This investigation centers on the case of an adult male whose finger was allegedly amputated by the steering wheel of his car during a crash. The subject claimed to have been driving with his left index finger inserted through a hole in the spoke of his steering wheel and was subsequently involved in an offset frontal collision with a tree. The finger was found to be cleanly severed at the mid-shaft of the proximal phalanx after the crash. This injury was alleged to have been caused by inertial loading from the rotation of the steering wheel during the crash. To determine whether this injury m

BACKGROUND: Analysis of the mechanism and severity of injury over time may permit a more focused planning of acute care and trauma prevention programs.
METHODS: A retrospective, population-based study examining severe traumatic injury in a single county was undertaken. Three overlapping data sets were used to form a composite injury data set.
RESULTS: There were 55,664 patients included in the study. A total of 40,897 (73.5%) patients survived and 14,767 (26.5%) died. Of those patients who died, 8,910 (60.3%) died in the field and were not transported to a trauma center. There was an increas

Deaths due to decapitation are rare in civilian populations. A study in South Australia over 17 years from January 1986 to December 2002 revealed 20 cases (age range, 14 to 72 years; average, 30.7 years; male to female ratio, 15:5). There were 13 suicides consisting of 11 train-related deaths (age range, 14 to 72 years; average, 31.3 years; male to female ratio, 10:1), 1 hanging death, and 1 death using a ligature and a motor vehicle. Five cases related to motor vehicle crashes (age range, 19 to 46 years; average, 30.2 years; male to female ratio, 1:4) and 2 were associated with industrial tra

The annual report of the Regional Infant and Child Mortality Review Committee (RICMRC) is attached. This Committee has as its mission the review of infant and child deaths so that information can be transformed into action to protect young lives. The 2003 review area includes South Dakota's Minnehaha, Turner, Lincoln, Moody, Lake, McCook, and Union Counties. In 2003 there was one death in this region due to Sudden Infant Death Syndrome (SIDS), plus two infant deaths listed as undetermined that could potentially represent SIDS. Two of these three deaths had potential risk factors of soft beddin

Injuries account for 75% of all deaths among American Indian and Alaska Native (AI/AN) children and youth, and AI/ANs have an overall injury-related death rate that is twice the U.S. rate for all racial/ethnic populations. However, rate disparities vary by area and by cause. To help focus prevention efforts, CDC analyzed injury mortality data by Indian Health Service (IHS) administrative area and by race/ethnicity. This report summarizes the results of these analyses, which indicate that although death rates for some causes (e.g. drowning and fire) have shown substantial improvement over time,

As the elderly population increases and they lead more active and healthy lifestyles, their exposure to the threats of injury multiply. Undoubtedly, the geriatric population will comprise a growing percentage of trauma patients. The role of alcohol and drug use in geriatric trauma has not been clearly defined. The purpose of this study is to determine the incidence of alcohol and illicit drug use in association with mechanism of injury in all elderly trauma patients presenting to level I and II trauma centers in the State of Illinois over 3 years. A retrospective analysis was performed on 3 ye

The Health Status Indicators (HSIs) were developed as part of the Healthy People 2000 process to facilitate the comparison of health status measures at national, State, and local levels. In this report national trends in racial and ethnic-specific rates for 17 HSIs are examined for the period from 1990-8. One of three overarching goals of Healthy People 2000 was to reduce health disparities. Examination of trends in the HSIs indicates that rates for most racial/ethnic groups improved. Rates for American Indian or Alaska Natives did not improve for six of the HSIs. An index of disparity, a summa

BACKGROUND: In the United States, only motor vehicle crashes and cancer claim more lives among children than do firearms. This national study attempts to determine whether firearm prevalence is related to rates of unintentional firearm deaths, suicides, and homicides among children.
METHODS: Pooled cross-sectional time-series data (1988-1997) were used to estimate the association between the rate of violent death among 5-14 year olds and four proxies of firearm availability, across states and regions.
RESULTS: A statistically significant association exists between gun availability and the ra

