Extramedullary plasmacytoma in children is a genetically distinct localized neoplasia curable by surgical resection.

Extramedullary plasmacytomas (EMPs) are exceedingly rare in children and adolescents. We describe clinical, pathological, and molecular features of 13 patients with EMP in this age group (8 males and 5 females; age 3-18 years). EMPs presented as localized disease involving the Waldeyer's ring (n = 9), larynx (n = 2), ocular adnexa (n = 1), or epipharynx and conjunctiva simultaneously (n = 1). One patient with EMP of the tonsils had a concurrent Epstein-Barr virus infection. Twelve of 13 patients with follow-up information reached sustained complete remissions after surgical resection (median f

Pediatric tonsillar pathology: a comprehensive ultrasonographic approach.

Palatine tonsils are part of the Waldeyer's ring and consist of lymphoid tissue. As a first-line immune system mechanism, they act against entering pathogens. They recognize and initiate an immune response to exogenous antigens. Tonsillar disease in children, particularly inflammatory conditions, is well known in the medical community. Inflammatory diseases include conditions such as tonsillitis and peritonsillar cellulitis and other more complicated diseases such as intratonsillar abscess, peritonsillar abscess, and parapharyngeal abscess. Their treatment varies, with more severe cases often

Large B-cell Lymphoma With Interferon Regulatory Factor 4 Rearrangement Presenting as a Primary Central Nervous System Lymphoma in the Brain.

Large B-cell lymphoma (LBCL) with interferon regulatory factor 4 (IRF4) rearrangement (LBCL-IRF4r) is a rare type of lymphoma with an overall favorable prognosis, and has been included as a distinct entity in the 2022 revision of the World Health Organization Classification of Lymphoid Neoplasms. More common in pediatric/young adult populations and rare in older adults, it typically presents in the head and neck, specifically, the Waldeyer ring, and rarely in other sites. To our knowledge, it has not been reported as a primary central nervous system (CNS) lymphoma in the English language medic

Heterogeneity of CD8 T-Cell Changes in Advanced Melanomas After Initiation of Immunotherapy.

Whole-body CD8+ T-cell PET imaging can detect spatial and temporal localization of CD8+ T cells. To obtain insight into early CD8+ T-cell response to immunotherapy in patients with melanoma, a highly immunogenic tumor, we performed serial PET imaging with the 1-armed CD8 antibody tracer 89ZED88082A. Methods: Immunotherapy-naïve adult patients with stage IV melanoma underwent PET scanning 2 d after receiving 10 mg of 89ZED88082A intravenously at baseline and 6-8 wk after initiation of standard-of-care immunotherapy. Tracer uptake in lesions, normal lymph nodes, and Waldeyer ring was assessed us

Kimura disease of the tongue base: a rare case diagnosed through cytological examination of Warthin-Finkeldey-type multinucleated cells.

Kimura disease (KD) is a rare chronic inflammatory condition that primarily affects Asian males and typically presents in the head and neck region. We describe an exceptionally rare case of KD involving the lingual tonsil of Waldeyer's ring in a 39-year-old Japanese man, marking only the second reported instance of lingual involvement and the first specifically affecting the tongue base. The patient presented with a well-circumscribed, 3.5-cm mass extending from the lingual tonsil to the epiglottis. Laboratory findings revealed significant peripheral eosinophilia (13.5%) and elevated serum IgE

Smoking-associated inflammation: A dual 18F-FDG PET/CT analysis of tonsillar and aortic inflammation using liver-normalized SUVmax.

OBJECTIVE: Cigarette smoking alters immune and vascular systems, yet its dual inflammatory impact on mucosal lymphoid tissues and arterial walls remains underexplored in vivo. This study employs fluorine-18-fluorodeoxyglucose (18F-FDG) positron emission tomography/computed tomography (PET/CT) with liver-normalized maximum standardized uptake value (SUVmax) to assess metabolic inflammation in Waldeyer's ring and the thoracic aorta across smoking groups. SUBJECTS AND METHODS: We retrospectively analyzed 18F-FDG PET/CT scans from 222 patients categorized by smoking status. Maximum SUV was record

[68 Ga]Ga-CXCR4 PET/CT imaging in high-grade glioma for assessment of CXCR4 receptor expression.

PURPOSE: Gliomas account for 75 % of primary malignant CNS tumors. High-grade glioma (CNS WHO grades 3 and 4) have an unfavorable treatment response and poor outcome. CXCR4 is a G protein-coupled receptor that plays an important part in the signaling pathway between cancer cells and tumor microenvironment. CXCR4 overexpression has been shown in a variety of cancers. In this study, we evaluate the potential value of [68Ga]Ga-Pentixafor as a PET/CT CXCR4-probe for in vivo assessment of CXCR4 expression in patients with high-grade glioma and its correlation with tumor grade. Significance of increased 18F-FDG uptake in determining pathological status in post-chemotherapy lymphoma cases involving head-neck region on PET-CT.

