Case Report: Retroperitoneal bronchogenic cyst of imaging features and pathological analysis in a young female.

Bronchogenic cysts are cystic lesions arising from the bronchial epithelium. They are very rare in the retroperitoneum. In this report, we present the case of a young woman found to have a cystic mass in the hepatorenal space during a routine health examination. Imaging studies confirmed a cystic lesion, and surgical excision revealed adherence to the lower esophagus and gastroesophageal junction. This retroperitoneal location, along with non-specific imaging features overlapping with other benign masses, poses a significant diagnostic challenge. The imaging appearance of bronchogenic cysts ca

Integrative transcriptomic profiling reveals molecular signatures of disease progression from gastroesophageal reflux disease to Barrett's esophagus: A foundation for single-cell resolution studies.

BACKGROUND: Barrett's esophagus (BE) represents a critical precancerous lesion arising from chronic gastroesophageal reflux disease (GERD), with significant risk of progression to esophageal adenocarcinoma. Despite advances in transcriptomic technologies, including single-cell RNA sequencing (scRNA-seq), the molecular mechanisms underlying the GERD-to-BE transition remain incompletely understood. Comprehensive transcriptomic profiling at both bulk and, prospectively, single-cell resolution is essential for identifying disease-driving molecular signatures and cellular heterogeneity. GastroMalign: Vision Transformer-Based Framework for Early Detection and Malignancy-Risk Stratification for High-Risk Gastrointestinal Lesions.

Background: Current artificial intelligence (AI) systems in gastrointestinal (GI) endoscopy primarily emphasize binary detection or static classification, providing limited support for the graded assessment of malignant potential that underpins clinical decision-making. We developed GastroMalign, a transformer-based framework designed to stratify GI lesions according to ordinal disease severity while maintaining clinical interpretability, addressing this unmet need in endoscopic risk assessment. Methods: This retrospective development and validation study used the publicly available GastroVisi

Enlarging Inflammatory Granulation Tissue After Endoscopic Submucosal Dissection at the Esophageal Inlet During Intensive Stricture Prophylaxis: A Case Report.

Surveillance endoscopy in a 47-year-old woman with previous hypopharyngeal chemoradiotherapy and esophageal endoscopic submucosal dissection (ESD) revealed a 12-mm superficial squamous cell carcinoma at the cervical esophagus near the inlet. ESD achieved en bloc resection with a half-circumferential mucosal defect, and histopathology showed pT1a lamina propria mucosae with negative margins. For stricture prophylaxis, triamcinolone was injected into the post-ESD defect, oral prednisolone was tapered over several months, and prophylactic endoscopic balloon dilation was performed three times at 3

Follicular dendritic cell sarcoma associated with IgG4-related disease: a case report and literature review.

Follicular dendritic cell sarcoma (FDCS) is a rare malignant tumor involving lymph nodes and extranodal sites, classified into conventional and inflammatory pseudotumor (IPT)-like variants. The IPT-like variant predominantly involves the liver and spleen. Immunoglobulin G4-related disease (IgG4-RD) is a systemic immune-mediated disorder characterized by multiorgan involvement, whereas isolated esophageal and gastric involvement is exceedingly rare. Although FDCS may exhibit infiltration of IgG4-positive plasma cells, the coexistence of FDCS and IgG4-RD has not been previously reported. Herein,

Immediate Weight-Bearing and Early Ambulation Rehabilitation After Femoral Neck System Fixation for Undisplaced Femoral Neck Fractures: A Prospective Randomized Controlled Trial.

OBJECTIVE: 11 beta-hydroxysteroid dehydrogenase type 1 (11 beta HSD1) converts inactive cortisone to active cortisol. 11 beta HSD1 activity is increased in GH deficiency and inhibited by GH and IGF-I in acromegaly. However it is not known whether these changes in cortisol metabolism exert significant effects during hydrocortisone therapy, and the effect has not been studied in patients taking cortisone acetate. We have studied the effect of GH induced 11 beta HSD1 inhibition in hypopituitary adults with severe GH deficiency to determine whether this inhibition has a different magnitude of effe

The Impact of Ultracentral Tumor Location on Outcomes in Patients with Pulmonary Oligometastases: A Secondary Analysis of the Single-Arm Phase 2 SABR-5 Trial.

