A Systematic Review: Role of Pyloric Dilation in the Management of Gastroparesis.

BACKGROUND: Gastroparesis (GP) is a motility disorder characterized by pronounced delayed gastric emptying and associated with symptoms. Endoscopic pyloric dilation (PyD) is a minimally invasive option targeting pyloric dysfunction, though efficacy remains variably reported. AIMS: This systematic review assessed clinical outcomes and safety of PyD in adults and children with GP not associated with surgical manipulation. METHODS: PubMed, Embase, and Scopus were searched through December 2025 for studies evaluating PyD in GP. Post-surgical and non-GP etiologies were excluded. Twelve studies (1

Generalized Megaesophagus Concurrent With Toxocariasis in a Cat.

Feline megaesophagus is a rare disorder characterized by esophageal hypomotility and dilation, resulting in regurgitation, weight loss, and occasionally death. This report describes the clinical and paraclinical findings of generalized megaesophagus with a possible contribution of toxocariasis in a kitten. A 2-month-old female cat was referred with chief complaints of lethargy, hyporexia, dyspnea, and vomiting/regurgitation. Physical examination of the kitten revealed bradycardia, bradypnea, hypothermia, dehydration, and lung crackles. The most notable laboratory results were leukopenia with s

Gastric peroral endoscopic pyloromyotomy for delayed gastric conduit emptying: outcomes and predictors of treatment response.

BACKGROUND: Delayed gastric conduit emptying (DGCE) is common in patients following esophagectomy, and its treatment remains challenging. This study evaluated efficacy and safety of gastric peroral endoscopic pyloromyotomy (GPOEM) and predictors of treatment response. METHODS: We prospectively included patients with symptoms of DGCE following esophagectomy scheduled for GPOEM in two tertiary referral centers. EndoFLIP measurements were conducted before and 3 months after GPOEM. Gastric emptying scintigraphy (GES) was performed prior to, 3 and 12 months after GPOEM. Efficacy was evaluated by c

Successful Endoscopic Management of Bouveret Syndrome by Transpyloric Stone Relocation and Intragastric Mechanical Lithotripsy.

Background: Bouveret syndrome is a rare form of gallstone ileus in which a gallstone migrates through a bilioenteric fistula and impacts at the gastric outlet or proximal duodenum. It affects predominantly frail elderly women, in whom an endoscopy-first strategy is desirable; however, complete endoscopic clearance is achieved in fewer than half of cases and becomes markedly less likely as stone size increases, meaning that large stones often prompt surgery. Case Presentation: A 95-year-old woman presented with four days of vomiting and obstipation. Computed tomography demonstrated gastric outl

Outcomes of open Ramstedt's pyoloromyotomy among infantile hypertrophic pyloric stenosis patients at governmental hospitals in Amhara Region, Ethiopia, 2023.

BACKGROUND: Infantile hypertrophic pyloric stenosis is characterized by enlarged pyloric musculature and gastric outlet obstruction. In developing countries, it results in a high rate of malnutrition, dehydration, electrolyte and acid-base imbalance and associated mortality. Ramstedt's pyloromyotomy is the commonest procedure used to treat Infantile hypertrophic pyloric stenosis in Ethiopia, particularly in the study area. However, treatment outcomes of the procedure were not well addressed in Ethiopia, particularly at the study area. OBJECTIVE: To assess outcomes of open Ramstedt's pyloromyo

A novel variant of pancreaticobiliary maljunction: Complete opening of the common bile duct into the duct of Santorini in a patient with pancreas divisum.

Pancreaticobiliary maljunction (PBM) is a rare congenital anomaly in which the pancreatic and bile ducts unite outside the duodenal wall, forming an abnormally long common channel that permits reciprocal reflux of pancreatic juice and bile. This leads to a high risk of recurrent pancreatitis, cholangitis, biliary stone formation, and biliary tract malignancy. Type D PBM, particularly when associated with complete pancreas divisum and aberrant bile duct drainage, represents an exceptionally rare and complex variant. We report the case of a 20-year-old female with a long-standing history of recu

Prophylactic Pyloric Dilatation and its Impact on Early Delayed Gastric Conduit Emptying and Pneumonia Post-esophagectomy: A Multi-centre Cohort Study.

BACKGROUND: Early delayed gastric conduit emptying (DGCE) and pneumonia are among the most common complications following oesophagectomy. These issues can be challenging to address during minimally invasive surgery with pyloroplasty or pyloromyotomy. This study evaluated the impact of prophylactic endoscopic pyloric balloon dilatation (PBD) on early postoperative outcomes. METHODS: Consecutive patients undergoing oesophagectomy with gastric conduit reconstruction for cancer at two Australian tertiary centres (2017-2025) were retrospectively analysed. From 2023, intraoperative endoscopic-assis

A Rare Benign Mimic of Gastrointestinal Stromal Tumor: Gastric Plexiform Fibromyxoma Presenting With Iron-Deficiency Anemia and a Hypervascular Ulcerated Pyloric Mass.

Gastric lesions with suspicious radiological and endoscopic features can remain diagnostically challenging when initial tissue sampling is benign or non-diagnostic. This case highlights the importance of correlating endoscopic appearance, cross-sectional imaging, endoscopic ultrasound (EUS) findings, histology, and multidisciplinary assessment when investigating a deeper gastric lesion. A 28-year-old man was investigated for iron-deficiency anemia associated with intermittent abdominal pain and altered bowel habit. Laboratory investigations demonstrated microcytic iron-deficiency anemia, with

Rapunzel Syndrome as a Complication of Childhood-Onset Trichotillomania.