Adolescent morbidity and mortality are more often due to preventable causes and to risky behavioral choices than to "natural" causes, such as cardiovascular disease or cancer. The leading causes of death among adolescents in Western, industrialized nations are unintentional injuries, especially motor vehicle crashes, homicide, and suicide. The physical and cognitive development of adolescents also results in increased risky behavioral choices, and to high rates of sexually transmitted diseases, substance use and misuse, and inadequate nutrition. These lifestyle choices also have repercussions

PROBLEM/CONDITION: Priority health-risk behaviors, which contribute to the leading causes of mortality and morbidity among youth and adults, often are established during youth, extend into adulthood, are interrelated, and are preventable.
REPORTING PERIOD COVERED: This report covers data during February-December 2001.
DESCRIPTION OF SYSTEM: The Youth Risk Behavior Surveillance System (YRBSS) monitors six categories of priority health-risk behaviors among youth and young adults; these behaviors contribute to unintentional injuries and violence; tobacco use; alcohol and other drug use; sexual

A curious autopsy case following the car crash of a 20-year-old male, in which self-strangulation and lung collapse were observed, is presented. His motor vehicle had crashed into a restaurant as a result of self-strangulation using an electrical cord wound four times around his neck. At autopsy, we found small rupture holes of spontaneous bullae in both lung apices, which had probably taken place upon collision during driving, petechial hemorrhages in the face skin and the absence of severe injuries. Since it seemed unlikely that the small holes in both lung apices caused fatal pneumothorax i

OBJECTIVES: To describe the problem of falls, motor vehicle related injuries, and suicide among older adults, and issues related to their prevention.
METHODS: Summary and synthesis of selected literature.
RESULTS: About 39 000 adults aged 65 and older die each year in the United States from injuries; worldwide this annual toll is about 946 000 persons. The top three causes of injury related death in this age group in the United State are falls, those related to motor vehicle crashes, and suicide. Effective strategies exist for preventing fall related injuries and deaths. Preventing injuries

Injury is recognised internationally as the major threat to adolescent health. The purpose of this study was to describe the epidemiology of adolescent fatal injury in New Zealand, and to examine opportunities for prevention. National mortality data were searched to identify all 15-19 year-olds, who died from injuries in the period 1986-1995. Leading causes of injury were reviewed in light of known risk factors, injury mortality rates in other industrialised countries, and available prevention strategies. The results showed that injury accounted for 2,095 deaths (72.8 per 100,000 person years)

BACKGROUND: Injury is the leading cause of death among Canadian children between 1 and 19 years, and accounts for one sixth of all hospitalizations of children between 0 and 19 years. We examined the causes of injury in Manitoba children, and the relationship between injury rates and region of residence, premature mortality rate (PMR), and income.
METHODS: Regional differences in injury death and hospitalization rates, and causes of injury were derived from the Population Health Research Data Repository. The relationship between injury rates and area income levels was assessed and correlation

OBJECTIVE: To describe the epidemiology of drowning in New Zealand for the period 1980-94.
METHODS: Drowning-related incidents were identified by linking New Zealand Health Information Service and Water Safety New Zealand databases.
RESULTS: 2,606 drowning-related incidents were identified. In three-quarters of the incidents, drowning was listed as the immediate cause of death. The majority of drownings were unintentional (85%), involved males (76%), and 0-4 and 15-24 year age groups had the highest rates. Boating was the leading cause of unintentional drowning (28%) followed by swimming and

Approximately two thirds of all deaths among children and adolescents aged 5-19 years result from injury-related causes: motor-vehicle crashes, all other unintentional injuries, homicide, and suicide. Schools have a responsibility to prevent injuries from occurring on school property and at school-sponsored events. In addition, schools can teach students the skills needed to promote safety and prevent unintentional injuries, violence, and suicide while at home, at work, at play, in the community, and throughout their lives. This report summarizes school health recommendations for preventing un
