A site-specific risk stratification model for extranodal diffuse large B-cell lymphoma in the oral cavity and maxillofacial region.

OBJECTIVE: Extranodal diffuse large B-cell lymphoma (DLBCL) of the oral cavity and maxillofacial region (OC-MR) is rare compared to Waldeyer’s ring (WR) DLBCL, characterized by marked aggressiveness and prognostic heterogeneity. This study aims to develop a special risk stratification model for extranodal DLBCL in OC-MR to overcome the limitations of the conventional International Prognostic Index (IPI) to improve outcome prediction in this high-risk population. METHOD: We conducted a retrospective analysis of 76 OC-MR DLBCL patients (43 extranodal and 33 WR-DLBCL) diagnosed between January 20

Large B-cell Lymphoma With IRF4 Rearrangement in the Elderly.

Large B‑cell lymphoma with IRF4 rearrangement (LBCL‑IRF4) predominantly affects children and typically presents with Waldeyer's ring and/or head and neck involvement, limited stage, and favorable outcomes. Reports in older adults are rare, and comparability to pediatric disease remains uncertain. We describe an elderly LBCL-IRF4 case with a solitary parotid lesion. Histology showed diffuse large B‑cell lymphoma, germinal center B‑cell type, with co‑expression of CD10, BCL6, and IRF4/MUM1. Karyotyping identified t(4;6)(p14;p25) involving 6p25 (IRF4), and fluorescence in situ hybridization confi

Surgical removal of Waldeyer's ring and long-term risk of Sjögren's syndrome: a population-based cohort study investigating the mucosal immune link.

BACKGROUND: The palatine tonsils and adenoids are critical inductive sites for the mucosa-associated lymphoid tissue (MALT) system, educating B cells for salivary defense. While their removal is common, the long-term impact on mucosal autoimmunity remains unclear. We hypothesized that disrupting this lymphoid network increases susceptibility to Sjögren's syndrome (SS). METHODS: Using the TriNetX US Collaborative Network (2006-2023), we conducted a retrospective cohort study matching 302,737 patients who underwent tonsillectomy/adenoidectomy with 302,737 non-surgical controls (propensity score

Imaging of Extra-nodal Head and Neck Lymphoma and its Mimics.

Extra-nodal head and neck lymphoma encompasses diverse histological subtypes and imaging appearances. Thus, it can be challenging to discriminate lymphoma from other lesions that may have similar radiological features. Herein, various lymphoma mimics will be reviewed in different anatomical locations in the head and neck including Waldeyer's ring, sinonasal tract, orbits, and salivary and thyroid glands, with reference to their clinical presentation, predisposing factors, and laboratory markers. These mimics encompass non-neoplastic (idiopathic orbital pseudotumor, granulomatosis with polyangi

Nanomaterials for the Prevention, Detection, and Treatment of Pharyngeal Human Papillomavirus Infection: A Translational Roadmap.

Pharyngeal infection with high-risk human papillomavirus (HPV), particularly HPV16, is biologically distinct from cervical infection because it occurs within the specialized lymphoepithelial environment of Waldeyer's ring. This review evaluates nanoparticle materials for the prevention, detection, and treatment of pharyngeal HPV, with an emphasis on structure-property-function relationships, mucosal performance, and translational feasibility. Lipid nanoparticle platforms, polymeric nanoparticle platforms, inorganic systems, and hybrid platforms are compared with respect to composition, particl

Rare Sinonasal Case of IRF4-Rearranged Large B-Cell Lymphoma Mimicking Chronic Sinusitis.

Large B-cell lymphoma (LBCL) with IRF4/MUM1 rearrangement is a rare and newly recognized entity predominantly involving Waldeyer's ring of lymphoid tissue in the oropharynx. Unusual clinical presentations can result in delayed diagnosis of LBCL with IRF4 rearrangement, resulting in increased patient morbidity. We present a case involving a pregnant woman with a frontal sinus swelling and clinical diagnosis of chronic sinusitis and possible Pott's puffy tumor. An endoscopic procedure and subsequent histologic examination revealed diffuse LBCL with diffuse IRF4/MUM1 staining, supported by fluore

Systemic Immunoglobulin Changes Following Adenoidectomy, Tonsillectomy, and Adenotonsillectomy in Patients With Chronic Tonsillitis and Adenoiditis: A Prospective Study.

BACKGROUND: Chronic tonsillitis and adenoiditis are common inflammatory disorders of Waldeyer's ring that may lead to persistent systemic immune stimulation due to recurrent infection of tonsillar lymphoid tissue. Because the palatine tonsils and adenoids participate in mucosal immune defense and antibody production, concerns have occasionally been raised regarding the potential immunologic consequences of surgical removal of these structures. METHODS: A prospective clinical investigation was conducted involving 100 participants, including 75 patients with chronic tonsillitis or adenoiditis a

Mapping Australia's hospital care for malignant neoplasms of the lip, oral cavity, and pharynx: An ecological study.

Neoplastic malignant tumors originating from inside the upper aerodigestive tract are referred to as lip, oral cavity, and pharyngeal malignancies. This study aimed to analyze the hospitalization profile for malignant neoplasms of the lip, oral cavity, and pharynx in Australia. This was a longitudinal ecological study that utilized the National Hospitals Data Collection in Australia database between 1998 and 2023. Hospitalization data were identified using the ICD codes C00-C14. Between 1998 and 2023, Australia reported a total of 160,055 hospitalization episodes. A large proportion of these a

18F-FAPI-74 PET/CT findings of an inflammatory branchial cleft cyst mimicking cervical metastasis from carcinoma of unknown primary: A case report.