INTRODUCTION: Rapunzel syndrome is a rare and potentially life-threatening complication of trichotillomania with trichophagia, characterized by the extension of a gastric trichobezoar into the small intestine. Diagnosis is frequently delayed due to nonspecific presenting symptoms, and management requires both surgical intervention and psychiatric treatment to prevent recurrence. PATIENT CASE: We report the case of a 22-year-old woman with childhood-onset trichotillomania and trichophagia who presented with nonspecific abdominal complaints. Imaging revealed a large intragastric mass, and endos

Revisional surgery for delayed gastric conduit emptying after esophagectomy: the RESTORANT project.

Delayed gastric conduit emptying (DGCE) is a debilitating functional complication after esophagectomy. Because of a lack of large-scale outcome data, this project evaluated surgical revisional strategies for refractory DGCE within a recently published mechanism-informed classification system. REvisional Surgery for delayed gasTRic conduit emptying (RESTORANT) was a multicentric, retrospective cohort study including adult patients undergoing revisional surgery for DGCE from January 1, 2015, to December 31, 2024, across 26 high-volume centers that performed 11,807 esophagectomies during this per

The neuroscience of transformers.

Neuroscience has long informed the development of artificial neural networks, but the success of modern architectures invites, in turn, the converse: can modern networks teach us lessons about brain function? Here, we examine the structure of the cortical column and propose that the transformer provides a natural computational analogy for multiple elements of cortical microcircuit organization. Rather than claiming a literal implementation of transformer equations in cortex, we develop a hypothetical mapping between transformer operations and laminar cortical features, using the analogy as an

Certification or cash prize: the heterogeneous effect of venture competitions.

Cancer cells expressing constitutively active phosphatidylinositol-3 kinase (PI3K) are proliferative regardless of the absence of insulin, and they form dietary restriction (DR)-resistant tumors in vivo. Because the binding of insulin to its receptors activates the PI3K/AKT/mammalian target of rapamycin (mTOR) signaling cascade, activating mutations in the PIK3CA oncogene may determine tumor response to DR-like pharmacological strategies targeting the insulin and mTOR pathways. The anti-diabetic drug metformin is a stereotypical DR mimetic that exerts its anti-cancer activity through a dual me

Novel Gastric Helicobacter Species in Stranded Pygmy Sperm Whales (Kogia breviceps) on the East Coast of Florida, USA.

Gastric spirilliform bacteria consistent with Helicobacter spp. were identified in four pygmy sperm whales (Kogia breviceps) that stranded along the east coast of Florida, USA, during 2017-20. Six of nine (67%) stomach samples from PCR-positive cases contained spirilliform bacteria visible on histopathologic examination. Experimental PCR targeting Helicobacter-specific 16S rRNA gene sequences produced amplification in at least one gastric tissue sample (e.g., forestomach, fundus, pylorus) from all four cases. Three different novel Helicobacter sequences were identified in 13 samples from four

[Research development and controversial topics of function-preserving surgery for early gastric cancer].

With the advancement of the times and technology, the proportion of early gastric cancer has been increasing year by year, and its treatment methods have shifted towards more minimally invasive approaches, with better function-preserving and postoperative quality of life. Function-preserving surgery has gradually become the mainstream surgical option for early gastric cancer. Function-preserving surgeries mainly include segmental gastrectomy, pylorus-preserving gastrectomy, local gastrectomy, and sentinel lymph node navigation surgery. Although significant progress has been made in various fun

Expression of Ki-67 in Histopathologically Diagnosed Gastric Carcinoma Patients.

Gastric Carcinoma, often known as Gastric Cancer, is the second leading cause of cancer-related deaths globally. Gastric cancer symptoms are virtually non-existent and by the time they appear, the disease has most likely spread. There is an urgent need to develop techniques for detecting stomach cancer that do not rely exclusively on histological findings. The Ki-67 labeling index analysis can show the tumor's aggressiveness. The investigation was carried out to assess the utility of Ki-67 as a tool for detecting stomach cancer. The aim of the study was to observe the expression of the Ki-67 e

Pylorus Endoluminal Functional Luminal Imaging Probe Measurements in Children Are Determined by Age and Are Distinctly Different From Adult Values.

BACKGROUND/AIMS: : The pylorus muscles are known to play a vital role in the process of gastric emptying and are directly implicated in symptom associated with delayed gastric drainage. Endoluminal functional luminal imaging probe (EndoFLIP) can predict clinical response to pylorus directed therapy in symptomatic children and adults. Normal pylorus EndoFLIP values are available for adult but not in children. We aim to assess the impact of age on pediatric pyloric EndoFLIP values. METHODS: : Retrospective review of all children who underwent pylorus EndoFLIP in Evelina London Children's Hospit

Pylorus Resecting Pancreatoduodenectomy With or Without Feeding Jejunostomy-A Randomized Controlled Trial.

INTRODUCTION: Postoperative nutrition after pancreatoduodenectomy (PD) remains controversial. Although many centers routinely place a feeding jejunostomy tube (FJT) after PD, it is associated with morbidity. We conducted this study to compare the perioperative outcomes with and without FJT placement post-PD. METHODS: This was an open-label randomized controlled trial, in which the FJT was placed in one arm and the nasojejunal tube (NJT) in the other. All patients with periampullary neoplasm who underwent pylorus resecting PD were included in this study. The primary outcome assessed was clinic