PURPOSE/OBJECTIVES: There are limited data on outcomes in patients with ultracentral pulmonary oligometastases treated with SABR. The purpose of this study was to determine whether ultracentral location was prognostic for toxicity and survival. MATERIAL AND METHODS: Oligometastatic lung lesions treated on the single-arm phase 2 SABR-5 trial were retrospectively stratified into 2 cohorts: ultracentral tumors (UC), defined as planning target volume overlap or direct tumor abutment to the proximal bronchial tree, esophagus, great vessels, or heart, and nonultracentral tumors. Cohorts were compar

Lesion Transmurality and Continuity of Non-Occlusive Cryoballoon Ablation on Canine Ventricle.

BACKGROUND: Our understanding of lesion transmurality and continuity of non-occlusive cryoballoon ablation (NOCA) is limited. In the present study, lesion dimensions under different conditions during NOCA were assessed. METHODS: Simulated NOCA was performed on freshly harvested canine left ventricular myocardial using the cryoballoon. We conducted experiments to evaluate the effects of (1) flow rate (0, 1, and 1.5 L/min) and freezing time (120, 150, and 180 ) on lesion dimensions during segmental NOCA and (2) overlapping manners between two sequential cryoablations (overlaps of half and two-t

A Case of Esophageal Retention Cyst with High Fluorodeoxyglucose Uptake on PET/CT Scan.

INTRODUCTION: Esophageal retention cysts are rare, benign lesions that can mimic submucosal tumors. Their clinical presentation and imaging characteristics may lead to diagnostic challenges, particularly when fluorodeoxyglucose-positron emission tomography/CT (FDG-PET/CT) shows increased uptake, raising suspicion of malignancy. CASE PRESENTATION: A 77-year-old man presented with epigastric pain. Upper gastrointestinal endoscopy revealed an esophageal mass, prompting referral to our hospital. Endoscopic ultrasonography (EUS) identified a hypoechoic submucosal tumor with multiple cystic compone

A Rare Pulmonary Complication of Esophageal Carcinoma: A Case of an Acquired Esophago-Pulmonary Fistula.

Esophageal-respiratory fistulae are abnormal communications between the esophagus and the respiratory tract, most commonly appearing as tracheoesophageal or bronchoesophageal fistulas. Esophago-pulmonary fistulas represent a rare subtype, typically associated with malignancy, and may lead to severe complications such as lung abscesses. We report a case of a 58-year-old male patient who presented with a two-week history of fever, foul-smelling mucoid sputum, dyspnea, dysphagia, and weight loss. Imaging revealed a distal esophageal mass with a fistulous communication into the right lower lobe an

Diagnostic Challenges in Sessile Serrated Lesions: Progression to Adenocarcinoma in a High-Risk Patient.

BACKGROUND Sessile serrated lesions (SSLs) are precursors in approximately 20-30% of colorectal cancer (CRC) cases, often characterized by B-Raf proto-oncogene (BRAF) mutations and CpG island methylator phenotype (CIMP), and follow the serrated neoplasia pathway. Histopathological differentiation between SSLs with dysplasia and conventional adenomas is diagnostically challenging because their visual and pathologic features often overlap. SSLs with dysplasia carry a high malignant potential and can progress rapidly to invasive carcinoma, underscoring the need for accurate classification and tim

Assessment of toxicity in patients with ultra-central thoracic tumours treated with stereotactic body radiotherapy with a dose of 50 Gy in 5 fractions.

BACKGROUND: Stereotactic body radiotherapy (SBRT) is a well-accepted treatment for metastatic and primary lung cancer; however, an optimal regimen is still unclear for ultra central thoracic lesions. The objective of this manuscript is to report the toxicity of SBRT in patients with ultra-central tumors treated with 50 Gy in 5 fractions. MATERIALS AND METHODS: We performed a retrospective review of patients with ultra-central lung lesions treated with SBRT in our institution at the dose of 50 Gy in 5 fractions, delivered every other day. Lesions were defined as ultra-central when the planning

Safety and Efficacy of Stereotactic Magnetic Resonance-Guided Adaptive Radiation Therapy (SMART) for Ultracentral Metastases in Non-Small Cell Lung Cancer.

