A long-term experience with dissemination of tumor of different parts of the esophagus was evaluated using the results of resected material examination and post-operative follow-up. Schemes of treatment for different degree of metastatic spread of esophageal cancer are discussed. An analysis is presented of late results in 144 reconstructions of oesophagus with transplants (137 isoperistaltic, 7 anisoperistaltic) from transverse colon for correction of stricture, mainly after corrosive trauma. The grafts were interposed retrosternally in 126 cases and intrathoracically in 18. Delayed passage due to excessive size of the colon segment was managed in four cases by plication of the intra-abdominal part of the graft or by jejunal by-pass. The structure and arrangement of muscle fibres and vessels in various parts of the esophagus was investigated in 22 dogs using light and scanning electron microscopical techniques. A new one-stage surgical technique for postlaryngectomy speech was applied to 30 patients and its results are described. Attention is directed to the synchronous and sometimes delayed appearance of squamous carcinoma of the oesophagus with other head and neck squamous carcinomas. The rare occurrence of double carcinoma of the oesophagus is emphasized. Two cases of double carcinoma of the oesophagus occurred in a group of 383 patients who were investigated as part of an epidemiological survey (an incidence of 0,5%). A third case of double carcinoma of the oesophagus is additionally reported.

The best procedure for substituting the oesophagus is the interposition of colon. Leakage of the cervical anastomosis is the most frequent complication. In our own material this complication was more frequent after anisoperistaltic interposition. There is no criterion for more severity with a second layer. For replacement of the cervical part of the oesophagus we used revascularized jejunal-autografts twice with success.

Cases of esophageal diverticula treated surgically at the Department I of General Surgery, Institute of Surgery from 1950 to 1973 are reported. Their location, clinical manifestations, indications for operation and management are described. During these 23 years 45 patients were treated surgically. In 21 cases the diverticula were situated in the cervical part of the esophagus, in 14 cases in the middle part and in 10 cases in the supradiaphragmatic part.