Fibroblast-activation protein inhibitor (FAPI) positron-emission tomography (PET) is emerging as a promising alternative to 18F-fluorodeoxyglucose (FDG) PET for head-and-neck cancer. Herein, we describe the case of a 51-year-old man with a cystic level-II neck mass that was intensely FDG-avid and even more avid on 18F-FAPI-74 PET. FDG-PET was performed as standard imaging modality to search for the primary tumor, while 18F-FAPI-74 PET was conducted as part of a clinical trial to detect potential occult lesions missed by FDG. While cytology suggested metastatic squamous-cell carcinoma and FDG P

IL-6-JAK2-STAT3 Signaling Pathway and the Pathogenesis of Chronic Tonsillitis.

Tonsils are among the largest lymphoid tissues within the Waldeyer's ring. They generate lymphocytes that facilitate immune responses and play essential protective roles in host defense. When tonsillar crypts are exposed to specific bacterial or viral infections, persistent allergic stimuli, or immune dysregulation, an inflammatory cascade may be triggered, increasing the risk of chronic tonsillitis (CT). CT is a common otolaryngological condition characterized by recurrent episodes of inflammation or a single prolonged episode (>3 months), and it typically presents with repeated acute pharyng

Case Series: Large B-cell Lymphoma With IRF4 Rearrangement Involving the Lung and Tonsil in Two Different Patients.

Large B-cell lymphoma (LBCL) with interferon regulatory factor 4 (IRF4) rearrangement is characterized by the monotonous proliferation of medium- to large-sized cells with strong expression of IRF4/multiple myeloma oncogene 1 (MUM1). It predominantly affects children and young adults with a median age of 12. The reported disease sites include head and neck lymph nodes, the Waldeyer's ring, and the gastrointestinal tract. Here, we report two cases of LBCL with IRF4 rearrangement in 73-year-old and 71-year-old patients. The first patient presented with a lung nodule, while the second patient pre

Non-Hodgkin lymphoma (NHL) is a diverse group of lymphoid malignancies, with varied clinical presentations depending on the anatomical site of involvement. Although typically presenting with nodal disease, extranodal manifestations are not uncommon. NHL involving Waldeyer's ring is considered rare, with the palatine tonsils being the most frequently affected location. This case report presents a 30-year-old male patient with bilateral tonsillar hypertrophy, ultimately diagnosed as NHL. The patient presented with a persistent sore throat, dysphagia, and snoring associated with sleep disturbance

Challenging differentiation of oral malignant lymphoma from osteomyelitis of the jawbone: A case report.

Malignant lymphoma (ML) of the head and neck frequently occurs in the cervical lymph nodes and Waldeyer's ring. Although ML of the maxillofacial region is rare, extranodal ML can present with symptoms similar to those of inflammation and other tumors, making diagnosis difficult. The present study describes the case of a 59-year-old female patient with ML, which was difficult to distinguish from mandibular osteomyelitis. The patient presented to Department of Oral Medicine, Hokkaido University Hospital (Sapporo, Japan) for evaluation of left cheek swelling and osteomyelitis. The X-ray examinati

Machine Learning Analysis of Lipid and Metabolic Profiles in Adults with Adenoid Hyperplasia.

Background and Objectives: The nasopharynx, unlike other pharyngeal regions, includes an important part of the immune system, called the adenoid (nasopharyngeal tonsil); its posterior wall contains lymphoid tissue belonging to Waldeyer's ring. Nasopharyngeal posterior wall thickness is often associated with adenoid hyperplasia in adults. The current study aimed to compare the blood lipid and metabolic profiles of adult patients with increased nasopharyngeal posterior wall thickness to those of the healthy population. Materials and Methods: This study included a cohort of 98 patients, 52 in the

Multiple relapses of mantle cell lymphoma as different extranodal lesions: A case report.

Mantle cell lymphoma (MCL) usually affects the lymph nodes; however, extranodal involvement is also common, particularly in Waldeyer's ring and the gastrointestinal tract, spleen and bone marrow. Other organs that may be affected include the skin, endocrine glands, lungs and central nervous system, with these sites most commonly affected by relapsing disease. However, in rare, extranodal MCL has arisen in the ocular adnexa, soft tissue and heart. The present study reports the case of a 75-year-old male patient, who experienced repeated relapses of MCL, involving ocular adnexa, soft-tissue and

Laryngeal ectopic tonsillar tissue as a cause of dysphagia: a case report and literature review.

Ectopic tonsillar tissue outside Waldeyer's ring is rare, with laryngeal involvement being significantly uncommon. Only four cases of laryngeal ectopic tonsillar tissue have been previously documented, all in older adults presenting mostly with dysphonia. A 31-year-old male smoker presented with intermittent dyspnea and dysphagia. His medical history included chronic reflux managed with proton pump inhibitors. Flexible nasal endoscopy revealed a left aryepiglottic fold mass, later confirmed by computed tomography as a 4 × 4 mm non-enhancing nodule. Microlaryngoscopic excision was performed und

First pediatric case of follicular dendritic cell sarcoma in the parotid gland: a case report.

BACKGROUND: Follicular dendritic cell sarcoma (FDCS) is an extremely rare malignancy originating from antigen-presenting dendritic cells of the B-cell follicles. While typically affecting adults and occurring in lymphoid tissues such as tonsils and the Waldeyer's ring, FDCS in the parotid gland is exceptionally rare, with only five adult cases reported to date. To our knowledge, this is the first documented pediatric case of FDCS of the parotid gland. MATERIALS AND METHODS: We present the case of a 13-year-old male with a progressively enlarging mass in the right parotid region. Diagnostic wo
