Refining the diagnosis of 46,XY disorders of sex development: insight from whole-exome sequencing.

INTRODUCTION: Differences in sex development (DSD) with 46,XY karyotype are a group of rare congenital conditions affecting the structure and function of the urogenital system. Published data indicate, that despite the increasingly widespread use of genetic testing, the etiology remains unclear in approximately half of cases. AIM OF THE STUDY: To clarify the molecular causes of 46,XY DSD by performing whole-exome sequencing (WES) in a precisely phenotyped and clinically comprehensively evaluated group of patients. PATIENTS AND METHODS: WES was performed in a consecutive cohort of 39 children

Prostatic tuberculosis incidentally discovered during transurethral resection of prostate with subsequent diagnosis of rectal and pulmonary involvement: a case report.

BACKGROUND: Tuberculosis (TB) is a globally prevalent infectious disease, including in Taiwan. Prostatic TB is a rare manifestation of genitourinary tuberculosis (GU-TB), which is the third most common extrapulmonary form of the disease. However, due to its insidious onset and non-specific symptoms, prostatic TB is often diagnosed late. CASE DESCRIPTION: We report a case of a 72-year-old male patient who presented with lower urinary tract symptoms (LUTS) and painful scrotal swelling. Following transurethral resection of the prostate (TURP), histopathological examination revealed prostatic TB.

[Tuberculosis of the male genital organs as the first manifestation of generalized tuberculosis].

Urogenital tuberculosis is difficult to diagnose in the general healthcare network because it has no pathognomonic symptoms, and there is no screening for urogenital tuberculosis, which complicates its timely diagnosis. This article presents a clinical case of generalized tuberculosis that first manifested as a lesion of the external genital organs. A high level of clinical suspicion for tuberculosis prompted special investigations using widely available methods. Real-time PCR diagnostics allowed to identify tuberculosis of the male genital organs as the first manifestation of generalized tube

MAG3 SPECT/CT Detection of Vesicocolic Fistula in Genitourinary Tuberculosis.

Enterovesical fistula is a rare complication of genitourinary tuberculosis and may be difficult to diagnose, particularly in the setting of impaired renal function. We report a 41-year-old woman with chronic kidney disease and bilateral obstructive uropathy secondary to treated genitourinary tuberculosis. 99m Tc MAG3 renography demonstrated a nonfunctioning left kidney and preserved right renal function without obstruction. Delayed images revealed abnormal tracer propagation from the urinary bladder into bowel loops. SPECT/CT localized the tracer within the colon, confirming a vesicocolic fist

Imaging Modalities in Tuberculosis.

Tuberculosis (TB) remains a major global health challenge, with increasing prevalence of multidrug-resistant and extrapulmonary forms complicating diagnosis and management. Imaging plays a pivotal role in the early detection, characterization, and treatment monitoring of TB, particularly when clinical or microbiological findings are inconclusive. Modalities such as chest radiography, computed tomography (CT), ultrasound, magnetic resonance imaging (MRI), and 18F-fluorodeoxyglucose positron emission tomography combined with CT (18F-fluorodeoxyglucose [18F-FDG PET-CT]) provide critical insights

Extrapulmonary Tuberculosis with Seminal Vesicle Involvement in an Immunocompetent Patient.

Extrapulmonary tuberculosis often presents diagnostic challenges due to its involvement in multiple organ systems. We report a case of a 54-year-old, immunocompetent man who presented with prolonged fever and a generalized seizure. Neuroimaging revealed ring-enhancing lesions suggestive of tuberculomas. Further evaluation demonstrated a seminal vesicle abscess and cervical lymphadenitis. Histopathology and microbiological analysis confirmed Tuberculosis. The patient was initiated on antitubercular therapy, along with anti-convulsants and steroids. This case highlights the importance of conside

Diagnostic Imaging of Extrapulmonary Tuberculosis Across Organ Systems.

Extrapulmonary tuberculosis (EPTB) is an infectious disease characterized by the invasion of Mycobacterium tuberculosis beyond the lungs. Diagnosis is frequently delayed due to nonspecific clinical presentations that vary by organ system, making diagnostic imaging essential for disease detection, characterization, and treatment monitoring. The objective of this review is to examine and summarize imaging-based approaches for the diagnostic evaluation of EPTB across multiple body systems, including the central nervous system, spine, cardiovascular system, lymphatic system, abdominal and hepatic

Lyell´s syndrome associated with the use of topical agents: case report.

