Machine learning predictors of post-radiotherapy cardiotoxicity in a Latin American breast cancer cohort: Clinical risk factors outperform cardiac dosimetry.

BACKGROUND: Most evidence on post-radiotherapy cardiotoxicity (CDT) derives from Global North cohorts, and the relative contribution of baseline clinical risk versus cardiac dosimetry remains poorly characterized in Latin American populations. PURPOSE: To examine, in a Chilean breast cancer cohort, whether baseline clinical risk factors or cardiac dose-volume metrics better predict post-RT cardiac events, using supervised and unsupervised learning. METHODS: Retrospective cohort of 42 breast cancer patients treated with thoracic radiotherapy (RT, median heart dose 2.6 Gy; median follow-up 84.

Therapeutic Climbing in Pediatric Practice: Therapist Perspectives on Mechanisms, Delivery, and Contextual Factors - A Qualitative Study.

Therapeutic climbing is increasingly used in pediatric occupational therapy, yet implementation practices and reasons for non‑adoption remain underexplored. This qualitative study examined how Austrian pediatric occupational therapists position therapeutic climbing within occupation‑centered, movement‑based practice; how they deliver it in community contexts; perceived benefits and mechanisms; and barriers to adoption, including non‑user perspectives. Semi‑structured interviews were conducted with 24 state‑certified pediatric occupational therapy practitioners (12 climbing users, 12 non‑users)

Racial/ethnic disparities in grade point averages and absences among 9th graders in Georgia: the role of sleep differences.

PURPOSE: This study assessed racial/ethnic differences in grade point averages (GPA) and school absences among 9th graders in semi-rural Georgia, USA, and whether sleep duration and chronotype helped explain these differences. METHODS: This cross-sectional study of 406 ninth graders from two diverse high schools used counterfactual-based mediation analysis to examine whether sleep duration and chronotype mediated racial/ethnic differences in academic outcomes. Separate mediation models were estimated for GPA and school absences, specifying race/ethnicity as the exposure (Asian, Black, Hispani

From reference epigenomes to response epigenomes: a public-health case built on the developing brain.

Reference epigenome projects, such as the NIH roadmap, ENCODE, and the international human epigenome consortium, have started mapping the baseline regulatory states of human cell types with extraordinary detail. However, these projects were not designed to address the urgent questions posed by exposome science and child-health policy: how these states change after environmental exposure, in whom, during which developmental window, and whether they can recover. I argue that the missing object is a human response epigenome: the cell-type-specific change in epigenomic state over time following a

Myopic Shift in Patients During Hyperbaric Oxygen Treatment: A Systematic Review.

INTRODUCTION: CReversible myopic shift (MS) is a common side effect of hyperbaric oxygen treatment (HBO₂) resulting from refractive changes in the lens. Single studies indicate that the myopic shift increases with longer duration of HBO₂ and may differ between hyperbaric chamber types and oxygen breathing systems. This review aims to improve knowledge on the frequency and magnitude of myopic shift after standard-length HBO₂. METHODS: A systematic search for original studies of myopic shift after HBO₂ was conducted in medical databases. Included were published full-text articles in English wit

Evaluation of Two Practical Field Methods for Estimating Operational Overmilking Duration Using Standard Milking-System Sensors.

The objective of this study was to quantify the method-to-method variation between two widely used field indicators of the end-of-milking vacuum-exposure period (i.e., operational overmilking duration), and to identify cow- and milking-level factors associated with this variation. Operational overmilking was defined using two approaches: (i) MPC vacuum fluctuation patterns collected via VaDia™ recording devices, and (ii) milk flow curves generated from milking system data, with simulated ACR take-off thresholds ranging from 0.2 to 0.8 kg/min. Seven quarter combinations were analyzed to determi

Incidence of psychotic disorders by birth cohort: a population-based cohort study in Ontario, Canada.

BACKGROUND: Previous reports have suggested that the incidence of psychotic disorders has either been stable or decreasing over time in high-income countries, but overall trends may not capture birth cohort differences influenced by varying exposures to risk factors. We sought to assess whether the incidence of psychotic disorders has changed across birth cohorts. METHODS: We conducted a retrospective cohort study using linked health administrative data on all people born between 1960 and 2009 in Ontario, Canada. We identified diagnoses of schizophrenia spectrum disorder (SSD) and psychosis n

Short-Term Survival Analysis of Resin-Composite Restorations by Dental Students Using Real-World Data.

OBJECTIVES: The clinical skills of dental students with limited experience may be inferior to those of dentists. This study aimed to investigate the prognosis of resin-based composite (RC) restorations performed by dental students compared to those performed by dentists. METHODS: Data from patients who underwent RC restoration between April 2014 and March 2020 at a university hospital were analyzed. Because the database could not distinguish between re-restorations and new restorations on the same teeth, follow-up was initiated 6 months after the initial RC restoration. The follow-up period c

Determinants of AI Adoption in Healthcare: Insights From a Unified Theory of Acceptance and Use of Technology (UTAUT) Study Among Doctors and Nurses in a Tertiary Care Hospital in North India.

