Adenine di-Phosphate (ADP) is an important physiological agonist that plays a vital role in normal hemostasis and thrombosis. The importance of ADP in normal hemostasis is clearly demonstrated in patients suffering from storage pool disease who show excessive bleeding tendencies. It is well established that ADP activates platelets through 3 purinergic receptors, namely P2Y1, P2Y12 and P2X1. The P2Y1 receptor is a Galpha q coupled G-protein receptor that is important for platelet shape change, aggregation, thromboxane A2 generation, procoagulant activity, adhesion to immobilized fibrinogen and

BACKGROUND: Preoperative management of Graves' disease (GD) typically includes antithyroid drugs, β-blockers, and short-term iodine. However, intensified therapy may result in excessive elevation of thyroid-stimulating hormone (TSH). Whether a TSH threshold (≥10.0 μIU/mL) can determine greater operative difficulty in such patients is unclear. Therefore, we assessed this association in patients with GD who underwent total thyroidectomy.
METHODS: This retrospective cohort study included consecutive patients with GD who underwent total thyroidectomy at a single center between 2009 and 2022 (n =

BACKGROUND: Over recent decades, variations have been observed in the incidence of pediatric hyperthyroidism (HT) and Graves' disease (GD), preferred treatment modalities, and long-term outcomes. This study aimed to evaluate trends and long-term outcomes of pediatric HT across a 20-year period in a large cohort.
METHODS: We conducted a retrospective study using the electronic database of a large health maintenance organization. Children aged 2-18 years who were diagnosed with HT between 2000 and 2019, received antithyroid drugs (ATDs) for at least 6 months, and were followed up through 2024 w

BACKGROUND: The optimal duration of antithyroid drug (ATD) therapy in Graves' disease is uncertain. Although guidelines recommend 12-18 months, shorter courses may be sufficient in selected patients. We evaluated whether ATD duration <12 months is non-inferior to 12-18 months for relapse among patients with Graves' disease and lower thyrotropin receptor antibody (TRAb) levels (>1.8 and <10.0 U/L) at diagnosis.
METHODS: We analyzed real-world data from adults with Graves' disease and baseline TRAb <10 U/L treated with ATDs at a single center. The primary endpoint was relapse within 12 months o

Clinically significant liver enzyme elevation is a recognized adverse effect of antithyroid drugs (ATDs) and often necessitates treatment discontinuation and switching to an alternative agent. Using TriNetX, we conducted a retrospective cohort study of patients with hyperthyroidism who initiated methimazole/carbimazole (MMI) or propylthiouracil (PTU), developed significant liver enzyme elevation within 4 months, and switched to the alternate ATD within 12 months. Baseline characteristics were indexed to the initial prescription, and outcomes were assessed for 120 days following the second pres

Thyroid peroxidase (TPO) is a crucial enzyme in thyroid hormone (TH) biosynthesis, catalyzing iodination and coupling reactions for triiodothyronine (T3) and thyroxine (T4) production. In addition to the known enzymatic inhibition mediated by some compounds, increasing evidence indicates that various drugs can modulate TPO mRNA and/or protein expression (e.g., protein kinase activators, cytokines, epigenetic drugs). This review comprehensively explores current evidence on the pharmacological regulation of TPO expression, encompassing its molecular structure, enzymatic function, and involvement

INTRODUCTION: Ultrasound-guided thermal ablation (U-GTA) techniques, including radiofrequency ablation (RFA) and microwave ablation (MWA), have recently been proposed as minimally invasive alternatives to surgery or radioiodine (RAI) therapy for patients with Graves' disease (GD) who are unresponsive to antithyroid drugs. However, evidence supporting its effectiveness and safety remains limited.
METHODS: A PubMed/MEDLINE, Web of Science, and Scopus research updated until January 31, 2026, was performed. The review question was: What is the euthyroidism rate (outcome) one year after U-GTA (int

