Brisbane Drowning Study which has now also analysed 255 fresh water child immersions (both fatalities and near-fatalities) over the eleven year period, 1978-1981, and as such forms a consecutive unselected series for over one decade. The annual fatality (drowning) rate is 3.53 per 100,000. Details of immersion accidents by site, sex and by outcome (survivors versus fatalities) are presented. An analysis of secular trends revealed that

The incidence of childhood immersion injury in a defined population is reported. The data were drawn from this Hospital's Child Injury Surveillance programme and cover the period July 1, 1984 to June 30, 1985. The overall rate of immersion injury was 26.8 per year per 100 000 children at risk and the rate for fresh-water immersions was 25.3. The magnitude of this public health problem is revealed by an annual fresh-water immersion rate of 70.2 per 100 000 at risk for the critical 0 to four years' age group. Eighty per cent of all immersions occurred in swimming-pools and the rate of pool immer

Eye banks are occasionally asked to retrieve corneal tissue from drowning victims. The ambient water surrounding the eyes before recovery of the victim has chemical, pH, and osmotic characteristics different from those of aqueous humor, thereby possibly making corneas recovered from these eyes unsuitable for transplantation. Corneal endothelial cell function and ultrastructure were maintained following soaking of rabbit eyes for three hours in either seawater, fresh lake water, or chlorinated swimming-pool water at 25 degrees C. The data suggest that physiological and ultrastructural integrity

Findings of the serial EEGs of 9 children involved in fresh water near-drowning were correlated with neurological signs and final outcome. Our observations revealed that in young near-drowning victims there is a distinct evolution of the EEG which differs considerably from any stereotyped classification outlined for other types of encephalopathies. In patients with a poor clinical outcome, the early phase of near-drowning encephalopathy was characterized electrically by diffuse delta waves, often with alpha or beta frequencies superimposed (alpha-delta and beta-delta pattern). Additional featu

Sequential neurological and intellectual recovery after childhood near-drowning is discussed. Decisions concerning the persistence and intensity of resuscitation require a knowledge of the natural history of intellectual improvement after rescue from near-drowning. A severe case of fresh-water immersion, leading to recovery, is described. Evidence is presented to suggest that the time interval of one hour before the first spontaneous respiratory gasp forms the upper limit of the apnoeic time bracket after which survival can still be expected, and to indicate that intellectual improvement (to a

A study of all serious childhood immersion accidents (both drowned and near-drowned cases) is reported from Hawaii. This is a total population-based survey of 140 consecutive cases (0--15 years) occurring during the five-year period (1973--1977. Age-specific, sex-specific, and osmolality-specific (salt versus fresh water) data are presented both for survivors and fatalities. The overall annual drowning rate of 3.1 per 100,000 children at risk is low, for a water-oriented society. The survival rate following loss of consciousness in the water is 73 per cent. There is no evidence from this study

One hundred and ninety-six fatal immersion accidents occurred in Geelong and district during the 20-year period, 1959 to 1978. Of 54 drownings in children aged 16 years and younger, 43 (80%) occurred in males and 33 (61%) in fresh water. All 14 drownings in children under four years of age occurred in fresh-water sites, 12 of which were about the home. Immediate and optimal resuscitative measures in such children are extremely important.

The role of epileptiform seizures in causing drowning and near-drowning among children was studied by examining the case reports of all 140 childhood immersion accidents that occurred in an area of Hawaii over five years. Four of the 140 immersion accidents were caused partly by epileptiform seizures, but none were fatal. The combined results of the Hawaiian and Brisbane studies (total population studied over five years 1 600 000) showed that no epileptic children died from accidents in the sea or in swimming pools; and the 2.9% incidence of immersion accidents due to seizures in the Hawaiian

A study of childhood survival rates, after loss of consciousness in fresh water, has been undertaken. Age-specific, sex-specific and site-specific survival rates for childhood fresh water immersion accidents are reported for the first time. The overall survival rate, after loss of consciousness in the water was 0.49; swimming pool and domestic bath tub serious immersion accidents have a survival rate of 0.60 compared with a rate of 0.05 for similar immersion accidents in creeks and rivers. Young male schoolboys have the lowest potential survival (0.20 or less) of any group. Survival rates were

A total population study of all serious immersion accidents is reported from the Australian Capital Territory. The annual immersion accident rate of 4-69 per 100,000 of the population at risk is low, and the fresh water fatality rate of 2-58 per 100,000 is very low. The annual fatality rate for childhood swimming pool accidents is 0-34 per 100,000 children (that is, under 16 years of age) at risk. Serious immersion accidents are not increasing in frequency, in striking contrast to the epidemic trends seen from other centres. Factors which might explain this are discussed.

An analysis of a consecutive series of 66 swimming pool immersion accidents is presented; 74% of these occurred in in-ground swimming pools. The estimated accident rate per pool is five times greater for in-ground pools compared with above-ground pools, where pools are inadequately fenced. Backyard swimming pools account for 74% of pool acidents. Motel and caravan park pools account for 9% of childhood immersion accidents, but the survival rate (17%) is very low. Fifty per cent of pool accidents occur in the family's own backyard pool, and 13.6% in a neighbour's pool; in the latter the surviva

A large total population study of childhood fresh water immersion accidents is reported. The study was undertaken in the City of Brisbane over the five-year period 1971 to 1975 inclusive, and 111 fresh water immersion accidents involving children were studied and analysed. The childhood fresh water immersion accident rate, including drowning and near-drownings, of 10-43 per year per 100,000 at risk (fatality rate of 5-17) is the highest reported. If an unsupervised child gets into difficulties in fresh water and loses consciousness he has a 50% chance of dying. The immersion accident rate has

This paper reviews the circumstances of 142 deaths by drowning which occurred in Geelong and district during the years 1959 to 1974. The effectiveness of safety nets and protective fences as a means of preventing the accidental submersion of young children in home swimming pools is questioned. Consumption of alcohol before submersion played a major role in the death of many adult males. Of males aged 26 years and over, 79% imbibed alcohol before death, and 57% had a blood alcohol concentration greater than 0.1% at autopsy. Of males aged 17 to 25 years of age, 25% imbibed alcohol before death a