UNLABELLED: Pseudomonas aeruginosa AG1 (PaeAG1) is a Costa Rican strain that was isolated in 2010 in a major Hospital. This strain has resistance to multiple antibiotics such as β-lactams (including carbapenems), aminoglycosides, and fluoroquinolones. PaeAG1 is considered critical (Priority 1) due to its resistance to carbapenems, and it was the first report of a P. aeruginosa isolate carrying both VIM-2 and IMP-18 genes encoding for metallo-β-lactamases (MBL) enzymes (both with carbapenemase activity). Owing to these traits, we have studied this model for 10 years using diverse approaches inc

Systematic Review and Meta-Analysis of Urine-Based Molecular Testing for Urogenital Tuberculosis.

BACKGROUND: Early confirmation of urogenital tuberculosis (UGTB) remains challenging in routine practice because conventional culture is slow and has variable sensitivity. Urine-based molecular testing has emerged as a rapid alternative, but its performance estimates vary across studies. This systematic review evaluated the diagnostic value of urine-based molecular testing, including Xpert Mycobacterium tuberculosis/rifampicin, multiplex polymerase chain reaction and loop-mediated isothermal amplification, for UGTB. METHODS: Risk of bias was assessed using Quality Assessment of Diagnostic Acc

Congenital tuberculosis presenting as acute cervical lymphadenitis in a 28-week preterm infant.

Congenital tuberculosis is a rare infection in infants with extremely low birthweight. We report the case of an infant born at 28 weeks' gestation who developed isolated, cervical lymphadenitis at age 84 days. Despite the infant's continuous hospitalisation in the neonatal intensive care unit and a negative maternal history, Mycobacterium tuberculosis was identified through culture and molecular testing of surgical drainage fluid. The diagnosis was substantially delayed due to several factors: a low index of clinical suspicion based on the infant's isolation from external sources, the presence

Many varied presentations of genitourinary tuberculosis (GUTB) such as recurrent urinary tract infection, sterile pyuria (pus cells in urine despite a negative routine bacterial culture) with/without haematuria, irritating voiding symptoms, infertility, hydronephrosis and renal failure had been published in the literature. Tuberculous epididymo-orchitis (inflammation of the epididymis and testis) is an underreported and rare form of GUTB. Non-specific symptoms of epididymal tuberculosis (TB) are difficult to differentiate from cysts, tumours and other infections. Bilateral epididymal involveme

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

The Fate of Oral Cavity Verrucous Hyperplasia in a Diagnostic Biopsy: A Conundrum for Subsequent Management.

BACKGROUND: Although the relationship between umbilical cord clamping time and various parameters such as hemoglobin (Hb) levels, iron deficiency, and risk of neonatal jaundice has previously been studied, to the best of our knowleadge there have been no studies investigating the relationship between cord clamping time and the risk of significant hyperbilirubinemia. We aimed to investigate the relationship between the time of umbilical cord clamping and transcutaneous bilirubin (TcB) measurements made on various postnatal hours, Hb and serum total bilirubin (STB) levels measured on postnatal 4

Comprehensive analysis of vaginal microbiota in Chinese women with genital tuberculosis: implications for diagnosis and treatment.

BACKGROUND: Tuberculosis remains an infectious disease of global concern, with potential impacts on respiratory and intestinal microbiota owing to prolonged broad-spectrum antibiotic therapy. Despite its potential to cause infertility, the vaginal microbiota of women with genital tuberculosis remains poorly understood. We comprehensively analyzed the vaginal microbiota in Chinese women with genital tuberculosis. RESULTS: We recruited women with pelvic (n = 28), endometrial (n = 16), and pulmonary (n = 12) tuberculosis as the research group, and healthy women (n = 11) as the control group. Vag

Diagnosis of Genital Tuberculosis Unveiled by Utero-Cutaneous Fistula and Superimposed MRSA Infection: A Case Report.

BACKGROUND: Both tuberculosis and Methicillin-Resistant Staphylococcus aureus (MRSA) are known to be notorious for causing fistulas due to their characteristics of persistent, difficult-to-treat infections that lead to chronic inflammation, abscess formation, and tissue necrosis. There are several case reports highlighting the invasiveness and potential for fistula formation associated with both tuberculosis and MRSA infections independently, but to the best of our knowledge, this is the first case of a utero-cutaneous fistula caused by chronic infection due to MRSA, superadded on genital tube