Background Artificial Intelligence tools are increasingly entering clinical practice, yet their adoption depends on how healthcare professionals perceive, experience, and intend to use them. Understanding factors associated with adoption is critical for designing targeted AI education. Objectives To assess acceptance of AI-based tools among doctors and nurses in a tertiary hospital in North India using a tool based on the Unified Theory of Acceptance and Use of Technology (UTAUT), and to identify early and late adopters based on prior AI exposure, training, and behavioral intention. Methods A

Integration of Time-Varying Pharmacometric Modeling With Cox Regression for Time-to-Event Analysis in NONMEM.

Although time-varying Cox regression modeling approaches have been developed, exposure-response analyses for time-to-event (TTE) endpoints often rely on static exposure covariates and may overlook the real-world dosing variability and drug concentration fluctuations over time. To better characterize pharmacokinetic (PK) or pharmacodynamic (PD) effects on TTE endpoints, a methodology was proposed to integrate time-varying pharmacometric models with Cox regression in the non-linear mixed effects modeling software, NONMEM. Clinical trial simulations were conducted with different sample sizes and

Interpreting multivariable regression coefficients in observational clinical research: The Table 2 fallacy.

Multivariable regression tables are common in observational clinical research, but their coefficients are often over-interpreted. A model built to estimate the effect of 1 exposure may also report coefficients for age, sex, comorbidities, behaviours, and other adjustment variables. These additional rows are frequently read as independent risk factors, even when the analysis was not designed to estimate their effects. This is the Table 2 fallacy. The problem is not the use of adjustment, but the interpretation of adjustment terms as if each were a separate causal estimate. In this commentary, w

Misspecified seasonality adjustment in temperature and preterm birth studies may introduce severe bias: a real birth cohort-based simulation.

BACKGROUND: Seasonality is typically controlled for to mitigate potential confounding by seasonally varying variables in environmental epidemiology research. We aimed to illustrate that correctly specifying the seasonality variable is crucial for effectively controlling confounding by seasonality in the context of prenatal temperature exposure and preterm birth. METHODS: We conducted a simulation study based on an Israeli birth cohort of 131 599 births in Southern Israel, 2005-19. Weekly mean temperature exposures throughout pregnancy and last menstrual period (LMP) date were taken from the o

Human gut microbiome as a composite health biomarker.

Using a cohort of 893 adults who underwent medical check-ups with rich data collection across diet, lifestyle, clinical labs, cardiac stress testing, and microbiome profiling, we show that a gut microbial health index (MHI), originally defined using a large meta-analysis of various human disease cohorts, is also substantially associated with lifestyle and diet. This association persists across the overall cohort, disease-free subsets, and individuals with common medical conditions such as hypertension and dyslipidemia. The gut MHI was positively correlated with factors such as variation in car

Derivation of evidence-based oral-to-parenteral uncertainty factors for medical device toxicological risk assessment.

Uncertainty factors (UFs) are used in toxicological risk assessment to address uncertainty when extrapolating from available data to intended exposure scenarios. In medical device biocompatibility evaluation, oral toxicity data are frequently extrapolated to parenteral exposures, for which ISO 10993-17 specifies a default oral-to-parenteral UF of 10. Although the standard permits lower values when bioavailability data are available, such data are uncommon for medical device constituents. In this study, in silico physiologically based pharmacokinetic (PBPK) modeling was used to estimate oral bi

Context-dependent toxicity of diclofenac in Daphnia magna: roles of water chemistry, neonate source and preparation procedure.

Daphnia magna is a well-established reference organism for ecotoxicological studies of pharmaceuticals. Standardized acute toxicity protocols specify the composition of test media, and any alternative water type must be fully characterized and reported. In the case of acute toxicity testing for D. magna, neonates are typically employed as test organisms. Despite the strict methodological framework, reported EC50 values for diclofenac (DCF) in D. magna vary considerably, ranging from 22.4-123.3 mg L-1, with test water being the most unreported factor. Therefore, in our study, we investigated th

Nicotine Absorption During Tobacco Harvesting: A Scoping Review of Urinary Cotinine Levels Among Tobacco Farm Workers.

OBJECTIVE: Tobacco farm workers have a high prevalence of acute nicotine poisoning, known as green tobacco sickness, caused by dermal exposure during leaf harvesting. Although urinary cotinine is the most reliable biomarker of nicotine exposure, methodological inconsistencies across studies hinder the identification of absorption patterns and limit comparability. This scoping review aimed to summarize methods used to assess urinary cotinine, identify individual and occupational factors influencing its measurement, and examine temporal patterns of urinary cotinine levels. METHODS: The review f

Machine Learning for Dynamic and Short-Term Prediction of Preeclampsia Using Routine Clinical Data.

IMPORTANCE: Preeclampsia is a leading cause of maternal and perinatal morbidity and mortality, yet its unpredictable onset and rapid progression hinder timely management. Existing prediction tools often rely on specialized biomarkers, static assessments, or limited study cohorts, impeding clinical utility and generalizability. OBJECTIVE: To develop and validate machine learning models for dynamic, short-term prediction of preeclampsia onset using longitudinal electronic health record (EHR) data.
