Early prediction of insulin requirement in gestational diabetes using a parsimonious LASSO model and a point-based risk score.

AIMS: This study developed and validated a practical prediction model to forecast insulin requirements in pregnant women with gestational diabetes mellitus (GDM). METHODS: A retrospective single-center cohort of 490 consecutive women with GDM was analyzed. The primary outcome was insulin therapy initiation based on routine clinical decisions. Candidate predictors were collected at the first visit. LASSO logistic regression with 10-fold cross-validation was used for model development, excluding underweight women (BMI < 18.5 kg/m2) and incomplete cases. Coefficients were converted into a point

Transcutaneous allicin oleogel restores energy homeostasis for obesity treatment.

Allicin, a bioactive compound derived from garlic, exhibits therapeutic potential against metabolic disorders but is hindered by instability and a pungent odor, limiting its clinical application. This study develops an allicin oleogel (OG) formulation, leveraging molecular interactions with unsaturated fatty acids and gelators to stabilize allicin and mitigate its odor for transcutaneous delivery. The skin analysis confirmed superior transdermal delivery and accumulation in subcutaneous fat with OG, highlighting its advantages for topical application. In a high-fat diet (HFD)-induced mouse mod

Inpatient safety, effectiveness of SGLT2 inhibitors and GLP-1 RAs in type 2 diabetes: ENDOCARE, a pragmatic prospective cohort study.

BACKGROUND: Current guidelines recommend baseline insulin as the standard treatment for most people hospitalised with type 2 diabetes (PWT2D). However, they generally discourage the use of sodium–glucose cotransporter 2 (SGLT2) inhibitors and glucagon-like peptide-1 (GLP-1) receptor agonists due to safety concerns and a lack of randomised evidence. We aim to assess primarily safety, and secondarily, effectiveness of non-insulin glucose-lowering therapy for inpatients, as implemented within a structured start-stop clinical practice guideline. METHODS: We conducted a 9-month pragmatic prospectiv

Sugar-sweetened beverages and acute metabolic crises in type 2 diabetes mellitus: a report of two cases.

Diabetic ketoacidosis (DKA) is an uncommon but serious complication in patients with type 2 diabetes mellitus (T2DM). Excessive intake of sugar-sweetened beverages (SSBs) may represent an underrecognized trigger for DKA in individuals with T2DM. We present two male patients with T2DM who developed ketosis following excessive consumption of SSBs (aerated drinks). The first patient, aged 33 years, reported consuming 2.5 litre/day of SSBs and developed ketosis without acidosis. The second, aged 18 years, reported daily intake of 4 litre/day of SSBs and presented with frank DKA. Both patients were

In vitro and in silico evaluation of copper nanoparticle-phytoconjugates as multi-target antidiabetic agents.

Type 2 diabetes mellitus (T2DM) is characterized by elevated blood glucose levels and insulin resistance. Trigonella foenum-graecum (fenugreek) has been extensively reported for its hypoglycemic and insulin-sensitizing properties mediated through multiple biochemical pathways. This study aimed to synthesize copper nanoparticles (CuNPs) using fenugreek seed extract and the investigation of their antidiabetic activity through an integrated in vitro and in silico approach. Qualitative phytochemical analysis confirmed the presence of secondary metabolites. The in vitro enzyme inhibition assays rev

Sex differences in exogenous glucose oxidation are attenuated by obesity: insights from a [U-13C6]glucose home breath test.

Biological sex influences whole body glucose metabolism, with females generally displaying greater insulin sensitivity than males, yet paradoxically similar or poorer glucose tolerance when assessed with oral glucose tolerance tests (OGTTs). This discrepancy suggests that plasma glucose concentrations alone may not capture sex-specific differences in postprandial glucose metabolism. Measuring exogenous glucose oxidation during a [U-13C6]glucose-enriched OGTT may offer a noninvasive approach to detect such differences, but this method has not yet been applied to examine sex differences. Artificial Intelligence-Guided Optimization of Hyaluronic Acid-Coated Liposomal Linagliptin for Targeted Management of Polycystic Ovary Syndrome.

Linagliptin, a DPP-4 inhibitor commonly used in the management of diabetes mellitus, has shown potential activity in polycystic ovary syndrome (PCOS). Linagliptin's therapeutic effectiveness is limited by its poor membrane permeability and low oral bioavailability. This study aimed to formulate hyaluronic acid-coated liposomal linagliptin optimized through I-optimal design and AI-based entrapment efficiency (EE%) prediction. The effects of hyaluronic acid and drug concentrations on particle size (PS), polydispersity index (PDI), zeta potential (ZP), and EE% were systematically evaluated to dev

Efficacy of continuous glucose monitoring in comparison to self-blood glucose monitoring in patients with type 2 diabetes mellitus under insulin treatment: a systematic review and meta-analysis.

BACKGROUND: Diabetes Mellitus (DM) affects approximately 10.5% of the global population, necessitating effective glucose monitoring to mitigate complications. Self-monitored blood glucose (SMBG) lacks dynamic glycemic profiling, while continuous glucose monitoring (CGM) provides real-time/intermittent data. This review compares CGM and SMBG in insulin-treated type 2 diabetes (T2DM) patients. METHOD: A systematic review and meta-analysis of randomized controlled trials (RCTs) from PubMed, Web of Science, and Scopus was conducted until May 2025 and included RCTs compared CGM with SMBG in terms o

Chronic Glycaemic Control Modulates the Relationship Between GIP and Glucagon Secretion Following Oral and Enteral Nutrients in Type 2 Diabetes.

AIMS: Glucose-dependent insulinotropic polypeptide (GIP) may exert both insulinotropic and glucagonotropic effects. While its insulinotropic action is reportedly attenuated or abolished with worsening of glycaemic control in type 2 diabetes (T2D), the relationship between GIP and glucagon secretion across different levels of glycaemic control remains unclear. This study evaluated the relationship between postprandial GIP and glucagon responses to a mixed meal in T2D and to intraduodenal fat and glucose infusion in healthy and/or T2D individuals. MATERIALS AND METHODS: Data were analysed from

Novel Small Molecule GLP-1R Agonists Based on 1H-Benzo[d]imidazole-5-Carboxylic Acid Scaffold.

Glucagon-like peptide-1 (GLP-1) is an incretin hormone secreted by intestinal endocrine L cells that activates the GLP-1 receptor (GLP-1R), leading to glucose-dependent insulin secretion and suppression of glucagon release. In recent years, GLP-1R agonists (GLP-1RAs) have become one of the leading therapeutic options for the treatment of type 2 diabetes mellitus; however, for a long time clinically approved GLP-1RAs were limited to peptide drugs unsuitable for oral administration. The discovery of the "first-in-class" small molecule agonist danuglipron in 2018 demonstrated the feasibility of o

Appetite Perception and Cerebral Blood Flow in Aging Adults Following a Single Bout of Exercise.

Insulin acts in the brain to promote satiety. Aging individuals may have brain insulin resistance and altered appetite perceptions. However, it is unclear if exercise impacts cerebral reward centers and appetite perception in middle-aged to older individuals. Purpose: To assess whether a single exercise bout alters cerebral blood flow (CBF) in reward centers in relation to appetite perceptions. Methods: Fifteen sedentary adults (12F; ~56 ± 2y; ~31 ± 1 kg/m2) completed a control and acute exercise condition (70% maximal oxygen consumption) in a randomized, counterbalanced order in the evening.
