At the Department of Orthopaedics and Traumatology, Helsinki University Central Hospital, an acute accident unit with more than 3,000 trauma admissions per year, a protocol was kept over the years 1982-84 of patients who were treated for injuries sustained from intentional falls from heights. The number of patients included in the series was 73. The mean age of the patients was 29.8 (range 15-65) years. The male:female ratio was 1.5:1. In an attempt to identify factors contributing to survival of free fall and impact, we evaluated the records of four patients who survived a jump from the Brooklyn Bridge into the East River in New York Harbor between 1977 and 1985. The authors suggest to reserve the expression "combined" suicide to cases, where a combination of several suicide methods were previously intended. The other cases, where the victims proceed from ohne failing attempt to the other by using different methods and finally succeed after a certain period should be called "protracted" or "prolongated" suicides. Two typical cases are presented. In the first case a young man tried to commit suicide by causing a traffic accident, then tried to stab himself. This study investigated suicides by persons aged ten to nineteen during the years 1978 to 1982 in the City of Montreal. Records of death in the Office of the Coroner, City of Montreal, were examined and suicides designated according to standard criteria. Age, sex, method of suicide, and month of death were noted. This report reviewed 996 emergency room visits and 279 hospital admissions of patients with complications of cocaine abuse seen at the San Francisco General Hospital between 1979 and 1986. Peripheral vascular injuries associated with falls from heights are uncommon. 13 patients who had used violent means of parasuicide, e.g. jumping from a height and hanging were compared with 19 wrist-slashers. The former had significantly higher 24-hour urinary cortisol levels and significantly lower 24-hour norepinephrine to epinephrine ratio. In a study of suicide in Singapore in 1980 there were 230 cases and the suicide rate for the general population was 9.5 per 100,000. The age-specific rates increased steeply after 50 years and there was a male preponderance especially in the age group 60 years and over. Coroners' statistics indicated there were 684 suicides for the whole of Hong Kong in 1981. Autopsy findings in 10 cases of high-velocity impact with water--all suicides--are demonstrated. The injuries vary from no lesion at all to severe injuries, the fatal injuries predominately located to the thorax and the spine. All but one victim fell 35-37 meters; the last one fell 51 meters. The cause of death was drowning in six cases; in four cases, injuries were responsible for the fatal outcome. Head and neck injuries of any importance were not observed in the present investigation.