BACKGROUND: Currently, all follow-up cases of lymphoma showing fluorodeoxyglucose (FDG) avid uptake on positron emission tomography/computed tomography (PET-CT) in head-neck region undergo biopsy for tissue confirmation of diagnosis. There are no consensus or practice guidelines in literature pertaining, whether to biopsy all such FDG avid lesions post-chemotherapy. Hence, this study was conducted to determine the significance of PET-CT (SUVmax 5.0 or above) in determining remission or pathological status in these patients. METHODS: A retrospective cohort analysis conducted between July 2019

Mapping Human Immunity and the Education of Waldeyer's Ring.

The development and deployment of single-cell genomic technologies have driven a resolution revolution in our understanding of the immune system, providing unprecedented insight into the diversity of immune cells present throughout the body and their function in health and disease. Waldeyer's ring is the collective name for the lymphoid tissue aggregations of the upper aerodigestive tract, comprising the palatine, pharyngeal (adenoids), lingual, and tubal tonsils. These tonsils are the first immune sentinels encountered by ingested and inhaled antigens and are responsible for mounting the firs

Clinical characteristics and prognostic analysis of primary extranodal non-Hodgkin lymphoma of the head and neck.

OBJECTIVE: Primary extranodal non-Hodgkin's lymphoma (PE-NHL) of the head and neck is the second common site of extranodal lymphoma, accounting for approximately one-third of all extranodal non-Hodgkin's lymphoma (E-NHL). However, in recent years, large-scale PE-NHL case studies in China and worldwide are rare and not comprehensive enough. This work analyzed the clinical manifestations, pathological features, immunophenotypes and diagnosis of PE-NHL, as well as the factors affecting the treatment and prognosis. METHODS: A retrospective study was performed on 74 patients who were diagnosed wit

Atypical Kikuchi-Fujimoto Disease: FDG-PET Contribution To The Diagnosis.

UNLABELLED: Kikuchi-Fujimoto disease (KFD), also called histiocytic necrotizing lymphadenitis, is more common in young women and typically presents with small, painful, localized cervical lymphadenopathy that resolves spontaneously within a few weeks. Laboratory findings are variable. As many as 40% of KFD cases are reported to be painless, and up to 22% to be generalized lymphadenopathy. Therefore, malignant lymphoma could be a differential diagnosis of KFD. A histopathologic diagnosis is needed when it is difficult to distinguish KFD from lymphoma. KFD typically shows small, highly accumulat

Prevalence of IRF4 rearrangement in large B-cell lymphomas of the Waldeyer's ring in adults.

Diffuse large B-cell lymphoma (DLBCL) is the most common B-cell non-Hodgkin lymphoma (B-NHL) in adults. These lymphomas are classified according to gene expression profiling (GEP) into germinal center B-cell (GCB) and activated B-cell type (ABC). Recent studies have suggested new subtypes of large B-cell lymphoma, based on genetic and molecular alterations, among them is large B-cell lymphoma with IRF4-rearrangement (LBCL-IRF4). We used fluorescence in situ hybridization (FISH), GEP (using the DLBCL COO assay by HTG Molecular Inc), and next generation sequencing (NGS) to comprehensively charac

Unveiling the Enigmatic Adenoids and Tonsils: Exploring Immunology, Physiology, Microbiome Dynamics, and the Transformative Power of Surgery.

Within the intricate realm of the mucosal immune system resides a captivating duo: the adenoids (or pharyngeal tonsils) and the tonsils (including palatine, tubal, and lingual variations), which harmoniously form the Waldeyer's ring. As they are strategically positioned at the crossroads of the respiratory and gastrointestinal systems, these exceptional structures fulfill a vital purpose. They function as formidable "gatekeepers" by screening microorganisms-both bacteria and viruses-with the mission to vanquish local pathogens via antibody production. However, under specific circumstances, the

Neck Node Squamous Cell Metastasis from Unknown Primary and Mutagen Sensitivity: A Case Series.

Most of the neck node metastases from cancer of unknown primary (CUP) are squamous cell carcinomas (SCCs). The majority of which are human papillomavirus (HPV)-related, frequently show cystic morphology referring to Waldeyer's ring origin. Here, we report four cases of neck node SCCs metastases from CUP. In our institute, 432 patients with head and neck (HN) SCC underwent pretreatment mutagen sensitivity (MS) assay between 1996 and 2006. Among them, 4 patients ≤50 years of age had metastatic cervical nodes from CUP. The primary treatment was cervical node dissection ± radiotherapy. All patient
