Target Trial Emulation of Alcohol Intake Interventions in Young Adults and Risk of CVD over 30 years: Data from the CARDIA study.

AIM: This study uses a target trial emulation framework to estimate how different hypothetical sustained alcohol intake patterns affect 30-year cardiovascular disease risk in young adults. METHODS: This target trial emulation used longitudinal data from the CARDIA study, a multi-site cohort of Black and White young adults aged 18-30 at baseline (1985-1986), followed for 35 years. The exposure of this study consisted of self-reported daily alcohol intake categorized by sex into abstainer, light, moderate, heavy, and very heavy use categories. The emulated intervention compared hypothetical sce

Outcomes of Patients With High-Risk Plaques Treated With a Bioresorbable Magnesium Scaffold: Insights From the BIOMAG-I Study.

BACKGROUND: Early identification and targeted treatment of coronary lesions at higher risk of future events remains a major unmet need in coronary artery disease research. Invasive treatment of such plaques is controversial. The third-generation bioresorbable magnesium scaffold (DREAMS 3G) may provide a valuable alternative, but it has not been evaluated in this setting. The BIOMAG-I study, with comprehensive OCT and IVUS imaging, offers a unique opportunity to investigate high-risk plaque morphology and its vascular response. AIMS: We aimed to assess the association between lesions with high

The Effect of Prior Proton Pump Inhibitor Use on Gastric Cancer Survival: A Population-Based Cohort Study in the 5 Nordic Countries.

INTRODUCTION: Proton pump inhibitors (PPIs) might mask gastric cancer symptoms, leading to delayed diagnosis and worse oncological outcomes. However, current evidence is sparse and underpowered. This study aimed to elucidate whether PPI use influences oncological outcomes of gastric cancer in a well-powered study. METHODS: This population-based cohort study included individuals with gastric non-cardia adenocarcinoma from the 5 Nordic nations. Multiple nationwide registries provided prospectively collected data. The individuals were grouped into non-users, light users, or heavy PPI users durin

Global patterns of upper gastrointestinal cancer incidence by subtype and subsite in 2022: a population-based study.

OBJECTIVE: To provide an updated global assessment of esophageal and gastric cancers incidence by histological subtype and anatomical subsite. METHODS: We utilized data from GLOBOCAN 2022 and Cancer Incidence in Five Continents Vol. XII (2013-2017) to estimate the incidence of esophageal cancer subtypes (squamous cell carcinoma [ESCC] and adenocarcinoma [EAC]) and gastric cancer subsites (cardia [CGC] and non-cardia [NCGC]). Age-standardized incidence rates (ASIRs) were calculated by sex for each country, world region, and Human Development Index (HDI) category. Spearman correlation analysis

Incidentally Detected Ovarian Teratoma on Fluorodeoxyglucose (FDG) PET/CT Scanning in a Patient With Gastric Cancer.

This report aims to present the imaging findings of an ovarian teratoma incidentally detected during fluorodeoxyglucose (FDG) positron emission tomography combined with computed tomography (PET/CT) examination performed for the staging of gastric carcinoma, and to discuss these findings in the context of the current literature. A 53-year-old woman with a recently confirmed diagnosis of gastric carcinoma via endoscopic biopsy was referred for PET/CT staging. Imaging revealed pathological FDG uptake along the gastric wall from the cardia to the lesser curvature, consistent with the primary tumor

Artificial intelligence electrocardiography for left ventricular systolic dysfunction demonstrates preserved performance across demographic training imbalances.

AIMS: Artificial intelligence (AI)-enabled electrocardiograms (AI-ECG) can detect left ventricular systolic dysfunction (LVSD), but demographic imbalance in training datasets may introduce bias. Foundational models, pretrained on large and diverse datasets, may mitigate such concerns. We aimed to assess the impact of demographic composition in training datasets on the performance of an ECG Foundational Model (ECGFM) for diagnosing LVSD. METHODS AND RESULTS: We developed an ECG foundational model (ECGFM) using transformer architecture and self-supervised pretraining on 983 200 ECGs. Using 44 8

From Prediction to Horizon: Clinicians' Negotiations of AI-Driven Personal Prognoses in Clinical Practice.

