Clinical response to gastric peroral endoscopic myotomy and its association to changes in pyloric impedance planimetry measurements: a systematic review and meta-analysis.

BACKGROUND AND AIMS: Gastric peroral endoscopic myotomy (G-POEM) has emerged as a potential therapy for patients with medically refractory gastroparesis. Although adequate patient selection is crucial, there is a paucity of data on factors that may help select patients and predict a favorable response to G-POEM. METHODS: We performed a systematic review and meta-analysis to evaluate pyloric functional lumen imaging probe (FLIP) changes in patients undergoing G-POEM. Pre- and post-G-POEM data were collected and analyzed. Clinical response was defined as an improvement of at least 1 point in th

Does minimally invasive pylorus-preserving pancreaticoduodenectomy (MI-PPPD) triumph over open pylorus-preserving pancreaticoduodenectomy (O-PPPD)? A systematic review and meta-analysis.

BACKGROUND: PPPD is purported to be better than the Whipple procedure as it preserves full gastric capacity. Approaching this surgery using the MIS technique [minimally invasive pylorus-preserving pancreaticoduodenectomy (MI-PPPD)] may confer additional advantages compared to the open method. However, literature comparing MI-PPPD to open pylorus-preserving pancreaticoduodenectomy (O-PPPD) is scarce. This study seeks to review the efficacy and long-term outcomes of MI-PPPD against O-PPPD. MATERIALS AND METHODS: This PRISMA-adherent review involved a systematic search of PubMed, Scopus, Cochran

Incidence and risk factors for symptomatic gallstone disease after gastrectomy for gastric cancer: a nationwide population-based study.

BACKGROUND: Gallstone formation is a potential long-term complication of gastrectomies. However, data on symptomatic gallstone disease after gastrectomy for gastric cancer are limited. This nationwide population-based study aimed to determine the incidence and risk factors of symptomatic gallstone disease requiring invasive intervention. METHOD: This nationwide cohort study was based on claims data from the Korean National Health Insurance Service Database. The study included 90 456 patients who underwent gastrectomy for gastric cancer between 2007 and 2020 after excluding individuals with pr

Early gastric cancer with malignant invasion into the duodenum: A case report.

RATIONALE: Well-differentiated early gastric cancer (EGC) typically exhibits limited invasive potential, and duodenal involvement is exceedingly rare. This report presents a unique case of a well-differentiated early gastric cancer with extensive duodenal invasion. Magnifying endoscopy is critical for elucidating the growth and infiltration patterns of such lesions. Furthermore, this study provides a detailed magnified endoscopic comparison between the gastric and duodenal mucosa. PATIENT CONCERNS: A 62-year-old female patient presented with 20 days of epigastric discomfort, but no nausea, vo

A novel nomogram to predict length of stay in patients undergoing pylorus-preserving pancreaticoduodenectomy: A retrospective study.

Prolonged length of stay (LOS) following pylorus-preserving pancreaticoduodenectomy (PPPD) is associated with increased healthcare costs and patient morbidity. Accurate preoperative prediction of LOS remains a clinical challenge. This study aimed to develop and validate a novel nomogram for predicting LOS in patients undergoing PPPD. A retrospective cohort of 809 patients who underwent PPPD was analyzed. The dataset was randomly divided into training (n = 243) and validation (n = 566) sets. Univariate and multivariate analyses were performed to identify independent predictors of LOS. A nomogra

Primary epithelioid angiosarcoma of the jejunum presenting with severe anemia and distant metastasis: a case report and literature review.

Angiosarcoma (AS), constituting approximately 2% of all soft tissue sarcomas, is an aggressive endothelial malignancy. We report on the case of a 72-year-old man admitted with persistent melena and severe anemia, ultimately diagnosed with jejunal angiosarcoma (JAS) and associated lung and bone metastases. Temporary stabilization of his hemoglobin level was achieved via fasting, hemostasis, and blood transfusion. The initial plain CT identified multifocal solid lung nodules, but failed to reveal a primary digestive system lesion. Subsequent small enteroscopy localized a submucosal bulge in the

Role of interstitial cells of Cajal in regulating tone and responses to enteric motor neurons in the murine pyloric sphincter.

Gastric slow waves fail to propagate through the pyloric sphincter (PS), thus isolating the specialized motility patterns of the stomach and small intestine. We investigated the role of interstitial cells of Cajal (ICC) in PS of mice. Ca2+ waves in ICC, events responsible for electrical slow waves, propagated along the gastric wall but failed to propagate into the PS. ICC in PS fired localized Ca2+ transients and displayed low expression of voltage-dependent Ca2+ conductances. These are properties of intramuscular ICC (ICC-IM) that cannot regenerate and propagate slow waves. A T-type Ca2+ chan

Development of an in situ small intestinal injection technique for targeted macromolecule delivery and in vivo functional studies in mice.

