Recurrent laryngeal nerve identification and assessment during thyroid surgery: laryngeal palpation.

Electrical identification and monitoring of the recurrent laryngeal nerve (RLN) has been proposed as an adjunct to standard visual identification of the nerve during thyroid and parathyroid surgery. This study was undertaken to assess laryngeal palpation as an intraoperative technique for identifying and assessing the RLN during surgery and to investigate the relation between laryngeal palpation and associated laryngeal electromyographic (EMG) activity. The postcricoid region of the larynx during surgery was palpated through the posterior hypopharyngeal wall to sense posterior cricoarytenoid m

Postcricoid hemangioma presenting as dysphagia: a report of 4 cases.

Hemangiomas involving the postcricoid region of the hypopharynx are rare. This report reviews our experience with 4 cases of postcricoid hemangioma, including a set of twin siblings. All patients underwent panendoscopy. All patients were managed nonsurgically. Three patients did well with dietary modification. One patient, with multiple medical problems, remains partially dependent on her gastrostomy tube. To our knowledge this report represents the largest series in the literature and the first to describe similar lesions in twin siblings. This report highlights the importance of a complete f

Angiofibroma of the hypopharynx.

Primary extranasopharyngeal angiofibroma is very rare. To date, approximately 60 cases have been reported in the English literature. Only 1 case was confined to the posterior wall of hypopharynx. In August 2000, a 68-year-old man presented with a 6-month history of progressive foreign-body sensation in the throat and intermittent inspiratory stridor. Endoscopic examination of the larynx and pharynx revealed a nonpulsatile, pink-grayish, polypoid mass arising from the posterior pharyngeal wall of the left hypopharynx. Under general anesthesia, this tumor mass was subsequently removed by the end

[Clinical manifestations of diffuse idiopathic skeletal hyperostosis (DISH)].

BACKGROUND: Diffuse idiopathic skeletal hyperostosis (DISH, Forestier disease) is in most cases a harmless, asymptomatic disease characterized by a massive, noninflammatory ossification with intensive formation of osteophytes affecting ligaments, tendons, and fasciae, especially of the spinal column. If the disease becomes symptomatic at all, the leading complaints will usually be dysphagia in the sense of a lump in the throat and difficulty in swallowing. However, in rare cases the osteophytes may influence the laryngeal function ranging from hoarseness and immobility of the vocal cord to lif

The nasogastric tube syndrome: two case reports and review of the literature.

BACKGROUND: The nasogastric tube syndrome is a potentially life-threatening complication of an indwelling nasogastric (NG) tube. The syndrome is thought to result from ulceration and infection of the posterior cricoid region with subsequent dysfunction of vocal cord abduction. This dysfunction may present as complete loss of vocal cord abduction manifested as serious airway compromise. Reports of this syndrome are infrequent, with only 29 cases published to date. METHODS: Two additional cases of nasogastric tube syndrome diagnosed at the University of Iowa Hospitals and Clinics over a 2-year

Transillumination laryngoscopy: a new way of evaluating vocal fold and hypopharyngeal lesions.

Conventional methods of observing the larynx and the hypopharynx use reflected light to illuminate the larynx. The aim of this study was to see whether transilluminating the larynx was possible in subjects with and without disease. The larynx and the hypopharynx were observed by means of a rigid scope with a low-light charge-coupled device camera without the light guide inserted. Illumination was provided by a second rigid scope attached to a light source that was held at the neck by an assistant. The larynx and hypopharynx were observed by transillumination using both constant lighting and st

Sensory nerve supply of the human oro- and laryngopharynx: a preliminary study.

To date, the details of human sensory innervation to the pharynx and upper airway have not been demonstrated. In this study, a single human oro- and laryngopharynx obtained from autopsy was processed with a whole-mount nerve staining technique, Sihler's stain, to determine its entire sensory nerve supply. The Sihler's stain rendered all mucosa and soft tissue translucent while counterstaining nerves. The stained specimen was then dissected and the nerves were traced from their origins to the terminal branches. It was found that the sensory innervation of the human pharynx is organized into dis

Esophageal Plexiform Schwannoma.

We report a patient with a plexiform schwannoma located in the cervical part of the esophagus. A large pedunculated intraluminal polyp, originating in the postcricoid region and protruding into the oropharynx, was found in association with multiple submucosal nodules. Both the intraluminal polyp and submucosal nodules consisted of cellular schwannomatous tissue. The patient showed no signs of neurofibromatosis or schwannomatosis. Plexiform schwannomas are uncommon tumors, mainly confined to the dermis and subcutis of trunk, head, and neck region and upper extremities. Visceral examples are van

Sensory innervation of the pharynx and larynx.

To shed light on supraesophageal complications of reflux disease, sensory innervation--particularly, distinct distribution, area, and density of sensory fibers--of the feline pharyngolaryngeal mucosa was reported. The investigations were performed by means of histochemistry (tracer techniques) and immunohistochemistry. The pharyngeal mucosa from the Eustachian cushion to the middle level of aryepiglottic fold, except the laryngeal surface of epiglottis, was supplied by the glossopharyngeal sensory fibers, whereas the laryngeal sensory fibers innervated between the apex of epiglottis and the le

Variation in survival of patients with head and neck cancer in Europe by the site of origin of the tumours. EUROCARE Working Group.

The study describes the prognosis of head and neck cancer in Europe on the basis of information available to population-based cancer registries collaborating in the EUROCARE II project. Variation in survival in relation to country and the anatomical site/sub-site of origin of the tumours was examined. Survival analysis was carried out on 35,004 head and neck cancer cases (ICD 141, 143-148 and 161) diagnosed between 1985 and 1989 in 17 European countries. Prognosis varied considerably according to anatomical site: the best 5-year survival rates were seen for cancer of the larynx (63% in men) an

The role of computed tomography and magnetic resonance imaging in planning for conservation laryngeal surgery.

Refined laryngeal conservation surgeries have been devised to maintain the voice while eradicating even extensive glottic and hypopharyngeal tumors. The role of diagnostic imaging is to adequately define the extent of disease in conjunction with endoscopy. Critical areas to be evaluated are the tongue base, vallecula, pre-epiglottic space, arytenoid cartilages, postcricoid region, hypopharynx, thyroid cartilage, and subglottis.