Computational methods and tools under the label 'artificial intelligence' (AI) are increasingly promoted as solutions to the challenges of under-resourced and understaffed healthcare systems, with predictive modelling positioned as a means to improve efficiency and individualise care. Yet little is known about how predictive algorithms are taken up in the everyday practices of clinical decision-making. Drawing on ethnographic fieldwork with Danish cardiologists working with the CARDIAIHD algorithm, this article examines how AI-driven personal prognoses intersect with clinical reasoning and org

Metabolic pathways associated with cardiometabolic risk effects on cognition in middle-aged adults: the CARDIA study.

BACKGROUND: Evidence increasingly suggests a connection between cardiovascular disease and brain health in later life; however, the mechanistic pathways from human studies remain unclear. This study aimed to investigate whether urinary metabolites account for part of the association between cognition and cardiometabolic risk. METHODS: Data from 606 participants (aged 48-60; 55% female; 45.5% Black/African American) in the Year 30 follow-up of the Coronary Artery Risk Development in Young Adults Study were analyzed. Urinary metabolites were profiled using nuclear magnetic resonance spectroscop

Association between aortic wall thrombus and stroke among patients undergoing transfemoral aortic valve replacement.

BACKGROUND: Stroke is a major adverse event following transfemoral transcatheter aortic valve replacement (TAVR), often attributed to embolization of debris from the aortic valve or aorta. This study evaluated the relationship between severe aortic wall thrombus (AWT) on pre-TAVR imaging and peri-procedural stroke in a single-center cohort. METHODS: Patients with peri-procedural stroke after transfemoral TAVR (January 2013-December 2023) were compared with a randomly selected stroke-free control group. Severe AWT on baseline CT was assessed using a validated segmental scoring system. Associat

Real-World Outcomes of Dedicated Leaflet Modification for Bioprosthetic Valve-In-Valve TAVR: A 150-Patient Commercial Experience.

BACKGROUND: Coronary obstruction during valve-in-valve (ViV) transcatheter aortic valve replacement (TAVR) is infrequent but catastrophic. Mechanical splitting of bioprosthetic leaflets may mitigate this risk, but real-world experience remains limited. OBJECTIVES: The aim of this is to evaluate procedural performance and in-hospital outcomes of a dedicated mechanical leaflet-splitting device in ViV TAVR. METHODS: Consecutive postapproval cases were performed at 28 centers from February to November 2025. Patients were selected for elevated risk based on preprocedural computed tomography crite

Neoplastic transformation of sporadic gastric hyperplastic polyps: a systematic review and meta-analysis of risk factors and clinicopathological features.

This systematic review with meta-analysis aims to analyse the existing literature on clinicopathological features of sporadic gastric hyperplastic polyps (GHP), with special emphasis on risk factors associated with neoplastic transformation, as well as available immunohistochemical and molecular data relevant to GHP carcinogenesis. We searched two electronic databases and included studies reporting the presence of dysplasia and adenocarcinoma arising in GHP. Meta-analysis of odds ratios (ORs) was performed using random-effects models. We included 58 studies, 11 of which were included in quanti

Endoscopic combined esophageal semi-tunnel and gastric exposed resection for a large transesophagogastric leiomyoma: a case report.

BACKGROUND: Resection of large subepithelial tumors at the gastroesophageal junction remains technically demanding because of the narrow operating space, the need to preserve junctional function, and the risk of postoperative reflux or stenosis. CASE PRESENTATION: A 36-year-old man was referred after a gastric cardia lesion was detected during routine endoscopy at an outside hospital. Contrast-enhanced CT with axial, coronal, and sagittal reconstructions showed a well-defined 30 × 18 mm lesion in the cardia-gastric fundus region. Endoscopy, however, revealed a subepithelial lesion extending f

Serial OCT-based coronary physiology and plaque composition in vessels with nonobstructive coronary lesions following intensive lipid-lowering therapy: YELLOW III sub-study.

BACKGROUND AND AIMS: Non-obstructive coronary plaques are a major source of future coronary events, particularly when characterized by high lipid burden and adverse plaque morphology. While intensive lipid-lowering therapy promotes plaque regression, serial data linking quantitative plaque compositional changes to coronary physiology are limited. We aimed to evaluate the impact of maximal lipid-lowering therapy on coronary plaque composition and coronary physiology, using artificial intelligence (AI) based tissue characterization derived from OCT and optical flow ratio (OFR). METHODS: This wa
