Suicide and schizophrenia in Singapore--a fifteen year follow-up study.

This is a prospective follow-up study of 482 schizophrenic patients. At the end of 15 years, 41 had died of suicide and 30 had died of natural causes, giving a suicide rate of 567 per 100,000 per year. Suicide patients were significantly younger and experienced less delusion at the time of their first presentation. There were otherwise no significant differences in the socio-demographic or clinical variables between the suicide group and the controls (surviving patients). Jumping from height was the commonest method of suicide and our rate of suicide for schizophrenia is comparable to those re

[Clustering of suicides under age 20--seasonal trends and the influence of newspaper reports].

The existence of temporal clustering of suicides was analyzed by comparing the distribution of weekly numbers of suicides under age 20 in Japan (1982-1987) with a Poisson distribution. The linkage between newspaper reports about teenage suicides and these clusters of suicides was examined in an ecologic study. The following results were obtained: 1) The distribution of weekly numbers of suicides under age 20 was not consistent with a Poisson distribution. The observed numbers for weeks with clustering was higher than the expected numbers. Therefore it is suspected that teenage suicides was not

A clinical study of schizophrenic suicides. 42 cases in Taiwan.

A clinical study of 25 male and 17 female schizophrenic suicides is presented. Jumping from a height was the most common method used. The 42 suicidal schizophrenics in Taiwan were compared with both 84 sex- and age-matched and 60 5-year illness course non-suicidal schizophrenic control groups. The suicidal schizophrenics were not significantly different from the non-suicidal counterparts of both control groups with regard to age, sex, ethnicity, religion, educational background, the presence of suicide cases in the family history, and the presence of insight, but were significantly different i

Suicide on the London Underground System.

Over the past 50 years there has been an increase in the numbers of people jumping/falling in front of trains on the London Underground system. Case-fatality rates have fallen from 70% in the 1950s to 55% today. The proportion certified as suicide has fallen while the proportions certified as accidents or open verdicts have risen. There is unusual clustering of events at some stations which are adjacent to psychiatric units. The hypothesis that ease of access to London Underground stations may sometimes be a determinant of suicide is investigated.

[Secondary combined suicide viewed by the wife].

A 47-year-old man was found dead at the bottom of a wall outward the door of the cellar of his house. Preliminary physical examination at the scene of the death revealed a laceration of the scalp, an incised wound in front of the neck and several stab wounds in the left chest penetrating through the clothing. Questioned by the police his wife told that her husband who suffered from severe depression had inflicted the incised and the stab wounds himself, had made an unsuccessful attempt at self-strangulation and had finally jumped down from the wall beside the stairs declining to the cellar of

Empirical indicators of near-term suicide risk.

In practice, clinicians estimating the likelihood of suicide are most concerned with short-term risk. To address this issue, an analysis of prospective data from 3,005 psychiatric patients at risk for suicide was carried out, focusing on 38 persons who committed suicide within 60 days of evaluation. Nine high-risk variables were identified: prior psychiatric hospitalization, contemplation of hanging or jumping, presence of suicidal impulses, divorced, a threat of financial loss, feeling a burden to others, unable to cry or severe crying, severe or moderate ideas of persecution or reference, an

Suicide and parasuicide in Singapore (1986).

Suicide and parasuicide rates have increased in Singapore since 1980 and in 1986 were estimated to be 13.1 per 100,000 and 92 per 100,000 respectively. Those most prone to suicidal behaviour are the Indians and the aged above 70 years. Jumping from a height is still the commonest method of suicide and self-poisoning is the most popular method of parasuicide.

Suicide from river bridges.

The opening of a new high river bridge in Brisbane allowed a naturalistic experimental testing of whether the sample engaging in suicidal behaviour from the new bridge was similar to that from the adjoining older bridge. Substantial differences were found for the two samples. This suggests that persons prevented from jumping from one bridge, for example by a barrier, will not automatically jump from the alternative bridge although a minority may do so.

Suicide by jumping from high-rise hotels. Fulton County, Georgia, 1967-1986.

During a 20-year period from 1967 through 1986, 19 suicidal jumps from high-rise hotels (HRHs) accounted for 24% of all fatal jumping episodes and 1% of all suicides in Fulton County, Georgia, U.S.A. The rate of suicidal jumps from HRHs did not increase during the study period. The number of fatal jumps per hotel-year correlated with the height of the interior hotel atrium. The mean age for all victims was 34 years, and 63% of victims were white males. The majority of victims were local residents who were alone when they jumped and were not registered hotel guests. Registered guests tended to

[Epidemiologic analysis of spinal injuries resulting from high falls].