Graves' disease, the most common cause of hyperthyroidism, is a chronic autoimmune disorder that can lead to serious health problems. Treatment options for Graves' hyperthyroidism have not changed in several decades and include antithyroid drugs (ATDs), radioactive iodine (RAI), and thyroidectomy. Few literature reviews comprehensively evaluating the burden of Graves' disease, including epidemiological, clinical, humanistic, and economic outcomes, have been published since the American Thyroid Association and European Thyroid Association treatment guidelines for Graves' disease were developed

CONTEXT: Severe thyrotoxicosis and thyroid storm have a high morbidity and mortality risk and are commonly managed with antithyroid drugs. In some patients, a combination therapy with medications and thyroidectomy as definite treatment are required. Therapeutic Plasma Exchange (TPE) can be used as a temporizing measure to achieve thyroid hormone normalization and to prevent perioperative complications before surgery.
OBJECTIVE: We describe 6 cases of severe thyrotoxicosis and/or thyroid storm who underwent TPE as a bridge to thyroidectomy and present the clinical characteristics and outcomes.

Treatment of Graves' disease (GD) is complex and controversial. Conventional therapy of GD includes antithyroid drugs (ATDs), surgery, and radio-iodine therapy (RIT) and each has limitations. ATDs are associated with high rates of relapse and side effects. Definitive therapies result in permanent hypothyroidism and increased risk of Graves' ophthalmopathy. With advancements in the understanding of precise autoimmune mechanisms causing GD, novel therapies targeting immune pathways are in the pipeline of development. Newer drugs switch off a part of the immune system that is causing the disease,

Thyroid storm is a rare but life-threatening endocrine emergency that requires rapid recognition and multidisciplinary care. We report the case of a 16-year-old female patient admitted to the emergency department with acute confusion, fever, and tachycardia, in whom severe thyrotoxicosis was confirmed. Her Burch-Wartofsky score was 40, consistent with an impending thyroid storm. Graves' disease had been previously diagnosed, but treatment was delayed, and influenza B infection acted as a precipitating factor. Management with antithyroid drugs, beta-blockers, and intensive care led to clinical

Graves' disease (GD) is the most common cause of hyperthyroidism, characterized by autoimmune stimulation of the thyroid gland and typically managed with antithyroid drugs (ATDs), radioiodine (RAI) therapy, or surgery. While most patients respond to standard antithyroid therapy, a minority exhibit refractory thyrotoxicosis, posing significant clinical challenges. Pregnancy further complicates management due to immunological shifts, increased risk of adverse maternal and fetal outcomes, and limitations in therapeutic options. We present the case of a 39-year-old woman with severe, persistent Gr

BACKGROUND: Graves’ hyperthyroidism is caused by stimulatory autoantibodies. Its diagnosis and monitoring are commonly based on measurement of thyrotropin receptor antibodies (TRAb) with unspecific immunoassays, that also detect neutral and blocking antibodies. TRAb analyzed using the Siemens IMMULITE® 2000 TSI immunoassay (TRAb-IM), designed to target stimulatory immunoglobulins, represents a promising alternative. This study aimed to determine the clinical performance of TRAb-IM and the Thermo Fisher BRAHMS TRAK KRYPTOR immunoassay (TRAb-KR) in a real-world setting. METHODS: Over 3 months, T

Hyperthyroidism is a prevalent endocrine disorder that can lead to severe multisystem complications, including atrial fibrillation (AF), thyrotoxic periodic paralysis (TPP), and thyroid storm (TS). This review discusses the pathophysiology, clinical presentation, and management challenges of these complications through illustrative clinical cases. AF is the most common cardiovascular manifestation, with restoration of euthyroidism and rate control being central to treatment. TPP presents with transient muscle weakness due to hypokalemia from intracellular potassium shift, requiring cautious po

Amiodarone-induced thyrotoxicosis (AIT) is an uncommon but potentially severe adverse effect that may significantly compromise arrhythmic control. Thyroid dysfunction induced by amiodarone can precipitate or perpetuate atrial fibrillation, particularly in patients with underlying structural heart disease. We report the case of a 54-year-old woman with non-obstructive hypertrophic cardiomyopathy, heart failure with preserved ejection fraction, and paroxysmal atrial fibrillation, who developed recurrent episodes of atrial fibrillation with rapid ventricular response refractory to conventional ra
