Comparative analysis of outcomes and costs of lung and esophageal resection: epidural analgesia vs peripheral nerve block.

BACKGROUND: Thoracic epidural analgesia (EA) has historically been a key component of multimodal analgesia after open thoracotomy. EA use has decreased over time, however, and peripheral nerve blocks (PNBs) are increasingly used as alternatives. This study aimed to compare outcomes among patients receiving EA versus PNB as part of a lung or esophageal operative procedure. METHODS: Adult patients who received EA or PNB as part of a lung or esophageal operative procedure from 2016 to 2020 were identified in the Nationwide Readmissions Database. Outcomes included complications, 90-day readmissio

Carinal tracheo-oesophageal fistula repair using adjacent gastric conduit wall after Ivor-Lewis oesophagectomy: a case report.

BACKGROUND: Neo-oesophagus–airway fistula after transthoracic oesophagectomy is rare but life-threatening. Standard therapeutic options, including stenting, primary repair with muscle or omental interposition, and diversion with delayed reconstruction, can be contraindicated in the context of sepsis and significant mediastinal contamination. We describe an emergency, staged approach employing the adjacent viable gastric conduit wall as a local flap to achieve closure of a carinal fistula in a hemodynamically unstable patient. CASE PRESENTATION: A 75-year-old man with distal oesophageal squamou

Real-World Experience with Lenvatinib plus Pembrolizumab in Metastatic BRAF Wild-Type Anaplastic Thyroid Carcinoma.

INTRODUCTION: BRAF wild-type anaplastic thyroid carcinomas (BRAFWT-ATCs) have a poor prognosis, in part, due to limited effective therapies. The preliminary results from the phase II ATLEP trial suggest the promising clinical activity of lenvatinib plus pembrolizumab (L/P). We aimed to confirm the real-world efficacy of L/P in BRAFWT-ATC. METHODS: A retrospective single-center study of patients with BRAFWT-ATC treated with first-line L/P between January 2016 and July 2024 outside of a clinical trial. The primary endpoints were confirmed overall response rate (ORR), progression-free survival (

Erbin deficiency exacerbates sepsis‑induced liver injury through mitochondrial ROS/NLRP3‑mediated inflammatory responses.

BACKGROUND: Idiopathic pulmonary fibrosis (IPF) is a rare interstitial lung disease with a poor prognosis that currently lacks effective treatment methods. Preventing the acute exacerbation of IPF, identifying the molecular subtypes of patients, providing personalized treatment, and developing individualized drugs are guidelines for predictive, preventive, and personalized medicine (PPPM / 3PM) to promote the development of IPF. Oxidative stress (OS) is an important pathological process of IPF. However, the relationship between the expression levels of oxidative stress-related genes (OSRGs) an

Aberrant Right Subclavian Arteries in Dogs Have a Prevalence of 1.2% and Are More Likely to be an Incidental Finding on Computed Tomographic Studies of the Thorax.

Aberrant right subclavian arteries (ARSAs) are a form of vascular ring anomaly (VRA) in dogs and the most common VRA in people. To date, there has been no large-scale study on ARSA in dogs and their potential clinical significance. For part one, a single-center retrospective observational study was performed to determine the prevalence of ARSAs in a population of 1000 dogs undergoing contrast-enhanced CT for various reasons. For part two, further canine ARSA cases were collected to characterize their imaging features further and determine whether any imaging findings were more frequent in dogs

Laparoscopic hyperspectral imaging for in vivo detection of the vagal nerve in upper gastrointestinal surgery.

BACKGROUND: Accurate intraoperative detection of nerves in critical anatomic regions during oncologic resection, such as the vagal nerve, is crucial. The vagal nerve regulates many internal organs and is at risk during surgeries, potentially leading to severe complications. While different types of intraoperative neuromonitoring methods exist for functional assessment, Hyperspectral Imaging (HSI) offers a non-invasive, real-time alternative for morphologic identification. This study is, to our knowledge, the first to assess laparoscopic HSI for imaging and delineating the vagal nerve during mi

Breath- and blood-based molecular assessment for gastroesophageal cancer.

Gastroesophageal cancer remains a significant health care problem characterized by diagnosis at advanced stages, rendering a significant proportion of patients ineligible for curative treatment. Conversely, early diagnosis and treatment of gastroesophageal cancer demonstrates markedly improved outcomes, with 5-year overall survival rates ranging from 83% to 96%. To date, gastroesophageal cancer surveillance primarily focuses on patients with Barrett's esophagus, relying on invasive upper endoscopy. However, despite its high specificity, upper endoscopy demonstrates a suboptimal cancer yield an

Extraesophageal reflux: Clinical manifestations and tools for diagnosis and treatment.

Extraesophageal reflux is a complex clinical entity, classically presenting with laryngopharyngeal symptoms including chronic cough and vocal changes, but it is also implicated in conditions such as subglottic stenosis and lung injury. Diagnosis is challenging, in large part due to the oftentimes vague presenting symptoms with multiple possible etiologies, as well as limited consistency of currently available diagnostic tests. Furthermore, effective medical treatment is limited, and acid suppression therapy such as proton pump inhibitors has shown low to mixed efficacy in relieving signs and s

Peak oxygen uptake in combination with ventilatory efficiency improve risk stratification in major abdominal surgery.

This pilot study aimed to evaluate if peak VO2 and ventilatory efficiency in combination would improve preoperative risk stratification beyond only relying on peak VO2 . This was a single-center retrospective cohort study including all patients who underwent cardiopulmonary exercise testing (CPET) as part of preoperative risk evaluation before major upper abdominal surgery during years 2008-2021. The primary outcome was any major cardiopulmonary complication during hospitalization. Forty-nine patients had a preoperative CPET before decision to pursue to surgery (cancer in esophagus [n = 18], s

Edge roughness quantifies impact of physician variation on training and performance of deep learning auto-segmentation models for the esophagus.

Manual segmentation of tumors and organs-at-risk (OAR) in 3D imaging for radiation-therapy planning is time-consuming and subject to variation between different observers. Artificial intelligence (AI) can assist with segmentation, but challenges exist in ensuring high-quality segmentation, especially for small, variable structures, such as the esophagus. We investigated the effect of variation in segmentation quality and style of physicians for training deep-learning models for esophagus segmentation and proposed a new metric, edge roughness, for evaluating/quantifying slice-to-slice inconsist