A series of 1365 patients treated between 1965 and 1987 because of spine injury resulting from the fall from the height was analysed. A fall from the height may cause hyperflexion, axial loading or hyperextension type of injury. Hyperflexion injury occurs usually after fall from horsedrawn wagon, scaffold, roof or pillar. Axial loading injuries dominate in falls from the ladder, tree, suicidal jumps and falls from lower height. There is remarkable correlation among the height of the fall, degree of nervous system injury and results of management.

Suicide among Chinese schizophrenics in Hong Kong.

A descriptive study of suicides in 74 Chinese schizophrenic out-patients (43 male) is presented. The mean age at death was 31.3 years. The mean duration of illness was 8.5 years and 50 of the 74 died within ten years of onset of illness. Only five lived alone; 35 were openly employed and 20 were married at the time of death. Twenty had a history of depression, and 27 had attempted suicide previously. More females had been depressed or had attempted suicide than males. Over half were last admitted for reasons other than schizophrenic symptoms alone and ten died within one month of discharge. Ei

Suicide by jumping from buildings.

A study of 82 suicides during 1982-1986 that were committed by jumping from buildings was performed using the case files of the Office of the Medical Examiner of Metropolitan Dade County in Miami, Florida, U.S.A. The analysis involved the age, race, and sex of the victim along with the results of toxicologic studies and the cause of death. Additionally, the decedent's place of residence, location of the suicide, height of the building, and the reason for the suicide were investigated. Whether or not a note was left by the decedent was also ascertained. The suicidal "jumper" most commonly was a

Suicide and related behaviour from river bridges. A clinical perspective.

A survey was conducted of 47 suicides, 16 failed suicides, and 24 attempted suicides that occurred from Brisbane river bridges over 15 years. Compared with findings from other suicide surveys, subjects of bridge suicides and failed suicides had a much higher rate of schizophrenia (46%), with hallucinations often precipitating the jump. They also had extensive histories of previous self-harm. Those who attempted suicide by jumping had a higher rate of personality disorder (58%) compared with findings from other attempted suicide surveys, and had very extensive histories of previous self-harm wh

Vertical trauma: injuries to patients who fall and land on their feet.

We reviewed the patterns of injuries sustained by 12 consecutive fallers and jumpers in whom primary impact was onto the feet. The fall heights ranged from 20 to 100 ft. The 12 patients sustained 49 significant injuries. Skeletal injuries were most frequent and included 15 lower extremity fractures, four pelvic fractures, and nine spinal fractures. In two patients, paraplegia resulted. Genitourinary tract injuries included bladder hematoma, renal artery transection, and renal contusion. Thoracic injuries included rib fractures, pneumothorax, and hemothorax. Secondary impact resulted in several

The geographical pattern of methods of suicide in Belgium: implications for prevention.

The geographical variation of reported suicide among the 43 Belgian districts was analysed for each method of suicide in both sexes and in two time periods: 1968-72 and 1978-81. Different descriptive methods were used to assess the patterns and changes in method-specific indirectly standardized death rates. It appeared that the overall suicide pattern was not always a good reflection of the method-specific patterns. Moreover, these seemed consistent in time and between sexes. Two important methods, which could be partially influenced, showed a remarkable concentration in some southern district

Attempted suicide by jumping: clinical and social features.

A group of 46 persons who had attempted suicide by jumping was compared on clinical and social-demographic parameters with another group of 214 persons who had attempted suicide by drug overdose. The differential characteristics of persons attempting suicide by jumping are the following: more often men, of a more advanced age, married or widowed, more often suffering from major psychopathology (that is affective psychosis-depressive type or schizophrenia), and quite frequently having a serious somatic illness.

Injuries in attempted suicide by jumping from a height.

This study was undertaken to investigate the pattern of injuries and the management of 28 patients who attempted suicide by jumping from a height. Most of the patients had serious psychiatric disorders. All but two of the patients sustained multiple injuries. The most common injury patterns were fractures of the spine and lower extremities. It was concluded that, in addition to urgent orthopaedic and surgical care, early psychiatric assessment and therapy with transfer to a psychiatric department as soon as possible facilitates the recovery and rehabilitation of these patients.

A model for the visual perception of direction.

An attempt is made to explain how the eyes can be moved with respect to the head and the observer can still assess the direction of an object in space fixed with respect to the head. My explanation is based on the assumption that the subject is made aware of the direction in which the eyes are pointing by the output from the brain center controlling the direction of fixation. He associates this signal with an awareness of "looking in a certain direction." The image falling on a certain point of the retina (usually near the center of the fovea), which I call the anchor point, is perceived as ly
