We have used selected ion flow tube mass spectrometry (SIFT-MS) to analyse the vapours emitted by petrol and diesel fuels and the exhaust gases from petrol (spark ignition) and diesel (compression ignition) engine vehicles fitted with catalytic converters. Only those components of these media that have significant vapour pressures at ambient temperatures were analysed and thus particulates were obviously not detected. Cause investigation of a fire and explosion at the nuclear fuel waste reprocessing plant indicated that self-heating ignition of an asphalt-salt-waste, bituminized, mixture (AS) caused the disaster. A 220l drum was filled with the AS at a temperature of about 180 degrees C. About 20h later the drum ignited and burned as it was being cooled. From 1923 to 1996, 77 human fatalities occurred in 35 hyperbaric chamber fires, three human fatalities in a pressurized Apollo Command Module, and two human fatalities in three hypobaric chamber fires reported in Asia, Europe, and North America. Three patients, employed by janitorial service agencies, were burned severely while buffing floors. A solvent to remove floor lacquer was used in one case, and a solvent to remove glue that held carpet to the floor was used in the other two cases. The solvent used in each case is flammable and was ignited when a floor buffer was used near it. The floor buffers are specifically labeled as unsafe to operate near flammable liquids. The patients' injuries were severe. All three patients survived. This represents a rather unusual and unsafe industrial practice that should be avoided.

Safety has always been an important aspect of any laser application in surgery and medicine. In any review of the laser surgery literature, several issues continue to dominate. These issues include: wearing eye protectors, dealing with the plume of vaporized tissue, and controlling potential fire hazards. No one denies that lasers can pose a serious hazard to the eye, but the decision to wear eye protectors in all procedures has been frequently questioned.