BACKGROUND: Targeted delivery of biological macromolecules to the small intestine remains challenging due to their susceptibility to degradation in the hostile gastric environment. METHODS: This study introduces a minimally invasive, in situ injection technique for the murine small intestine that facilitates localized luminal delivery while circumventing gastric barriers. The procedure involves a small abdominal incision for direct injection into the duodenum near the pylorus. Postsurgical monitoring of physiological parameters, systemic inflammatory markers, liver function, and intestinal in

Gastric Outlet Obstruction Caused by Adhesion Between Remnant Greater Omentum and Lesser Omentum After Laparoscopic Transverse Colectomy.

Postoperative adhesive small bowel obstruction is common after abdominal surgery, whereas gastric outlet obstruction (GOO) due to adhesions on the anterior surface of the stomach is extremely rare. We report an 81-year-old man who developed persistent vomiting shortly after laparoscopic transverse colectomy. Computed tomography revealed marked gastric dilatation with a beak sign at the pylorus, and upper gastrointestinal endoscopy showed stenosis from the lower gastric body to the gastric angle without intraluminal lesions. Emergency laparoscopy identified a band-like adhesion between the less

A Successfully Treated Case of Pediatric Traumatic Gastric Rupture Caused by Seat Belt Injury.

INTRODUCTION: Gastrointestinal tract injury is estimated to occur in 0.9%-1% of blunt trauma cases, with gastric rupture occurring in approximately 3% of these cases. Blunt trauma injuries of the stomach are rare. In comparison to solid organ injuries, gastrointestinal tract injuries are less likely to be accompanied by hemorrhaging that can cause hemorrhagic shock, and the process is considered to be relatively slow. CASE PRESENTATION: The patient was a 9-year-old girl who was involved in a passenger-seat traffic accident and transferred to the emergency room of her previous hospital. She wa

The combined effects of temperature and salinity on the stomatogastric nervous system of two species of decapod crustacea.

Nervous systems must maintain coordinated activity patterns despite perturbations from unstable environmental factors (e.g., temperature, pH, and salinity). Marine organisms experience daily and seasonal changes in temperature while precipitation and ocean currents alter sea water salinity. We examined how the stomatogastric nervous systems (STNSs) from two species of decapod crustaceans (Cancer borealis and Homarus americanus) were affected by altered temperature and salinity. We used the patterned activity of the pyloric rhythm to assay the tolerance of the STNS to these two perturbations, i

Real-time imaging-guided visualization facilitates post-pyloric feeding tube placement in critically ill patients: a prospective randomized controlled study.

BACKGROUND: Early post-pyloric feeding is critical for improving outcomes in critically ill patients. While guided feeding tube placement has demonstrated superior efficacy to blind insertion, its clinical application remains constrained by technical and operational challenges. The integrated real-time imaging system (IRIS) addresses these limitations by combining continuous lumen visualization with device portability, potentially democratizing precise feeding tube placement. This study evaluated whether IRIS-assisted placement achieved higher efficacy while maintaining equivalent safety compa

Positioning of Diaphragm Electrical Activity Catheter in Premature Infants.

BACKGROUND: Neurally-adjusted ventilatory assist (NAVA) is a mode of ventilation that uses the electrical activity of the diaphragm (EAdi) to synchronize and to control delivered pressure. EAdi is measured with a naso-/orogastric feeding tube, containing an array of sensors. Proper placement of the EAdi catheter is critical for optimal synchronization and support. Positioning the catheter requires (1) predicting the insertion distance and (2) verification using a dedicated positioning window on the ventilator, based on electrocardiogram (ECG) and EAdi signals. In preterm infants, no study has

Spatio-Temporal Characterization of Gastric Distensibility in Upper Endoscopy Identifies the Presence of Helicobacter pylori.

Assessment of gastric distensibility is crucial for understanding various gastrointestinal disorders, particularly those affecting gastric motility and reflux conditions. Gastric distensibility can be altered by pathological conditions such as linitis plastica or gastric mucosal inflammation, as seen in Helicobacter pylori infections. Although distensibility is evaluated during routine endoscopy, the assessment remains purely visual. Moreover, some specialized endoscopic tools assess only specific gastric regions and are often unavailable. Artificial intelligence tools may used to estimate the