PURPOSE: Stereotactic ablative radiation therapy (SABR) is a standard of care for early-stage lung cancer and thoracic oligometastatic or oligoprogressive disease. However, ultracentral lesions remain challenging because of their proximity to critical mediastinal structures and the associated risk of severe toxicity. Stereotactic magnetic resonance-guided adaptive radiation therapy (SMART) allows for daily plan adaptation and real-time tracking in breath-hold, enhancing target coverage while improving sparing of adjacent organs compared to conventional SABR. METHODS AND MATERIALS: This retros

Alimentary mycosis in sloths.

Alimentary mycosis is seldom reported in sloths. Through a multi-institutional retrospective study, we described the histological features of fungal infections within the digestive tract of sloths of the Bradypus and Choloepus genera. In addition, panfungal polymerase chain reaction (PCR) targeting the ITS-2 gene was performed in all cases to determine a specific etiology. We retrieved 11 cases of alimentary mycosis in 3 sloth species: Bradypus variegatus (n = 1), Choloepus hoffmanni (n = 7), and Choloepus didactylus (n = 3). Eight were free-ranging, whereas 3 were held in captivity. Nine were

Jejunal calcifying fibrous tumor: A jigsaw puzzle with a pool of diagnostic dilemma.

Calcifying fibrous tumor (CFT) is a very rare benign fibroblastic tumor featuring a wide anatomical distribution and mimicking various spindle cell tumors. CFT has been documented in soft tissues, gastrointestinal tract (GIT), neck, adrenal glands, and pleura. Within the GIT, it is reported in the stomach, small bowel, large intestine, stomach, esophagus, and appendix. The occurrence of CFTs in the gastrointestinal tract presents a diagnostic dilemma, firstly due to the rarity of the lesion and secondly, due to the occurrence of a variety of other stromal lesions in the gastrointestinal tract

Esophageal squamous cell carcinoma with pagetoid spread: a clinicopathologic study.

Pagetoid spread in esophageal squamous epithelium associated with underlying esophageal adenocarcinoma (EAC) has been well studied. Case reports describing pagetoid spread of esophageal squamous cell carcinomas (ESCC) also exist in the literature. The latter, however, has not been systematically studied. In this study, we report seven cases of pagetoid spread associated with ESCC. The clinical, morphologic, and immunophenotypic profiles of pagetoid spread in the context of ESCC and EAC are compared. Cases of pagetoid spread of ESCC were identified through computerized search of pathology archi

Prominent black esophagus, Wischnewsky spots, and black duodenum in a fatal hypothermia case with an underlying diabetic ketoacidosis.

Hypothermia can occur in patients with diabetic ketoacidosis (DKA), and these two conditions can exacerbate each other. Fatal hypothermia and DKA have overlapping features and findings such as Wischnewsky spots (WS), black esophagus, basal subnuclear vacuolization in the renal tubule, dehydration, and increased acetone levels. Therefore, it may be challenging to differentiate or clarify the context of these two conditions. Herein, we report a case of a 49-year-old man with type 1 diabetes who was found lying in his house in mid-winter. He experienced cardiopulmonary arrest 10 h after the initi

Comparison of radiation exposure from dual- and single-energy CT imaging protocols resulting in equivalent contrast-to-noise ratio of lesions for adults and children: a phantom study.

OBJECTIVES: To compare the radiation exposure from single-energy CT (SECT) against rapid kV-switching dual-energy CT (DECT) imaging in both adults and children when resulting image data offer equivalent lesion identification power. MATERIALS AND METHODS: Lesions in an adult and a 10-year-old-child body phantom were imitated using iodine solutions of different concentrations. Phantoms were subjected to several SECT and DECT thoracic and abdominal scans using a rapid kV-switching DECT scanner. The contrast-to-noise ratio (CNR) of each lesion was measured on resulting SECT images and virtual mon

A Review of Emerging Biomarkers for Immune Checkpoint Inhibitors in Tumors of the Gastrointestinal Tract.

In recent years, immune checkpoint inhibition (ICI) therapy has made a tremendous improvement in the treatment of malignant tumors of gastrointestinal tract, especially for those with metastatic or recurrent lesions. However, while some patients benefit from ICI, others do not. In fact, predictive biomarkers can play a crucial role in screening patients who may benefit from a selected or targeted treatment, including immunotherapies such as programmed death-1/programmed death-1 ligand 1 (PD-1/PD-L1) inhibitors. A variety of techniques can be used to detect and quantify tumor biomarkers, each o
